Aetiology, cost of antimicrobial therapy and outcome in neutropenic patients who developed bacteraemia during antimicrobial prophylaxis: a case-control study.

Sixty four episodes of bacteraemia that appeared during antimicrobial prophylaxis with an oral quinolone plus an azole in neutropenic cancer patients were compared with 128 cases of bacteraemia in a cohort of controls matched for age, sex, underlying disease, neutropenia and vascular catheter in situ to assess differences in aetiology, cost of therapy and outcome. Patients who received prophylaxis had breakthrough bacteraemias of a different aetiology compared with the control group: they had significantly fewer multiply-resistant strains (21.9 vs. 51.5, P < 0.04) and a longer afebrile neutropenic period (9.55 days vs. 4.1, P < 0.001). Patients who received prophylaxis also had bacteraemias that were significantly more frequently caused by viridans streptococci (9.4%, vs. 1.7%, P < 0.01), enterococci (15.6% vs. 7.2%, P < 0.05) and Stenotrophomonas maltophilia (17.2% vs. 3.4%, P < 0.01). The cost of antimicrobial therapy per case (37401 SKK (1091 USD) vs. 31808 SKK (899 USD), P < 0.05) was also significantly higher in cases than controls; however, the number of administered antibiotics (4.18 vs. 3.21 per case, P = NS) was similar in both groups. There were no differences in outcome between both groups. However patients who received prophylaxis had significantly longer periods of afebrile neutropenia (9.55 days vs. 4.1, P < 0.001) and bacteraemia developed later than in controls. Also, the incidence of polymicrobial bacteraemia caused by multiresistant strains was lower among cases (21.9 vs. 51.5, P < 0.04).

Identification of Borrelia burgdorferi sensu lato tick isolates from Slovakia by PCR typing with 16S rRNA primers.

The first four strains of Borrelia burgdorferi isolated in Slovakia from ticks and mice were studied using monoclonal antibodies, the polymerase chain reaction (PCR) with 16S rRNA specific primers and plasmid profiles. Two tick isolates were typed as Borrelia garinii, one strain isolated from Apodemus flavicollis was found to be B. afzelii and the fourth tick isolate reacted as a mixed culture of B. garinii and B. afzelii. All four strains harboured several plasmids ranging from 6-50 kbp including a plasmid with a size of approximately 41 kbp.

[Preliminary identification of Borrelia burgdorferi isolated from Ixodes ricinus ticks in eastern Slovakia].

The first finding of spirochetes Borrelia burgdorferi sensu lato in eastern Slovakia pertains to ticks Ixodes ricinus collected in the Kosice park forest in September 1995. The authors tested 7 strains using immunoblotting with 4 monoclonal antibodies (Mab): H9724--specific to the genus Borrelia, identifying 41 kDa flagellar protein, H5332--specific to OspA protein of B. burgdorferi, J 8.3 and I 17.3 specific to OspA and OspB proteins of B. afzelii. All the strains belong to the genus Borrelia (reacted with Mab H9724), 6 of 7 strains designated Ir103, Ir105, Ir107, Ir108, Ir112 and Ir113 were identified as B. burgdorferi sensu lato (reacted with Mab H5332). None of the strains reacted with Mab specific for B. afzelii.

[Determination of NK cell activity on flow cytometry].

The authors assessed the activity of NK cells on a flow cytometer using 5-carboxyfluorescein diacetate as fluorescent substrate. They examined 25 healthy subjects aged 23-52 years. In NK cellular activity the normal distribution applies and the value of NK cellular activity for the whole group as regards the ratio of effector cells to target cells was 25:1, 23.46 +/- 15.84 percentage. The authors did not detect any significant differences in the activity of NK cells in relation to age and sex. They revealed statistically significant correlations of NK activity and the number of CD16+ and CD57+ cells as well as CD57+ CD3- cells. The optimal ratio of effector and target cells for assessment of NK activity was 25:1. Assessment of NK activity on a flow cytometer can be considered an equivalent substitute of the classical method, using radiochromium.

[Beta-2-microglobulin and ankylosing spondylitis].

Serum beta-2-microglobulin was examined by radioimmunoassay in 103 patients with ankylosing spondylitis and 111 healthy blood-donors. Statistically significant difference between the group of patients and healthy individuals was found related to increased levels of this protein in patients with ankylosing spondylitis. Furthers correlation analysis of beta-2-microglobulin in patients with ankylosing spondylitis and chosen clinical and humoral parameters of the disease activity was performed. Positive, statistically significant correlation with erythrocyte sedimentation rate, plasmatic gamma globulins, duration of morning stiffness and global activity of the disease was proved. Higher levels of beta-2-microglobulin in patients with ankylosing spondylitis could be attributed to activation of monocyte macrophages system in subpopulation of cytotoxic T-lymphocytes, which might be the evidence of the involvement of cytotoxic-T cell mechanisms in the pathogenesis of this disease.

[Human immunodeficiency virus infection and rheumatology].

The most important rheumatologic manifestations associated with infection with human immunodeficiency virus are described. Among arthritides, Reiter's syndrome, psoriatic arthritis, and some forms of reactive arthritis are the most common. Arthralgias are also relatively frequent. Manifestations of polymyositis, vasculitis and sicca syndrome with a relatively broad spectrum of various autoimmune phenomena are also very important. The author emphasizes the importance of interdisciplinary cooperation of internists, rheumatologists, infectionists and virologists in solving questions associated with laboratory and clinical aspects of the relationship between rheumatologic manifestations and infection with human immunodeficiency virus.

[Antibodies to Klebsiella pneumoniae in ankylosing spondylitis].

The authors investigated the incidence of antibodies against Klebsiella pneumoniae and E. coli in a group of patients with ankylosing spondylitis and in a group of healthy controls in all main immunoglobulin classes. The results revealed that in patients with ankylosing spondylitis there is a significantly higher incidence of specific antibodies of class IgA against Klebsiella pneumoniae and E. coli. This supports the thesis on the aetiopathogenetic interrelationship between enteric bacteria and the development of ankylosing spondylitis.
