Vitamin B12 absorption in total gastrectomy. Study with (57Co) B12 and radioimmunoassay.

Vitamin B12 absorption and real amount of circulating cyanocobalamin were determined by Thurangh Schilling's test and radioimmunoassay respectively, in 28 patients undergone total gastrectomy. The methods are described and the results discussed, in relation to the parameters under consideration, that is: age, sex, pathology of lesion, time elapsed from surgery, and type of reconstruction of intestinal continuity.

Giant lymph node hyperplasia in angiohamartomatous soft tissues.

The first case of Castleman's benign lymphoma associated with angiohamartomatous alterations in peripheral fibroadipose tissue is reported, and a pathogenetic connection is hypothesized. The lesion was encountered in a 48-year-old man; it was situated in the left and inferior retroperitoneal area with extension to the inguinal region of the same side.

[Reflections on ten years' experience in the treatment of hemorrhagic colorectitis and Crohn's disease (author's transl)].

The authors offer a critical review of eight cases of hemorrhagic colorectitis and Crohn's disease (regional ileitis) managed with medical treatments, and eleven like cases treated surgically, during the last ten years. After a discussion of the resources and limitations of medical therapy, the authors describe the various surgical strategies adopted in their cases; they define the possibility of obtaining final cures in Crohn's disease with extended ileocolectomy; and concerning ulcerative forms, they point out the greater efficacy of surgical procedures involving sacrifice of the rectum compared to those in which the terminal segment of the gut is used immediately or later for restoring continuity of the intestinal canal.

[Personal experience with anterior resection of the rectum].

Authors experience in the Dixon's operation regarding the surgical treatment of cancer of the rectum and recto-sigmoid junction is reviewed. It consists in 224 cases of malignant lesions which were treated by Miles operation (106 cases--47%), Dixon's operation (49 cases--21%), palliation anterior resection (16 cases--7%), other operations (53 cases--24%). After a brief discussion on anatomo-pathological conditions and tecniques, the Authors consider their results: operatory mortality 4%; complications: total anastomosis dehiscence 1 case, partial anastomosis dehiscence 14%; local repetitions 10%; hepatic metastasis within 5 ys 40%; survival at 5 ys 46%. Functional results were always very good. The Authors conclude that, even considering the large rate (56%) of wide-spread cancers treated, Dixon's operation, at least in their experience, doesn't seem to satisfy those needs of radically requested by a correct surgical therapy, which only Miles' operation guarantees.

[Our criteria for surgical treatment of acute obstructions of the large gut of neoplastic origin (author's transl)].

The authors review a total of 132 cases of acute neoplastic obstruction of the large gut seen in their clinical material over the last twenty years. In the light of the results obtained they discuss the indications and limitations of the various surgical strategies adopted (radical surgery, derivative surgery, and palliative resection); they conclude that radical procedures are best for the majority of cases.

[Raynaud's disease and syndrome. Presentation of clinical experience].

The Authors report their experience concerning 256 cases of Raynaud's Disease and Syndrome observed from 1968 to 1987; 23 patients underwent surgery for the treatment of dystrophic or gangrenous lesions of the extremities (12 cervico-thoracic and 8 lumbar sympathectomies, 1 popliteal aneurysmectomy, 2 scalenotomies): a significant improvement of the clinical picture was observed in 91.30% of the patients who underwent the operation. The pharmacologic treatment obtained satisfactory results in 51.26% of all the patients. The Authors propose a three stage classification of raynaudian symptoms: I stage: sporadic vasoconstrictive crises; II stage: recurrent vasoconstrictive crises not invalidating working activity and "restitutio ad integrum" between attacks; III stage: recurrent and frequent crises invalidating working activity with asphyxia and/or ulcer-gangrene of the extremities. Both pharmacological and surgical treatment obtained the improvement of the clinical condition to an inferior stage in a high rate of patients. In three patients only (two of whom underwent scalenotomy and one popliteal aneurysmectomy) the complete and definitive cessation of raynaudian crises was observed.

[Natural history of synchronous hepatic metastases from a non-treated colorectal cancer].

The authors present a retrospective study concerning 135 cases of untreated synchronous metastases from colorectal cancer. The median survival is 5.5 months and the natural history is depending on the percentage of liver replacement, on the "performance status" of each patient and on the stage of primary tumors classified according to Dukes. The seric levels of Alkaline Phosphatase and CEA are other useful prognostic factors. The age of patients and systemic chemotherapy (5-Fluorouracil) are not able to influence the evolution of the disease. At last different techniques, both curative and palliative, used in the current treatment of liver metastases from colorectal cancer are discussed and the reported survival improvements reaching with these forms of treatment are compared to the natural history of the disease.

[Inguinal hernia in children].

The authors report 611 cases of inguinal hernias of childhood operated between 1967 and 1985, which represent 28% of operations performed in all children in the same period. The mean age of patients was 33 +/- 4.1 months and the ratio M:F = 5.43:1. The right side was the most frequently affected (64.64%) with a bilaterality of 14.89%. The incarceration, observed at the rate of 13.23%, needed the surgical treatment in urgency in the 35.63% of the cases. A relapsing hernia appeared only two times (0.45%) and the incidence of contralateral metacronous hernias was 7.99%. They perform herniorrhaphy in the only symptomatic side without preoperative herniorrhaphy and operate on the silent contralateral side only when there a hernia will became symptomatic, as they think that herniorrhaphy and bilateral inguinal exploration cause a high number of unnecessary, sometimes harmful operations.
