[Indications and results of surgical removal of "congenital" epimacular membranes].

INTRODUCTION: Congenital epimacular membranes differ from acquired membranes of young adults by their slow evolution and absence of vitreously their cortex detachment. CLINICAL CASES: We report five observations. In all these cases of unilateral maculopathy, no etiologic factors were found. Surgical removal was required in only two of these cases. RESULTS: These membranes were whitish and presented extensions along retinal vessals to which they were very adhesive. The absence of posterior vitreous detachment was confirmed during vitrectomy and histopathologic examination found a predominance of extracellular matrix. DISCUSSION: In these idiopathic membranes, clinical and histopathological findings evoke a dysembryopathy. Surgery decision must attach great importance to the relative amblyopia of these eyes, the low evolutivity and the risks of iatrogenic tears during dissection.

[Tuberous sclerosis of Bourneville: contribution of the ophthalmologist in 2 cases].

Bourneville's tuberous sclerosis. Report on two cases. The tuberous sclerosis is a most rare congenital disease belonging to the phacomatoses with a non-predominant neural crest participation. The ophthalmologist's diagnostic does have a real importance because the fundus can show phacomas which are one of the most specific indications of this affection, and therefore it does help for the genetic advice. After the report of two tuberous sclerosis cases showing phacomas, we insist on the ophthalmologic signs and on the principal manifestations of this disease. The main problem to solve will be how to set up the differential diagnostic with a first appearance of a retinoblastoma. Most of the time, the evolution will be a slow increase of the phacomas' number and of their size as well. Sometimes it might be complicated by vitreous hemorrhages which may lead to a vitrectomy. The vital prognostic will be make out according to the general damages.

[Malignant hyperthermia in ophthalmologic surgery].

Malignant hyperthermia is a potentially fatal complication of general anesthesia that may occur with greater frequency in ptosis and strabismus surgery. The case of a two-year-old girl who suffered a malignant hyperthermia crisis during strabismus surgery is reported. The pathophysiology, clinical features, treatment and pre-anesthetic diagnosis are reviewed in detail.

[The Walker-Warburg syndrome. A new case].

Walker-Warburg syndrome is a fatal cerebro-ocular malformation. It appears to be of autosomal recessive inheritance, but materno-fetal infection cannot be excluded as a possible etiology. Antenatal diagnosis consists in screening for fetal hydrocephalus. The authors report one personal case of this syndrome and a review of 34 previous observations in the literature.

[Meningeal hemorrhage and Hermansky-Pudlak syndrome].

We studied the case of a young patient affected by a Hermansky-Pudlak syndrome: oculocutaneous albinism of variable intensity with essentially an haemorrhagic diathesis due to a "pool vide" thrombopathy. In beginning the only obvious symptom was a nystagmus and an ocular albinism. Cerebrospinal hemorrhage has up to now never been reported to our knowledge. Healthy carriers can be detected by ophthalmological examination and hematological coagulation tests. Albinos must benefit of systematical coagulation tests in order to prevent drug induced haemorrhagic accident.
