[Indications and surgical results in asymptomatic stenosis of the carotid artery].

In 101 patients 114 asymptomatic carotid stenoses were treated operatively, complicated by one lesion of hypoglossic nerve and one temporary neurologic deficit. During the follow-up period of 6-89 months cardiovascular diseases were seen in 60% of the patients. There were 9 late deaths, 5 due to myocardial infarction and 2 due to contrahemispheric cerebral ischemia. TIA's or cerebral infarctions were not observed in the hemisphere corresponding to the operated carotid artery. Interdisciplinary patient selection, non-invasive diagnostics and selective shunt protection are considered to be essential risk reducing factors.

Bilateral occlusive carotid disease: simultaneous or staged operation?

In a series of 231 carotid operations in 202 patients, 8 patients were operated on bilaterally by staged (group A), and 15 patients by simultaneous procedures (group B). Additionally 4 patients had a combined external carotid desobliteration plus extra-intracranial arterial bypass (group C), 3 of them in a simultaneous operation. Patients of the groups A and C had an uneventful recovery throughout. An additional group A patient, however, died during the staging interval due to an ischemic cerebral infarction at the not-yet-operated side. In group B one patient died after myocardial reinfarction and 4 showed other complications: diffuse hematoma of the neck, temporary palsy of the hypoglossal nerve, temporary left brainstem deficit and respiratory failure. Nevertheless, it is concluded that simultaneous bilateral carotid reconstruction offers specific benefits i.e. no risky interval, no second anesthesia and a reduced hospitalization period, and can therefore be recommended in selected cases.

[Reconstruction of the aorta in mycotic aortic aneurysm--report of two cases of salmonella infection (author's transl)].

Two patients with infected aortic aneurysm underwent surgical treatment. When using autologous tissue in place of the infected aorta a recurrent aneurysm was observed four months later. Extraanatomical bypass of the infected area with prosthetic material and consecutive removal of all infected tissue seems to be the only successful management. This is confirmed by another patient who was treated successfully in this way. Prolonged antibiotic therapy after resection of infected aneurysms seems to be mandatory.

[Measurement of cardiac output by impedance cardiography. Experimental studies (author's transl)].

The correlation of impedance cardiography against electromagnetic flow measurement was studied in the pig. The conditions tested were normal circulation, hypo- und hypervolemia and acute pulmonary hypertension as well as the influence of respiration and electrodes location. Correct quantification of cardiac output was obtained only in normovolemic circulation during exspiration (r = 0,96, m = 1,15, b = 0,04).

[Frequency-optimal pacing in chronic pacemaker therapy].

Hemodynamic studies were performed in 15 pacemaker patients at rest and during exercise by thermodilution technique and impedance cardiography. Particularly in myocardial failure, individual optimal heart rates were observed, ranging from 90 to 120/min. Mean increase of cardiac output at optimal pacing rate was 1.31/min against stimulation at 70/min. Impedance cardiography showed high correlation to thermodilution technique.

[Threshold changes in chronic transvenous cardiac pacing].

Alterations in stimulation threshold in long term transvenous cardiac pacing were recorded by serial measurements in 33 patients with EM 169 "Vario" pacemakers and additionally in 286 patients at operative revisions up to 6 years post implantation. A rapid threshold increase within the first 4 weeks was followed by stabilization at 160% of initial value. After about 1 1/2 years a further continuous increase of threshold with an annual rate of 15% occured. Prognostic calculations in individual cases, however, seem to be difficult because of frequent and considerable variations.
