Capitate-lunate instability: recognition by manipulation under fluoroscopy.

Videotape fluoroscopy was used to diagnose a previously undescribed carpal dissociation, the capitate lunate instability pattern. In eight patients with midcarpal pain and clicking, the examiner simultaneously applied pressure to the scaphoid tuberosity while applying longitudinal traction and flexion to the wrist under fluoroscopic control. This maneuver revealed dorsal subluxation of the proximal carpal row and capitate lunate subluxation in each of the eight patients. Plain radiography and arthrography were not helpful in the diagnosis. All eight cases were managed conservatively. Videotape fluoroscopy is the best radiologic method of diagnosing capitate-lunate instability.

Posterior dislocation of the elbow. A simplified method of closed reduction.

Reduction of a posterior elbow dislocation can be accomplished by many methods and can require special positioning of the patient, trained assistants, and special equipment. A method that provides a simplified alternative is described. Leverage rather than forceful strength is the prerequisite. The maneuver is performed with the patient in the supine position, without any skilled assistant or special equipment. In the author's patient group the leverage technique has provided prompt and effective reduction without compromise to the neurovascular status of the limb.

Hydrogen peroxide as a topical hemostatic agent.

Many research efforts have been directed toward enhancing the bone-cement interface of total joint arthroplasties. Hemostatic agents have been popularized as effective means of retarding the development of potentially harmful debris interposition adjacent to, and blood lamination patterns within, the methylmethacrylate. The hemostatic effectiveness of hydrogen peroxide was tested on six metaphyseal bone beds in ten mongrel dogs: three sites were treated with hydrogen peroxide and three were saline-treated control sites. The hemostatic effect of the treatments was determined by comparing blood loss per unit area per minute before and after application of the test solutions. The results indicate that hydrogen peroxide is an effective hemostatic agent and reduces the amount of blood and debris trapped within the trabecular bone methylmethacrylate interface.

Intraoperative radiographs: a modified technique.

Multiple intraoperative radiographs are often required during pediatric orthopedic surgery. Cumbersome cassette draping protocols take time and provide for potential breaks in sterile technique. A double-cassette method is described that facilitates the taking of multiple intraoperative radiographs.

Ulnar nerve palsy following rotational osteotomy of congenital radioulnar synostosis.

Two successive cases of ulnar nerve palsy following rotational osteotomy through a congenital radioulnar synostosis are described. Entrapment of the ulnar nerve by the fascia connecting the two heads of the flexor carpi ulnaris was documented in one patient. Recommendations to avoid this complication include shortening of the forearm at the osteotomy site and the release of unyielding soft tissue restraints. Rotational osteotomy at the distal radial metaphysis may provide a safe, alternative approach in the surgical management of this difficult problem.

Eastern massasauga rattlesnake bites.

The Eastern massasauga rattlesnake is native to many Midwestern states. Although this rattlesnake is smaller in size than many other pit vipers, envenomation can still produce significant morbidity, particularly in children. Children bitten by Eastern massasauga rattlesnakes appear to do well with treatment programs consisting primarily of antivenom and blood component replacement.

Distal biceps brachii tendon avulsion. A simplified method of operative repair.

Rupture of the distal insertion of the biceps brachii may lead to severe functional impairment of the upper extremity on the basis of resultant weakness of elbow flexion and supination. When the diagnosis of a posttraumatic distal biceps tendon avulsion is made, a simple method of reattachment is possible and functionally warranted.

A three-phase analysis of the prevention of recreational softball injuries.

Recreational sports injuries are expensive to society. Prevention of such injuries must be a major public health goal. In a previous retrospective study, base sliding was found to be responsible for 71% of recreational softball injuries. Because most injuries occurred during rapid deceleration against stationary bases, quick-release (break-away) bases were evaluated as a means to modify this mechanism of injury. In a prospective study, 633 softball games were played on a break-away base fields and 627 games were played on stationary base fields. Forty-five sliding injuries occurred on the stationary base diamonds (1 injury for every 13.9 games) and only two sliding injuries occurred on the break-away fields (1 injury for every 316.5 games). The medical costs for injuries on the stationary base fields was 79 times greater than that on the break-away fields. In a 1035 game follow-up study performed on all fields equipped with break-away bases, two sliding injuries occurred (1 injury for every 517.5 games). Installing break-away bases in fields used by recreational leagues would achieve a significant reduction of serious softball injuries (98%) and, therefore, should be mandatory. Based on our findings, the Centers for Disease Control has estimated 1.7 million injuries would be prevented nationally per year, saving $2.0 billion per year nationally in acute medical care costs.

Contact injuries to the hand.

This article stresses that all contact injuries to the hand require careful documentation and close follow-up. The late manifestations of such an injury can be very significant if it leads to impairment of the power of the worker's grasp.

Intraoperative excision of metal prominences: a technical note.

In situ modification of metal rods can be difficult. High-speed cutting discs are effective in cutting metal prominences which cause soft tissue irritation and bony erosion. Water-soluble jelly can effectively trap the metal debris generated during this cutting process.
