Treatment of "locally advanced" well-differentiated thyroid carcinomas.

PURPOSE: To analyse thyroid carcinomas having an extrathyroid extension in order to identify the principal prognostic factors and outline an effective therapeutic strategy. METHODS: We selected a sample of 160 patients suffering from locally advanced "well differentiated thyroid carcinoma (T4) who had undergone surgery at the Department of Surgery of University of Rome "La Sapienza". The sample was subdivided into three groups: T4, limited type I, and extensive type II, T4 microcarcinomas. RESULTS: We obtained excellent results with the T4 microcarcinomas, above all in patients under the age of 45, with a 94.5% survival rate, compared with 88% in patients aged over 45. In the extensive type II T4 carcinoma we obtained a survival rate of 29.4% in patients aged over 45 years. CONCLUSIONS: Age, combined with an aggressive histological variant (Sclerosing and tall-cell papillary carcinoma), is an important factor in prognosis. The radicality of surgical excision is considered an important prognostic factor, although the results reported in the literature are contradictory. Aggressive surgery can free from the disease a high percentage of patients over the age of 50 even with T4. We deem it fundamental to perform total thyroidectomy in all advanced cases of thyroid neoplasm and to extend neoplasm excision to the adjacent tissues, even involving justified surgical demolition.

[Sialomucins in the follow-up of patients surgically treated for colorectal cancer: preliminary results of a prospective study].

Preliminary results of a prospective study on sialomucin content found either at resection margins of colo-rectal cancer after curative surgery or in endoscopic biopsy specimens of colonic mucosa in thirteen patients selected for the follow up are reported. Though not statistically significant because of the small number of cases enrolled, preliminary data indicate that clinical application of differential mucin staining in colo-rectal neoplasms is potentially useful to predict subgroups of patients at risk for local recurrence as well as metachronous colo-rectal cancers.

[The biological effects of the CO2 laser studied in experimental intestinal resections].

An histological and E.M. study has been performed on rat's intestinal tract resection to evaluate CO2 laser effects. Necrotic effect of laser is evident up to 120 micron. While the intestinal tissue results normal at 480 micron from resection line. Muscular and fibrous components appear more resistant to damage while E.M. presents a new organization of its components forming a new compact and continue tissue. This new organization may be involved in the protection of the subepithelial spaces, and may have an important role in intestinal anastomosis scar process.

[The role of endoscopy in managing esophageal lesions caused by caustic fluids].

In the paper is described the chronological outline adopted from the authors about the endoscopic treatment in case of oesophageal caustications, both in the emergency and, respectively, after one week and after one month. It is emphasized the importance of an endoscopic recognition within 6/12 hours after the caustic ingestion, to prevent the immediate acute complications, the later acute complications and the scarring evolutions of the healing. Important parameter for a clinical evaluation is represented by the volontariety of ingestion to which is connected the quantity of ingested caustic and than the extension of oesophageal lesions.

[Arteriomegaly in the aorto-iliac-femoral area with or without associated aneurysm].

The authors present a group of 199 patients with arteriomegaly, an affection characterized by elongated and distended blood vessels of the arterial system, with or without accompanying aneurysms. Our study on this group of patients, drawn from a large arteriographic series of peripheral abdominal and lower limb arterial disorders, focuses on a comparison between atherosclerotic arteriopathy and arteriomegaly. Small tissue blocks were taken from the arterial wall of patients operated on for megadolichoarteries. Electron microscopic examination of such specimens revealed a specific alteration of the elastic component of the vessel wall. The authors believe that surgical treatment of this condition is indicated in order to prevent thromboembolic complications or aneurysmal rupture.

[Lymphadenectomy and nerve sparing technique in radical surgery of rectal cancer].

The extension that should be given to lateral lymphadenectomy in the surgical treatment of extraperitoneal rectal cancer has not yet been assessed because of the difficulty of realizing randomized prospective clinical trials. The theoretical advantage of an extended lateral lymphadenectomy is represented by an accurate staging of the tumour in patients undergoing surgery for possible curative resection; even if the high percentage (20 per cent) of pelvic node metastasis was already demonstrated, the prognostic impact of lateral lymphadenectomy was not proven until now. The results of the Literature demonstrated that an advanced rectal cancer can not be treated only by conventional surgery: radical surgery (with total mesorectal excision and pelvic lymphadenectomy) or, in alternative, a combined approach-chemo-radiotherapy+surgery-should-be applied in order to improve the 5-year survival and to reduce the percentage of local recurrence. With regard to surgery the main disadvantage for an extended lateral dissection is represented by the high incidence of urinary and sexual dysfunctions. The nerve-sparing technique, when combined to pelvic lymphadenectomy, allows preservation of autonomic innervation to the pelvis. The results of our experience demonstrated that nerve-sparing technique is an effective procedure that reduces the incidence of urinary and sexual dysfunctions when accurately performed.

[Method of biopsy and ultrastructural study of the parathyroid gland in patients with primary hyperparathyroidism].

The authors suggest a new method of biopsy of a seemingly normal human parathyroid gland during surgery for parathyroid adenoma. Such a method provides adequate surgical specimen to rule out a likely diffuse hyperplastic disease associated with the parathyroid adenoma, and for the ultrastructural study of the gland, without any risk of postoperative complication. In all patients, ultrastructural examination showed an abnormal cell population and signs of glandular atrophy as a consequence of the high level of serum calcium due to hormonal hyperfunction of the parathyroid adenoma.

[Carcinoma of the extrahepatic bile ducts].

In the last 5 years 25 patients (11 men, 14 women) with carcinoma of the extrahepatic biliary duct were studied. The most frequent localization of the carcinoma was the hepatic hilum. D.C.P. according to Whipple was possible only in one patient, while in 20 patients a palliative approach with external and internal biliary derivations was the choice. In the remaining 4 patients an explorative laparotomy with biopsy was performed. It is concluded that palliative resection allows for a better quality of life.

[Reconstruction of the defect in the parotid region].

The authors report their experience in the surgical management of a patient with an ulcerated tumor of the neck extending to the parotid region. The different types of reconstruction after radical surgery as well as the choice and reasons that have suggested the use of pectoralis major myocutaneous flap are discussed.
