Acquired factor X deficiency with associated defects in platelet aggregation. A response to corticosteroid therapy.

A 79 year old white women presented with a severe bleeding disorder. Evaluation revealed a prothrombin time of 27.6 seconds (control, 11 seconds) and an activated partial thromboplastin time of 61 seconds. Specific clotting factor assays showed an isolated deficiency of factor X ranging from 7 to 12 per cent on three determinations. Platelet aggregation and bleeding time were also abnormal in response to epinephrine and collagen. Factor X levels and platelet aggregation returned to normal and bleeding stopped after institution of corticosteroid therapy.

Eosinophilic gastroenteritis with obstruction. Immunological studies of seven patients.

We studied 7 patients with eosinophilic gastroenteritis presenting with obstruction of the gastrointestinal tract. Five had required surgical treatment for relief of obstructive symptoms or because of diagnostic uncertainty. Immunological studies, including serum immunoglobulins, serum complement, lymphocyte quantitation, and lymphocyte response to nonspecific mitogens revealed no striking abnormalities. None was clinically allergic or food sensitive, and serum IgE was normal in all. A review of the literature since 1970 suggests that the obstructive form of eosinophilic gastroenteritis accounts for about half of the reported cases. Allergy, especially hypersensitivity to food, is not required for the diagnosis.

Reiter's aortitis with pericardial fluid, heart block and neurologic manifestations.

A man who presented with Reiter's triad and keratoderma blenorrhagicum followed by cardiac manifestations is reported. He developed mild aortic insufficiency, A-V block and pericardial effusion. Painful ophthalmoplegia occurred 5 months later with exacerbation of the cardiac complications as corticosteroids were being tapered. Retreatment with high dose corticosteroids resulted in a remarkable remission of the neurologic complications and stabilization of his cardiac manifestations now maintained for 3 years.
