The doctor-patient relationship has undergone considerable changes over recent years. An increase in public discussion of real or supposed malpractice has led to an increasingly critical attitude in patients, who are no longer prepared to accept the doctor's instructions unquestioningly. Every doctor, surgeons in particular, should, therefore, strive to restore a satisfactory relationship with his patient, since this is an essential precondition for ensuring therapeutic success. The doctor is obliged to explain to his patients (or legal spokesman) the risks involved in therapy and give him adequate insight into his condition. Only then can the patient make an adequate decision as to acceptance or refusal of therapy, which is legally binding (written consent in the case of operations). Absolute priority must be given to uninhibited maximum efficiency of the doctor. Legal safety should be of only secondary importance.
1032 Interventions on geriatric patients beeing more than 70 years old between 1966 and 1975 demonstrate the significance of azotemia in the postoperativ stage of the disease appears as well as its dependence upon prehospital factors, type and localisation also on the extension and duration on the intervention in each single case. Since it is hardly possible to exert an influence on the preliminary stress due to the old age and on the dysfunction determined by involution of the parenchymatose organs important for the course of the postoperative stage, a particular emphasis should be laid upon the prophylactic aspekt to the purpose of an appropriate screening against vegetativ-visceral, renally effective shock reflexes (hepatorenal syndrome) and on the other, usually extra-renal factors (PH 203R, splanchnic block). Further possibilities: considerate elimination of pain and general anesthesia, purpuseful and quick operative technique, improvement of the apparatus and instruments, after-treatment specific for the given case so as one consisting of a prophylaxis against pneumonia through a rapid reactivation of the operated person. However, regardless to all the professional advance made in this field, the primary human approach to the aged patient and to his life expectancy should be made due allowance for.

Report on 179 operations on the biliary tract of patients over 70 years old. The significance of a preparation specific to the case, general anaesthesia and a quick technique of operation avoiding additional stress is emphasized. Special attention is devoted to postoperative treatment. The main complications in old age patients are still caused by postoperative azotemia due to the hepatorenal syndrome. The concentration of geriatric surgery at decentralized service units is suggested.
A ten-year survey is presented of the operative procedures carried out in the Department of Surgery at a regional hospital in a rural area. The requirements to be met in this type of hospital necessitate the inclusion of bordering disciplines; obstetrics and gynaecology, for examples, belonged to our domain until 1970. Teamwork by a well-trained staff and small technical modifications of surgical procedures can combine with the favourable psychological background in such a hospital and its more personal atmosphere on account of its relatively small size to enable optimum treatment. Endoscopy is frequently employed and assumes a very important role as diagnostic tool in the absence of more sophisticated appliances. Certain findings of apparent interest, as well as noteworthy complications and the causative factors involved are discussed and compared with the literature.
A description is given of the clinical symptoms, radiological features and pathological findings in this report of a large, pseudotumorous aberrant pancreas located in the stomach of a 52-year-old male patient. The presenting complication was a gastro-intestinal haemorrhage necessitating gastric resection. Questions pertaining to the clinical diagnosis are extensively discussed.

In the management and prognosis of severe accident cases there are a number of conditions that occur before hospitalization which cannot be influenced, these are: no immediate medical care by a physician, delay in sending the patient to hospital, long journey time and alcoholic damage to the liver and CNS. Often, therefore, laparotomy for massive haemorrhage after abdominal injuries has to be done on a severely shocked patient with a "zero" BP. Diagnostic criteria in the evaluation of such cases are: the clinical picture, shock index, paracentesis (trial abdominal drainage) with additional laparoscopic examination and IVP. In a series of 45 laparotomies there were 25 single organ injuries; 19 multiple organ injuries and 1 negative laparotomy due to misinterpretation of the trial drainage. 22 splenectomies demonstrate the type of injury occurring in a country area. The good results of treatment despite the difficulties mentioned can, to some degree, be attributed to the technical improvement in vascular clips for large vessels. The Tantalum vascular clip offers the advantage of simple application, sure positioning and immediate arrest of haemorrhage thus reducing the shock phase; additional effects are the permanent indication of the bleeding site, the marking of anastomoses and resections and there being no foreign body reaction (pure Tantalum).
