[Doppler examination in temporal arteritis. Preliminary results (author's transl)].

Nine patients with histologically confirmed Horton's diseases were explored by Doppler ultrasound examination, before and after corticotherapy, of the cervical arteries, temporal arteries (N = 9), and branches of the ophthalmic artery: internal nasal (N = 9), central retinal, and supraorbital (N = 6). Excellent correlations existed between histological findings and results of Doppler examination, concerning the severity of the lesions (80% correlation). Circulatory slowing was evident in the ophthalmic vessels studied, with alterations in the shape of peak velocities in 6 of the 9 cases. Major abnormalities were present in 3 patients; two of them presenting transient or permanent amaurosis. Doppler curves improved after corticotherapy, their shape and amplitude becoming equivalent to those of a control population of the same age. Doppler examinations appear to be of diagnostic value in Horton's disease, as well as being able to predict the risk of ophthalmological complications; more effective surveillance of corticotherapy is possible.

[Severe ventricular arrhythmia following parenteral administration of vincamine. Predisposing factors in 6 cases].

Six patients developed ventricular arrhythmias with parenteral administration of vincamine. Direct intravenous injection was the mode of administration in 2 cases, intravenous infusion in 3 cases (one at very high dosage) and intramuscular injection in 1 case. The same signs of toxicity were observed in al patients:--5 patients had recorded attacks of "torsades de pointe", which recurred in 1 of them when the drug was restarted.--1 patient had syncope and, although an ECG was not recorded at the time, an ECG shortly afterwards showed a long QT interval and R/T ventricular extrasystoles. Symptoms were generally neurological in nature with syncope, cyanosis, and convulsions. Spontaneous regression was observed in 3 cases but in the others the drug had to be stopped and cardiopulmonary resuscitation instituted. None of our patients died of their arrhythmia. In some patients a predisposing factor was found:--metabolic: hypokalaemia (1 case), moderate reduction of potassium pool (1 case), severe reduction of calcium pool (1 case);--pharmacological: previous treatment with fenoxidil (1 case), thioridazine (1 case);--cardiac: congenital long QT interval (Romano-Ward) (1 case), revealed by vincamine administration. Chronic obstructive airways disease with right ventricular strain and atrial fibrillation (1 case) which might have predisposed the patient to "torsades de pointes". Three patients had no predisposing factors apart from their age. "Torsades de pointes" occurred in a pacemaker patient, but pacemaker function was normal. These six cases may be grouped with the other ten or so cases of vincamine toxicity already reported; they carry and additional warning on the use of intramuscular vincamine. Vincamine toxicity is probably a direct effect on the myocardial cells. This fact merits verification by further electrophysiological studies.

[Death in Horton disease. Prognostic factors].

The authors reported causes of death and searched for prognosis factors in Giant Cell Arteritis (GCA). The diagnosis was confirmed by temporal biopsy in all cases. Fourteen patients died during treatment; thirty-six patients had completely recovered (follow up > 6 months after withdrawal of steroid therapy). The commonest causes of death were cardiovascular (n = 7) and digestive (n = 4); they occurred after an average of 195 days of treatment, half of them during the first three months. One death was due to GCA (autopsy) and five deaths were attributed to the treatment with corticosteroids. The prognosis factors were searched for by comparing age, sex, clinical signs, laboratory data before treatment, past medical history in the both series; further more initial dose of Prednisone and the dose after 180 days of steroid therapy were compared in the two groups. The adverse prognosis factors revealed by this study were: advanced age (p < 0.01), previous ischaemic heart disease (p < 0.05) and higher dose of corticosteroids administered at 6 months of treatment (< 0.01).

[Idiopathic cyclic edema in Klinefelter syndrome (author's transl)].

After a minor abdominal traumatism, M. B., 53 years of age, presents a sudden and acute edematous syndrome. Cardiac, renal, hepatic, nutritional and thyroid etiologies ares rapidly eliminated. A cavography and lymphography reveal the integrity of the drainage pathways. The idiopatic cyclic edematous syndrome is therefore evoked by few clinical details (diurnal weight gain, diurnal oliguria...). Exploration of the renin - angiotensin - aldosterone system, the Landis test and the marked radioactive serum albumin test attest to the exaggeration of capillary permeability. Likewise, it was discovered in this patient, a Klinefelter syndrome which was, until now, unknown. This case poses interesting pathogenic problems since the idiopathic cyclic edematous syndrome is a predominantly feminine disease. Only a few cases were described in the masculine sex and, to our knowledge, this syndrome has never been associated with Klinefelter syndrome.

[Skin fibrosis in hyperthyroidism treated by sotalol and radioactive iodine (author's transl)].

The authors present detailed data about skin fibrosis appearing in hyperthyroidism treated by Sotalol and radioactive iodine. Cutaneous thickening is discovered quite rapidly when the patient is monitored daily (as in case 4). It is asymptomatic and no other features of scleroderma are found. Regression occurs within 4-10 months. Histologically, fibrosis is located in the entire dermis. Dermal appendages are normal and no inflammatory changes occur. No anomalies of collagen structure and fibroblasts have been observed ultrastructurally. Immunological studies (direct immunofluorescence of the skin, lymphocyte transformation and leucocyte migration tests with Sotalol) were normal. The mechanism is unknown, but an immunological or a toxic one is excluded; however a pharmacological action is possible. The role of other betablockers must be assessed by a randomised study.
