Blood flow through irradiated rabbit submandibular glands measured with fluorescein angiography.

Microvascular injury has been regarded as a contributing factor in irradiation damage of biologic tissues, including morphologic changes and increased permeability of the vascular network. The late irradiation effect on the capillary circulation of salivary glands was studied with dynamic fluorescein angiography. In 31 rabbits one side of the head was exposed to a single radiation dose of 15 Gy. At 4 and 10 months thereafter no significant difference was detectable in the microcirculation between the unshielded and shielded glands.

Minimally invasive and non-invasive diagnosis of carotid artery obstructive disease: an evaluation of supraorbital fluorescein angiography and ultrasonic duplex technique.

The carotid arteries of twenty-five patients with transient ischaemic attacks, minor stroke or amaurosis fugax were examined by conventional angiography, the ultrasonic duplex technique and supraorbital fluorescein angiography (SOFA). The results of the last two techniques (non-invasive and minimally invasive, respectively) were compared with the results of angiography. We found that the ultrasonic duplex technique is highly sensitive and specific for both high and low-grade carotid stenoses while the less costly SOFA can only identify stenoses equal to or more than 75%. However, since tight stenosis has proved to be an indication for surgical reconstruction, SOFA - or possibly the still simpler procedure with direct ocular inspection of the supraorbital fluorescence pattern without any recording - would be useful for initial screening in smaller units without access to ultrasonic duplex examination. In this way potential candidates for surgery can be identified without delay and forwarded to larger units for further supplementary examinations.

A phase III trial of zoladex and flutamide versus orchiectomy in the treatment of patients with advanced carcinoma of the prostate.

In a multicenter Phase III trial 264 patients with advanced prostatic cancer were randomized to either bilateral orchiectomy or treatment with zoladex supplemented by flutamide. Presently, median follow-up time is 30 months. A small difference in objective response was recorded in favor of the combination therapy, whereas no statistically significant difference was found in subjective response to therapy, time to progression, and overall survival. Adverse effects were more commonly encountered in the pharmacologically treated patients. It is concluded that the combination of zoladex plus flutamide is not clinically superior to orchiectomy in the treatment of patients with advanced carcinoma of the prostate.

[Continuous monitoring of wound infections and other postoperative complications using MIKRO-ORT].

Registration of 883 operations, performed on the Orthopedic Department, Roskilde County Hospital, from 06.20.1988 to 12.31.1988, is presented. 95.9% of all operations were registrated primarily. 2.6% superficial wound infections and 1.2% deep were registered. Other infections constituted 2.3%, other complications 4.5%. A significant increase in the incidence of deep wound infections in contaminated wounds was observed. We also found an increase in incidence in prolonged operations (NS). Where types of infections and complications were concerned pronounced extension of the hospitalization occurred. The 4.1% of operations not registered primarily, were more often performed in the evening and night time, were more often contaminated operations and were more often patients with more than one operation during the registration period. No significant loss of information could be found. It is concluded, that establishing a continuous surveillance of wound infections has benefited to the department's control of its infection rate. We were satisfied with the program MIKRO-ORT, which we have used for the registration.

Secondary treatment of advanced cancer of the prostate with Zoladex.

Twelve patients with advanced prostatic carcinoma and relapse following previous hormone manipulation therapy were treated with Zoladex-depot every 4 weeks. The treatment was well tolerated and the endocrine response was satisfactory. However, no clinically important improvement was obtained.

Heterotopic bone formation after noncemented total hip arthroplasty. Location of ectopic bone and the influence of postoperative antiinflammatory treatment.

The regional-development of heterotopic bone around the hip and the influence of postoperative prophylaxis with antiinflammatory drugs were studied in 66 patients treated with 74 noncemented, porous-coated total hip arthroplasties (THA) using the McFarland approach. Thirty patients (34 hips) were treated with antiinflammatory drugs during the early postoperative period. Heterotopic bone developed in two different regions: central, around the neck of the femoral component, and lateral to the greater trochanter of the femur. Central ossifications were recorded in 53% of the hips and appeared significantly less frequently in patients postoperatively treated with antiinflammatory drugs. Lateral ossifications were recorded in 71% of the hips, and no significant prophylactic effect was found for postoperative antiinflammatory treatment. In response to the McFarland approach in THA, heterotopic ossifications may appear both centrally around the neck of the femoral component and lateral to the trochanter in the substance of the medial gluteal muscle. The latter may be comparable to post-traumatic myositis ossificans.

[Laser treatment of penile cancer].

The treatment of localized penile cancer has hitherto been surgical amputation of the penis. In our 12 cases, tumorectomy was performed by CO2 or ND:YAG-laser radiation with minimal tissue ablation. Local recurrences occurred and the patients must be followed regularly in the outpatient clinic. The recurrences were treated by repeated laser radiation. No patients died of penile cancer during a mean observation of 16 months (range 3-50). Laser tumorectomy has definitive therapeutic advantages compared with penile amputation, because the result is acceptable for the patient and his micturition and sexual life are not altered.

Fluorescein angiography and distal arterial pressure in patients with arterial disease of the legs.

Fluorescein angiography (FA) of the foot soles was performed in 119 patients with arterial disease of the legs. Fluorescein was injected rapidly intravenously and sequential photographs were taken of the foot soles. Densitometric measurements were performed on three areas of each foot image: the big toe, the foot pad (just proximal to the little toe) and the heel. The relationships between different FA measurements and systolic arterial pressure in the ankle or the big toe were analysed. The appearance times of fluorescence correlated inversely with ankle pressure (P less than 0.001). The initial slopes of the fluorescence-time curves at all three sites of measurement correlated with ankle pressure (P less than 0.001). The initial slopes of fluorescence-time curves of the big toe and the foot pad correlated with toe pressure (P less than 0.001). In 12 patients effective arterial pressure was lowered by elevation of the feet, and in eight patients external pressure was applied to the foot by enclosing it in a box. The changes in FA evoked by these manoeuvres further strengthened the relationship between arterial pressures and FA measurements. We conclude that FA is a good method for evaluating circulation in the foot when neither ankle nor toe pressure is obtainable. In addition, FA may be useful when vascular disease is suspected in the presence of normal pressures, because the fluorescence distribution pattern was clearly abnormal in 11 of 16 such feet.

Intrathoracic microvascular circulation in haemorrhagically shocked rats. Studies by fluorescence techniques.

The microvascular circulation in the heart, lung and thymus was studied by fluorometry in 32 haemorrhagically shocked rats. Exsanguination to a blood pressure of 35 mmHg for 180 min did not result in any reduction of this circulation in the heart or lung, but in the thymus it was reduced by 52%. Retransfusion of shed blood caused no change in cardiac microcirculation, whereas the microcirculation in the thymus was increased by 59%. In the lungs a heterogeneous fluorescence pattern was observed after retransfusion, with large dark areas alternating with normal fluorescent ones and even some small intensively fluorescent areas. After retransfusion, maldistribution of blood flow seems to be a phenomenon of haemorrhagic shock in the lungs.

Decreased red cell filterability in patients with acute myocardial infarction.

The red cell filterability was decreased in patients with acute myocardial infarction (AMI) when compared with healthy controls, 14.6 (12.2-16.3) units and 16.9 (15.6-17.4) units respectively, P50 (P25-P75), p less than 0.001). No significant correlations could be seen within the AMI group between the decrease in filterability and the levels of serum aspartate aminotransferase or serum lactate dehydrogenase. The erythrocyte filterability, however, correlated to the serum concentrations of hepatic enzymes in AMI. The addition of sodium lactate in vitro in physiological concentrations (0.9-3.6 mM/l final concentration) lowered the erythrocyte filterability markedly to 2.7 (0-9.8) units in a dose-dependent manner, supporting the hypothesis that the decrease in erythrocyte filterability in AMI might be caused by an increase in the lactate concentration.

Flutamide versus stilboestrol in the management of advanced prostatic cancer. A controlled prospective study.

In a prospective randomised study the effect of flutamide 750 mg daily was compared with that of stilboestrol 3 mg daily in the treatment of 40 previously untreated patients with advanced prostatic cancer. There was a good subjective response to both treatments. After 12 months, a response was demonstrated in 13 of 20 patients treated with flutamide and 8 of 20 patients treated with stilboestrol. The difference was not statistically significant. Treatment with stilboestrol caused more frequent, and more severe, side effects than flutamide.

Epidermal growth factor in urine from patients with urinary bladder tumors.

Epidermal growth factor (EGF) concentration and 24-hour excretion in urine were measured with a radioimmunoassay in 18 patients previously treated for various types of urinary bladder tumors and a comparable control group of 18 normals. The median concentration of EGF in urine from the patients was 1.50 nmol/l and from normals 3.03 nmol/l. The median 24-hour excretion of EGF in urine from the patients was 1.96 nmol and from normals 3.33 nmol. These differences between patients and normals were statistically significant (p less than 0.05 and p less than 0.005) although there was an overlap in individual values of EGF concentration and excretion in urine from patients and normals. This study suggests that urothelial neoplasia is related to a low concentration and excretion of EGF.
