Relationship of AgNOR counts and nuclear DNA content to survival in patients with parathyroid carcinoma.

The aim of this study was to evaluate the usefulness of DNA flow cytometry to determine tumor nuclear DNA index (DI), and nucleolar organizer region protein counts visualized by the argyrophil (AgNOR) technique, in confirming diagnosis and predicting clinical outcome of patients with parathyroid carcinoma (PC). We reviewed paraffin-embedded tissue sections, from 15 patients (median age 63 years, range 30-68 years) with PC who died of the disease, which were randomly compared with tissue sections from 15 age- and sex-matched patients with parathyroid adenoma (PA). The proliferative activity in parathyroid tumours as detected by DI and AgNOR counts was evaluated in all specimens. Both DI (1.37 +/- 0.33 vs 1.0 +/- 0.1) and AgNOR (3.01 +/- 0.31 vs 1.54 +/- 0.35) counts were higher (P < 0.001) (Student's t-test) in patients with PC than in those with PA. Diploid (DI = 1), aneuploid (DI > 1) and hypoploid (DI < 1) neoplasms were found in 11 (PC = 4, PA = 7), 14 (PC = 11, PA = 3) and five (PC = 0, PA = 5) patients respectively. The average postoperative survival in patients with PC was 46.9 +/- 37.4 months (range 21-146 months). The survivals of patients with aneuploid (n = 11) and diploid (n = 4) PC were 74.0 +/- 58.1 and 34.1 +/- 18.4 months (P=0.21) respectively. There was a significant relationship between DI and AgNOR counts (R=0.69, P < 0.01), but no correlation was found between survival and both DI (Rho = 0.17, P = 0.55) and AgNOR counts (Rho = 0.26, P = 0.35). Moreover, there was no correlation (P = NS) between the main preoperative biochemical parameters and survival. In conclusion, DI and AgNOR are useful in confirming the diagnosis of PC, but they are of little value in predicting the clinical outcome of patients with PC.

Fibrous dysplasia of middle turbinate associated with Widal syndrome: endoscopic treatment of a rare case.

Fibrous dysplasia, a rare bony disease, is characterised by substitution of normal bone with immature tissue embedded in a fibrous stroma. It can be either monostotic or involve several bones. Fibrous dysplasia is usually asymptomatic but, in the advanced stage, pain due to neural compression or pathological fractures may occur. In the case of cranio-facial involvement, ocular, masticatory, respiratory or auditory functional alterations are possible. A case of fibrous dysplasia, limited to the middle turbinate and associated with Widal triad (sinus-nasal polyposis, asthma, acetyl salicylic acid intolerance), is described. Craniofacial computed tomography revealed enlargement of left middle turbinate with characteristic "ground-glass" appearance. The patient underwent anterior bilateral functional endoscopic sinus surgery with near-total resection of left middle turbinate. Histopathological examination confirmed the diagnosis of fibrous dysplasia. After 2 years the patient is still asymptomatic. Videorhinoscopy shows good sinus-nasal patency without disease recurrence. Even though exceptional, localization of fibrous dysplasia at middle turbinate has been described, therefore, it must be considered in the differential diagnosis of the craniofacial ossifying disorder. For localized and symptomatic lesions, endoscopic surgery is an effective option. Scrupulous life-long follow-up is necessary due to the high percentage of recurrence and possible malignant degeneration.

Effects of dopaminergic stimulation on peripheral markers of apoptosis: relevance to Parkinson's disease.

We investigated the effects of dopaminergic stimulation on anti-apoptotic protein Bcl-2, pro-apoptotic enzyme caspase- 3, and anti-oxidant/anti-apoptotic enzyme Cu/Zn superoxide dismutase (SOD) in human lymphocytes exposed to dopamine (DA). The same determinations were also carried out in parkinsonian patients treated with L-dopa. Caspase-3 activity and Cu/Zn SOD levels tended to increase when lymphocytes were exposed to low or intermediate doses of DA, while a decrease was observed, particularly in caspase-3 activity, with the higher DA dose. Bcl-2 levels were unaffected. In patients, we observed a negative correlation between Cu/Zn SOD levels and daily intake of L-dopa, which also tended to be negatively correlated with caspase-3 activity, but not with Bcl- 2. Our results show that dopaminergic stimulation is associated with complex changes in regulatory proteins of apoptosis.

Structural and ultrastructural aspects of the pericardial sac in some sea water teleosts.

Generally, in fishes the parietal pericardium is considered to be a semi-rigid structure. It is widely known that the entire parietal pericardium in fish, in contrast to that of higher vertebrates, is fixed to the body wall which consists mainly of skeletal muscle. This structural arrangement could explain the low end-systolic intrapericardial pressure in fishes. The morphology of the parietal pericardium of 11 sea-water teleosts, either free or breeding, coming from various areas of the southern Italy was studied. Macroscopical evaluation, but even more, histological and ultrastructural studies, showed some differences among the hearts of the examined subjects, more pronounced in the European hake (Merluccius merluccius, L.), in which a true pericardial sac was demonstrated, well differentiated and distinguishable from the other structures, in contrast to the observations in other subjects. The pericardial connective tissue showed no continuity with that of the surrounding structures, mainly skeletal muscle. This difference could be retraced to the well-known pattern of life and, therefore, to other individual as well as evolutionary factors in comparison with other teleosteans examined.

Reinke's crystals in an interstitial cell tumour of a rabbit (Oryctolagus cuniculus).

One case of interstitial cell tumour in the testicle of a New Zealand White rabbit was reported. The rabbit was 9-years old and had monolateral testicular lesion. There was enlargement of left testis and the lesion were seen on the cut surface. Light microscopy revealed intracytoplasmic crystals of Reinke that resembled to the features observed for this tumour in human. Immunohistochemically, this tumour was positive for vimentin.

Characterisation and evaluation of the emissions from the combustion of Orimulsion-400, coal and heavy fuel oil in a thermoelectric power plant.

An experimental campaign was carried out in a thermoelectric power plant in Southern Italy in order to evaluate the emissions released during the utilisation of three different fuels, Orimulsion-400, coal and heavy fuel oil (HFO) with high content of sulphur. Macropollutant concentrations in the exhaust gases were recorded by the continuous monitoring system and the Institute on Atmospheric Pollution of the Italian National Research Council (CNR) carried out samplings and analysis of Polycyclic Aromatic Hydrocarbons (PAHs), Polychlorodibenzo-p-dioxins (PCDD) and Polychlorodibenzofurans (PCDF), halogenidric acids and metals. The pollutant concentrations, mass flows and emission factors were compared. Each fuel met the air emission limits for all the pollutants. Coal showed the highest emission levels, whilst HFO and Orimulsion were fairly similar. NOx, SO2 and particulate matter showed constant concentration for all the fuels, while Orimulsion and HFO SO2 concentrations were critical at the highest operating power. Orimulsion showed the lowest PAHs values and PCDDs and PCDFs were much lower than the limits, for all the fuels. Particular attention must be paid with vanadium and nickel during Orimulsion utilisation. For these metals mass balances were carried out.

Sarcoidosis and HIV infection: a case report and a review of the literature.

Sarcoidosis occurring in patients with AIDS is rare. This infrequent association has been attributed to the impairment of the immune system that may interfere with the granuloma formation in HIV infected patients. However, the introduction of highly active antiretroviral therapy (HAART) has brought about a substantial and sustained increase in CD4+ T lymphocyte cells, and has consequently led to the development of the so called "immune restoration disease". The case of an HIV infected man who developed sarcoidosis after the initiation of HAART is described. Skin nodule images and histological specimens are reported. The association between sarcoidosis and HIV infection is also reviewed.

Peripheral markers of apoptosis in Parkinson's disease: the effect of dopaminergic drugs.

In this study, we measured the lymphocyte levels of proteins involved in apoptosis regulation, such as Bcl-2, the peripheral benzodiazepine receptor (PBR), caspase-3, and Cu/Zn superoxide dismutase (Cu/Zn SOD), in patients with Parkinson's disease (PD), either untreated or under therapy with dopaminergic agents (l-Dopa alone or l-dopa + dopamine agonists) and in healthy volunteers. All PD groups showed increased activity of caspase-3, compared to controls, particularly those under treatment only with l-Dopa. In this latter group, the increase in caspase-3 activity was also paralleled by an increase in the concentration of Cu/Zn SOD. In addition, patients taking l-Dopa + dopamine agonists showed marked decrease in Bcl-2 levels and increased PBR expression, which seems in keeping with the hypothesis that PBR may be functionally related to Bcl-2. In conclusion, we found clear modifications in the levels of proteins involved in the control of apoptosis in lymphocytes of PD patients. These changes were disease related but also modulated by the pharmacological treatment, which confirms the potential role of apoptosis in PD pathogenesis and the modulatory influence of dopaminergic agents.

Light and scanning electron microscopy observations on Gnathia vorax (Isopoda: Gnathiidae) larvae.

A large population of gnathid isopods (Crustacea) has been observed on sea water teleost fishes coming from the Civic Aquarium of Messina (Italy). Gnathiidae are parasite of fishes only at the stage of praniza, while adults are free living. Several teleosts, reared in captivity at the Civic Aquarium of Messina were sent for parasitological examination over a period of three years. Numerous parasites, identified as pranizae of Gnathiidae, were found on dorsum, on dorsal, pectoral and caudal fins, as well as in oro-pharyngeal cavity and gills of the affected fishes. The cranial part of an exuvie of adult male belonging to Gnathia sp. was also found on the floor of the tank. Because adult parasites are rarely detected in the sand bottom, the taxonomic identification is based on the morphological characteristics of larvae, even if it is very hard to show interspecific differences among gnathiids. For this reason, the morphological study was performed also by SEM to better show the surface details with taxonomic meaning. One to five mm long pranizae had a flattened body characterised by a head followed by 5 thoracic and 6 abdominal segments and a caudal plate with two biarticulated uropods. In the oral part of the head there was a ventrally oriented and smooth trapezoidal rostrum. Oral appendages, not clearly visible by light microscopy, were: a) two jaws with denticulate internal sides longer than the rostrum; b) a single ipostoma apically divided into two long appendixes; c) long and thin maxillae, with denticulate distal margins; d) two paired maxillipedes showing ciliate internal side that reaches the extremity of buccal cone; e) two big gnatopods with apical nail. Flagellum of antennulae had 7 articles. Telson was triangularly shaped, longer than large, with truncated apex having two apical bristles. Uropods were short with rounded internal angle and were provided by smooth and plumose setae; endopod was larger than exopod. The jaw of the adult gnathiid male, studied only by light microscopy, was solid, with oblique and denticulated blade; long and thin apex; rounded internal lobe; prominent external spur. Morphological characteristics of the jaw of the adult male and the shape of larvae permitted us to identify our specimens as Gnathia vorax (Lucas, 1850).

Unitubulotestis sardae (Trematoda: Didymozoidae) infection in Atlantic bonito Sarda sarda (Perciformes: Scomberomoridae) in the Ionian and Tyrrhenian Seas: histopathological and SEM investigations.

Nematobothrinae are trematoda digenea that usually live in cysts in fish tissues. They have been reported in different teleost species although the host-parasite relationships remain to be elucidated. This report is aimed at analysing the presence of Unitubulotestis sardae in the Atlantic bonito (Sarda sarda) populations fished in the Straits of Messina. The histopathological change reported in literature in similar cases is an aneurisma verminosa saccata, whereas, in the specimens here described, we show neovascularisation and at the same time, hyperplasia of the gill epithelium, leading to the formation of a bistratified cystic wall. The SEM examination of the parasitic cysts gave us a better understanding of the surface microanatomical details of the cystic walls. The external surface appeared to be constituted by a pluristratified covering epithelium of the gill arch. The internal layer was constituted by a net of blood vessels richly anastomosed with those originating from the gill arch arteries. This could be in agreement with data reported by Grabda (1947) on the localisation of the juvenile stages of the parasite in this blood vessel, but the evolution in cysts formation does not show similarities with the formation of an aneurisma.

Scanning electron microscopy of the carapace of some Mediterranean crab species.

Observation by SEM of the carapace microstructure of crabs showed, in the past, numerous differences at a specific level (i.e. in the genus Ebalia and Liocarcinus) as described by Spanò et al. (1995, 1999). The aim of this study is to describe the carapace micromorphology of some crab species of very different genera and species. A critical analysis of ecological data and morphological parameters is carried out in order to determine if the differences have adaptive significance.

Cytokine, infiltrating macrophage and T cell-mediated response to development of primary and secondary human liver cancer.

BACKGROUND: Kupffer cells, monocytes and infiltrating T cells have been considered the major source of interleukin-1beta and tumour necrosis factor-alpha in the liver. AIMS; To explore the expression of interleukin-1beta and tumour necrosis factor-alpha and to evaluate the density and the distribution of T lymphocytes and monocytes/macrophages in the liver of patients with primary and secondary tumours. METHODS: Tumoural and peritumoural liver samples were examined from 21 patients with hepatocellular carcinoma, 10 with hepatic metastases, 5 with benign focal liver lesions and 4 healthy adult livers. Interleukin-1beta and tumour necrosis factor-alpha mRNAs were detected by a semiquantitative comparative reverse transcriptase polymerase chain reaction. T lymphocytes and monocytes/macrophages were detected by immunohistochemistry. RESULTS: Higher levels of interleukin-1beta, tumour necrosis factor-alpha, CD3+ and CD68+ cells were found in the tissue surrounding hepatocellular carcinoma and metastases than in the tumour itself. A strong expression of CD68+ and CD3+ cells was found mainly along the tumour-host interface but the highest expression of CD3+ cells was found at the metastasis interfaces. Interleukin-1beta expression, CD3+ and CD68+ cell densities were higher in peritumoural samples than in so-called "normal" liver tissue. CONCLUSIONS: An increased production of interleukin-1beta and, to a lesser extent, of tumour necrosis factor-alpha mRNA coincides with the presence of cancer be it primary or secondary, both in healthy and cirrhotic livers. The presence of cancer, irrespective of the presence of underlying liver damage, appears to play the most important role.

Mucopolysaccharidosis VI in a Siamese/short-haired European cat.

A 3-year-old Siamese/short-haired European cat was referred for clinical disease characterized by dwarfism, facial dysmorphia, paralysis, small and curled ears, corneal clouding and large areas of alopecia. X-ray examination showed multiple bone dysplasia. On the basis of clinical features a form of mucopolysaccharidosis was suspected. The cat, killed at the owner's request, presented several severe skeletal deformities such as long caudal limbs, enlarged thorax with sunken breastbone, vertebral ankylosis in many spinal segments and visceral involvement. Histologically, the cat showed diffuse vacuolization and enlargement of cells in cartilage, bone and visceral organs. Ultrastructurally, membrane-bound vacuoles were filled with fibrillar and fluffy-material or concentrically whorled lamellae. Arylsulphatase B activity was 3.24 nm/mg/h in the affected cat and 30.6 in a normal age-matched control (NC). The L-iduronidase activity was slightly increased. Quantitation of total glycosaminoglycans (GAGs) revealed a 4.5-fold increase in the affected cat as compared with NC, while electrophoretic run of specific GAGs [chondroitin sulphate (CA); hyaluronan (HA); heparan sulphate (HS); dermatan sulphate (DS); keratan sulphate (KS)] performed on a cellulose acetate sheet, showed a striking increase in the DS band. On densitometric analysis of the electrophoretic run stained with Alcian Blue 8GX, the absorption of DS was eight-fold increased as compared with NC. The clinical and morphological features, and the biochemical findings, were consistent with the diagnosis of feline mucopolysaccharidosis VI.

Organic aerosols in urban and waste landfill of Algiers metropolitan area: occurrence and sources.

In both downtown Algiers and the waste landfill of Oued Smar, the concentrations of particulate organic compounds comprising n-alkanes, n-alkanoic acids, n-alkan-2-ones, polycyclic aromatic hydrocarbons (PAHs), oxygenated (OPAHs), and nitrated polycyclic aromatic hydrocarbons (NPAHs) in ambient air were measured from May 1998 to February 1999. All the components except OPAHs had a tendency to strongly increase in colder weather. Motor vehicles were found to be the main source of airborne particles in downtown Algiers, while the combustion and pyrolysis processes and bacterial activity seemed to concur to the air pollution at the Oued Smar waste landfill. The biogenic emission, which was restricted to the lighter fraction of the n-alkanes and n-alkanoic acids, appeared to contribute at a lesser extent. The in-situ generation of some OPAHs and NPAHs seemed to contribute to air pollution, especially during the summertime. As expected, the ambient concentrations of NPAHs and OPAHs were lower than those of their parent PAHs. The seasonal variations in ambient NPAH and OPAH concentrations are due partly to fluctuations of precursors including NOx, 03, and OH radicals. In general, the wintertime concentrations of the organic pollutants in Algiers were similar to those measured in Europe and especially over the Mediterranean Basin. Further investigations have been planned in order to obtain a thorough knowledge of the air pollution as well as the organic content of materials burning atthe Oued Smarwaste landfill. In particular, our concern will be addressed to sources of toxic components, to formulate strategies suitable for reducing health risk for the populations living in the region of Algiers.

Chemical characteristics of organic aerosol in Bab-Ezzouar (Algiers). Contribution of bituminous product manufacture.

The organic compositions of atmospheric particulate matter from Bab-Ezzouar (Algiers) have been investigated to assess the air pollution levels suspected to be caused by asphalt product and yeast manufactures. After a medium-volume air sampling, soxhlet extraction, alumina elution and HPLC separation, the extracts were analysed by high-resolution gas chromatography (HRGC) and gas chromatography coupled to mass spectrometry (GC-MS). The composition of n-alkane and polycyclic aromatic hydrocarbons (PAH) fractions reflected the petrogenic origin from the emission of asphalt materials production in addition to vascular plant wax emissions. In contrast, microbial activities seemed to play the main role for the presence of n-alkanoic acids at Bab-Ezzouar. The sole nitrated polycyclic aromatic hydrocarbons (NPAH) observed, i.e., 2-nitrofluoranthene (2NFA), was very likely to arise from gas-phase photochemical reaction of parent PAH in the atmosphere. The total aerial levels ranged from 75 to 206 ng m(-3) for n-alkanes, from 153 to 345 ng m(-3) for n-alkanoic acids and from 44 to 100 ng m(-3) for PAH and NPAH. Although the samples were collected during the hot season, the levels of these pollutants seemed to be important and of environmental concern, especially for PAH species.

Follicular dendritic cell tumor of the mediastinum.

Follicular dendritic cell tumor (FDT) is a rare neoplasm usually occurring in the laterocervical lymph nodes, but presentations elsewhere are also well documented. We report a case of FDT in a 48-year-old man with myasthenia gravis, in whom a slow-growing mediastinal mass that had been surgically excised manifested after 3 years with a local recurrence in the same site. The lesion was aspirated and cytology showed a tumor composed of groups of oval or elongated cells intermingled with several mature small lymphocytes. On histology, the tumor was highly cellular with abundant perivascular lymphocytic infiltration; large cells with pale, eosinophilic cytoplasm and round nuclei, arranged in fascicles often showed a storiform pattern. Occasional binucleated cells were also present. A panel of antibodies showed positivity only for CD21 and CD35, and a weak response to S-100. Electron microscopy showed that the layer cells had convoluted nuclei and elongated interdigitating processes with desmosome-like junctions. The definitive diagnosis of FDT can be concluded only with positive immunostaining for CD21 and CD35. However, in the presence of a mixed population of lymphocytes and larger eosinophilic cells the hypothesis of FDT should always be considered.

Diazepam stimulates migration and phagocytosis of human neutrophils: possible contribution of peripheral-type benzodiazepine receptors and intracellular calcium.

In isolated human neutrophils, diazepam (10 nmol/l to 10 micromol/l) concentration-dependently increased migration and phagocytosis. Diazepam-induced migration and phagocytosis were inhibited by the peripheral benzodiazepine receptor (PBR) antagonist PK11195 (10 micromol/l). The PBR agonist Ro5-4864 (10 nmol/l to 10 micromol/l) did not affect migration but slightly enhanced phagocytosis, while clonazepam, which binds to the central-type benzodiazepine receptors but has no affinity for PBRs, was ineffective on both parameters up to 10 micromol/l. Phagocytosis induced by diazepam or Ro5-4864 was inhibited by the Ca2+ channel blocker L-verapamil (10 micromol/l), which however did not affect the action of diazepam on migration. Competition binding experiments performed by fluorescent staining of PBRs showed that diazepam directly interacts with PBRs on human neutrophils. Both diazepam and Ro5-4864 (10 nmol/l to 10 micromol/l) induced a rise of intracellular free Ca2+ concentrations ([Ca2+]i), which was inhibited by PK11195 (10 micromol/l) and L-verapamil (10 micromol/l) and prevented by extracellular Ca2+ chelation with EGTA (5 mmol/l). In conclusion, experimental evidence indicates that in human neutrophils diazepam stimulates both migration and phagocytosis through activation of PBRs. Diazepam-induced [Ca2+]i changes depend on a PBR-operated, L-verapamil-sensitive increase in the plasma membrane permeability and subsequent extracellular Ca2+ entry, and contribute to diazepam-induced phagocytosis. On the contrary, the effect of diazepam on migration seems to occur through Ca2+ -independent mechanisms.

HPLC-ED measurement of endogenous catecholamines in human immune cells and hematopoietic cell lines.

A rapid and simple HPLC-ED method is described to identify and measure catecholamines (CTs) and their major metabolites in immune cells. Using this method, intracellular CTs were quantified in human peripheral blood mononuclear cells (PBMCs), T and B lymphocytes, monocytes and granulocytes. Immune cell subsets were separated by density gradient centrifugation and immunomagnetic cell sorting. CTs were also found in the human hematopoietic cell lines NALM-6 (pre-B) and (in smaller amounts) in Jurkat (T lymphoblastoid) and U937 (promonocytic). In cultured PBMCs, intracellular CTs were reduced by both the tyrosine hydroxylase inhibitor alpha-methyl-p-tyrosine and the chromaffin granule depletant reserpine. In NALM-6 cells, both alpha-methyl-p-tyrosine and the dopamine-beta-hydroxylase inhibitor disulfiram reduced intracellular CTs, supporting the presence of active synthetic pathways in these cells. Since sympathoadrenergic mechanisms play a key role in the interactions between the immune system and the nervous system, these findings may be relevant for a better understanding of the neuro-immune network.
