Endocrinological studies in alcoholics during withdrawal and after abstinence.

Several endocrine parameters were assessed in 35 alcoholic inpatients after admission to hospital, and 17 of the 35 were retested after several weeks of sobriety. No difference was found in clonidine-stimulated growth hormone (GH) secretion between male alcoholics and male healthy controls, but significant positive correlations of GH secretion and alcohol content in expired breath on admission and gamma-GT values after abstinence were observed. Nonsuppression in the dexamethasone suppression test was found in 17% of the patients on admission, which seemed to be due to alcohol withdrawal. Postdexamethasone cortisol levels were significantly positively correlated with the "apathic syndrome" (r = 0.40; p less than or equal to 0.05). About one-third of the patients showed a blunted response in the TRH-test both on admission and after abstinence. No significant influence of alcohol intake, withdrawal or familial disposition on prolactin values could be detected. The results of the TRH test and the DST point to similar endocrinological patterns in alcoholics as in depressive patients and thus support the hypothesis of a link between alcoholism and depression.

[Praziquantel in human hydatidosis. Evaluation by preoperative treatment].

The efficacy of praziquantel has been studied in human hydatic disease due to E. granulosus, using parasitological and pharmacological criteria of improvement. This evaluation uses a prospective therapeutic trial in 15 patients with one or several hydatic cysts in different sites. Before surgery, nine of these received daily 75 mg/kg of praziquantel in 2 courses of 10 days each. The other six patients are considered as controls. The protoscolices vitality is determined by direct optic microscopy and by intraperitoneal mouse inoculation. According to the results of optic microscopy, praziquantel sterilizes hydatic cysts: 19 sterilized cysts out of 26 coming from treated patients, and 4 out of 11 in control group. This effect seems to be emphasized in hepatic localization. Nevertheless, according to the results of mouse inoculation, praziquantel does not significantly reduce the pathogenicity of inoculated proscolices and germinal layer: 11 mice out of 39 are healthy in the treated group, and 5 out of 15 in controls. Moreover, the determination with a fluorimetric method has not detected praziquantel in the hydatid liquid issued from treated patients. The drug does not pass through the cyst wall, and so, cannot have a scolicidal activity. Praziquantel cannot be considered as a medical treatment in human hydatidosis.

Cloning of cDNA sequences of a progestin-regulated mRNA from MCF7 human breast cancer cells.

A cDNA clone corresponding to an mRNA regulated by the progestin R5020, has been isolated by differential screening of a cDNA library from the MCF7 breast cancer cell line, which contains estrogen and progesterone receptors. This probe hybridized with a single species of poly A + RNA of 8-kb molecular weight as shown by Northern blot analysis and could also be used to total RNA preparation. This recombinant clone hybridized specifically to an mRNA coding for a 250,000 daltons protein when translated in vitro. This protein was identical to the 250 kDa progestin-regulated protein that we previously described (Biochem. Biophys. Res. Commun. 121, 421-427, 1984) as shown by immunoprecipitation with specific rabbit polyclonal antibodies. Dose-response curve and specificity studies show that the accumulation of the Pg8 mRNA and that of the 250-kDa protein was increased by 5 to 30-fold following progestin treatment and that this effect was mediated by the progesterone receptor. Time course of induction indicated that the accumulation of mRNA was rapid and preceded that of the protein. This is the first report on a cloned cDNA probe of progestin-regulated mRNA in human cell lines.

Breast Neoplasms

[Inpatient detoxification of alcoholic patients].

In the present paper, we report about a 2-year-experience with in-patient withdrawal of alcohol addicts. Out of 465 addicted pts., 178 pts. were treated with oral Clomethiazol. No delirium was observed during a mean treatment period of 6.6 days. 39 pts. who were delirious at admission, were also treated with oral Clomethiazol. In all of these pts., the delirium disappeared within 3-4 days. Possible explanations for these favourable courses will be discussed. Early and sufficient treatment of alcohol withdrawal syndrome with Clomethiazol seems to be one of the major factors in treating and avoiding delirium.

A computer program for parenteral nutrition solution preparation.

The preparation of single-bottle parenteral nutrition solutions for neonates has long posed a problem for pharmacists. Frequent formulation changes and the need for rapid supply of solutions after prescription has led to a variety of preparation techniques being developed. An interactive computer program is described which enables a nonspecialist pharmacist staff to safely and quickly produce the formula, label and fully document it from a prescription for parenteral nutrition. Ordered quantities are expressed in amounts required per kilogram per 24 hours, and are input to a computer using a "prompt" system. Features of the program include automatic compensations for all non-amino acid ingredients of a variety of commercial amino acid solutions; specified or unspecified chloride anion input; and logic checks which are tied to physiologie "danger" levels, signaling to the operator when "greater than a critical level" has been requested by the prescriber, as well as the conventional check of the logic. The program has allowed infinite variability to prescribing of all ingredients of parenteral nutrition, extremely rapid and accurate production of formulate and allows the computer storage of data about nutritional input to patients. The program is extremely simple in operation, and gives significant cuts in lag time between prescription and the commencement of the new formula.
