[Profile of the user and tolerance of main contraceptive methods in primary care: hormonal contraceptives, intrauterine devices (IUD) and diaphragm].

OBJECTIVE: To analyse the psycho-social profile of women using hormonal contraceptives (HCC), the intrauterine device (IUD) and the diaphragm; and to evaluate their tolerance to these methods. DESIGN: A prospective observation study. SETTING: The basic unit of family planning at an urban health centre. PATIENTS AND OTHER PARTICIPANTS: 503 patients, for whom contraception was provided between january and may 1992, with a clinical follow-up throughout the year. MEASUREMENTS AND MAIN RESULTS: HCC were administered to 285 women, with an average age of 24.8# 4.38 years. Secondary effects were detected in 14.73% of cases, with good metabolic tolerance. The IUD was provided to 216 users, with an average age of 35.4#-6.39. There was acceptable tolerance, with it being withdrawn in 18 cases, mainly due to spontaneous shedding. There were only two diaphragm users during the study period; and these had good tolerance. CONCLUSIONS: HCC were the contraceptive method chosen most often by younger women. The high tolerance to these methods and easy out-patient control of them make Primary Care, in coordination with other professionals, the ideal level for their prescription. [Do the patients medicate themselves correctly?].

OBJECTIVE: To find the main pathologies which give rise to self-medication, the most commonly used pharmacological groups, how these drugs are obtained and the correctness of their instructions. DESIGN: Observation study of a crossover type. SETTING: An urban Health Centre. PATIENTS AND OTHER PARTICIPANTS: 396 patients, who attended during the months of February and March 1993 with requests for medication or who were already taking something, were selected at random--95% Confidence Interval with 0.049 exactness. MEASUREMENTS AND MAIN RESULTS: 76% attended when they were already taking medication, a percentage which was significantly lower among elderly people (p < 0.001). The most common pharmacological groups were analgesics (32.6%), antibiotics (19.9%) and antiflu drugs (17.6%); and the most common pathologies were respiratory conditions with or without temperature (26.2% and 32.3% respectively). Self-medication was correct in 75.5% of cases. It was least correct for antibiotics and vitamins and in descriptions of asthenia, where, in turn, demand was more frequent than self-consumption (p < 0.001). The main reason for taking medication was its previous medical prescription (38.64%). CONCLUSIONS: Patients correctly use the commonest medicines used in primary care, especially if they have been prescribed before. This suggests the importance of daily health education for each prescription, especially for the most common pathologies.

[Rare manifestations of diabetic neuropathy].

Up to 50% of diabetic patients develop neuropathies during their lives. Distal symmetric polyneuropathy is the most frequent variety, but there are other forms that are not exceptional and must be considered for the differential diagnosis of these patients. In this paper, the authors present three clinical cases of rare types of diabetic neuropathy: oculomotor cranial neuropathy (Case 1), diabetic amyotrophia (Case 2) and mixoide type with proximal asymmetrical locomotor neuropathy and distal polyneuropathy (Case 3). Their most relevant clinical characteristics are described, along with their differential diagnosis, evolution and therapeutical alternatives, in order to contribute to a better management of these patients.
