Role of subtotal thyroidectomy in the management of the follicular neoplasm of the thyroid.

Clinically apparent thyroid nodules affect 4% to 7% of the population in North America. Malignant neoplasms of the thyroid comprise only about 1% of all cancers and are found in only 20% of patients undergoing thyroid surgery for suspected thyroid carcinoma. A follicular neoplasm of the thyroid is the most common histologic finding. During surgery, using frozen-section techniques, it is often difficult to distinguish between a benign and malignant lesion. The diagnosis may be deferred following intraoperative frozen-section examination of tissue or changed on evaluation of embedded tissue. In this series of 1224 thyroidectomies from 1976 to 1990, the pathology data from thyroidectomy surgical specimens submitted to tissue analysis were evaluated. It was found that subtotal thyroidectomy was effective in the management of the follicular neoplasm. This approach significantly reduced the morbidity and complications of patients undergoing thyroidectomy.

The carotid body tumor.

Six cases of carotid body tumor are the material for this report. The pathology and current thinking on physiology are reviewed. Diagnosis is made by angiography which must include both carotid systems. Ultrasound is the screening method of choice because it is safe, reliable, non-invasive, and inexpensive. Preoperative planning must consider the patient's medical condition and life expectancy as well as tumor size, blood supply, skull base erosion and condition of the carotid vessels. If surgery is undertaken, peri-adventitial dissection is advocated.

Surgical exploration of enlarged lymph nodes at the root of the neck.

Surgical exploration of presumed enlarged lymph nodes at the root of the neck may sometimes result in the exposure of neurogenic lesions. Careful exposure of the lesion should precede mobilization and excision to prevent unavoidable iatrogenic injury. Experiences with 45 neoplasms in this region are described.

Fine-needle aspiration biopsy of the thyroid gland: results of a five-year experience and discussion of its clinical limitations.

The results of a five-year experience with fine-needle thyroid aspiration cytology are reported to assess the value, accuracy and limitations of this biopsy procedure. The cytologic diagnoses on 1,465 thyroid aspirates are listed, and correlation of cytology with histology on 207 cases undergoing surgery is reported. The incidence of malignancy among the atypical and malignant cytology aspirates with histologic confirmed diagnosis was 65% (28/43) while a false negative rate of 11.5% (15/131) was found. Fine-needle aspiration biopsy is a safe and useful procedure with an acceptable accuracy rate that complements other investigations in the screening of patients for thyroid surgery.
