Pulmonary blastoma with hilar lymph node metastases: survival for 24 years.

Pulmonary blastoma is a very rare tumor, consisting of relatively well-differentiated tubular glands embedded in a malignant mesenchymal stroma. Only 12 examples have previously been recorded in children. Despite lymph node metastases in the present case, the patient has survived for 24 years without evidence of recurrence following treatment by lobectomy.

Aortic valve replacement for severe aortic regurgitation caused by idiopathic giant cell aortitis.

Giant cell aoritis occurred in a 25-year-old woman, with absent pulses in the left arm and severe aortic regurgitation from dilatation of the valvar annulus. The aortic valve was replaced by a Starr-Edwards prosthesis, and the patient was treated with steroids. Five years later, she continues asymptomatic and haemodynamically stable. The left brachial and radial pulses have returned.

Raised alpha-fetoprotein levels in amniotic fluid and maternal serum associated with distension of the fetal bladder caused by absence of urethra.

Raised alpha-fetoprotein concentrations were found at 29 and 30 weeks' gestation in the amniotic fluid and maternal serum of a woman who presented in her seventh pregnancy with apparent polyhydramnios. The fetus had multiple abnormalities including gross distension of the bladder resulting from absence of the urethra, intestinal artresia, and a congenital heart defect.
