[Multicenter histiocytosis].

INTRODUCTION: Multicentric histiocytosis is a rare systemic disease with active episodes and prolonged periods of remission. Immunosuppressor treatment (alkylating agents) can be effective. CASE REPORT: A new case of multicentric histiocytosis was observed in a 38-year-old man who was successfully treated with cyclophosphamide at the dose of 100 mg/d. Treatment was withdrawn after several weeks due to drug-induced hepatitis and replaced with chlorambucil at the dose of 0.1 mg/kg/day for 6 months. No relapse has occurred after a follow-up of 14 months. DISCUSSION: This case is particularly interesting because of the exceptional nature of spontaneous haemarthrosis which was the inaugural sign of joint manifestations with mediastinal lymph nodes and initially isolated pruritus occurring before the typical skin manifestations. Different management protocols have been discussed with emphasis on the presence of acute cyclophosphamide induced hepatitis. This immunosuppressor is not usually hepatotoxic. CONCLUSION: This case demonstrates the systemic nature of the disease and emphasizes the beneficial effect of alkylating agents.

[Efficacy of propafenone in supraventricular arrhythmias].

We evaluated the results of intravenous administration of propafenone in 44 patients with rapid symptomatic supraventricular arrhythmia of less than 1 month duration. Mean age was 54 years and 22 were females. Propafenone, 2 mg/kg was given over 15 min and blood pressure and heart rate were monitored in the following hour. Sixteen out of 19 (84%) episodes of paroxysmal supraventricular tachycardia in 15 patients reverted to sinus rhythm at a mean interval of 11 min. None of 14 patients with atrial flutter reverted to sinus rhythm, but the ventricular rate decreased from 143 +/- 38 to 110 +/- 41 (p less than 0.02). The mechanism involved was slowing of the atrial cycle with associated increase A-V block in 7 patients. One patient experienced 1:1 conduction at a rate of 214 per min. Twenty seven % of 15 patients with atrial fibrillation reverted to sinus rhythm; ventricular rate decreased from 134 +/- 40 to 98 +/- 22 (p less than 0.05) in the remaining cases. There were no significant side effects of the drug and systolic blood pressure decreased from 109 +/- 27 to 99 +/- 30 beats per min (p less than 0.05) with no change in diastolic pressure.

Metabolism of lipid in the human fatty streak lesion.

The composition and synthesis of lipid have been examined in normal intima and adjacent fatty streak lesions of human arterial segments which were incubated with [2-(14)C]acetate. The incorporation of acetate into lipid was greater in fatty streaks than in normal intima. There was increased acetate incorporation into all major lipid groups in the fatty streak, with the greatest relative increase in the cholesteryl ester fraction. The major radiolabeled arterial fatty acid was a long-chain polyenoic fatty acid with chromatographic properties of a C22:4 acid. The content of fatty acid in the fatty streaks differed from that in normal intima, with substantial increases in the C18:1 acid and decreases in C16:0, C18:0, and C18:2 acids of the fatty streak. It is concluded that lipid synthesis is increased in the human fatty streak lesion and that the local metabolism of lipid contributes to its accumulation within the fatty streak.
