Periodontitis as risk factor for acute myocardial infarction. A case control study of Spanish adults.

OBJECTIVE: The present study was designed to determine, in a case-control study of a Spanish population, whether periodontitis is a risk factor for acute myocardial infarction. BACKGROUND: Although part of cardiovascular risk could be explained by periodontal disease, available meta-analyses find significant heterogeneity and recommend the need for further observational and intervention studies. METHODS: A case-control study was conducted of 149 Spanish patients aged between 40 and 75 years, with 72 cases (acute myocardial infarction) and 77 controls (trauma patients). Periodontitis was measured as the percentage of sites with clinical attachment loss greater than 3 mm. A multivariate logistic regression model was constructed to estimate the adjusted effect of periodontitis on acute myocardial infarction, after considering the potential confounding effect of a large pool of risk factors. RESULTS: In a bivariate analysis, males, older patients, smokers, and those with hypertension, diabetes or hypercholesterolemia, showed an increased risk of acute myocardial infarction. The cases, compared to controls, showed worse results for all periodontal variables studied: gingival retraction, pocket depth, and periodontitis. The final multiple logistic model included sex, age, tobacco habit, hypertension, diabetes, hypercholesterolemia, regular exercise, and periodontitis. The association between periodontitis (dichotomized) and acute myocardial infarction was high and significant in both the unadjusted (odds ratio = 4.42, p < 0.001) and adjusted analyses (odds ratio = 3.31, p = 0.005). CONCLUSION: There is evidence of an association between periodontitis and acute myocardial infarction after adjusting for well-known risk factors for acute myocardial infarction.

[Spatial summation: its topography in the central visual field].

PURPOSE: Luminance (L) and area (A) of the stimulus are related by the equation LxAk=constant. We evaluated the k value in 66 positions of the central visual field. METHODS: Ten eyes of 10 healthy subjects were examined for conventional luminous thresholds at 66 positions of the central visual field with the TOP strategy using Goldmann sizes 4, 3.5, 3, 2.5 and 1.9 in the PULSAR perimeter. RESULTS: The k value increased in a lineal manner from the centre towards the periphery (slope=0.01 per degree, average k=0.616, r=0.98, p<0.01) but with unequal slopes at the different meridians. In the inferior hemi-fields it was higher (k=0.657) than in the superior hemi-fields (k=0.574). The k value at the supero-nasal hemifields is quite similar to that found at the paracentral region, and maximum at the temporal inferior region. CONCLUSIONS: Spatial summation in the central visual field has specific values for different positions.

Vigabatrin-induced modification of Ki-67 expression in gingival epithelium: immunohistochemical study of a short series.

OBJECTIVE: To study the expression and role in vigabatrin (VGB)-induced gingival enlargement of Ki-67 antigen and p27KIP1, p21WAF1, and p53, proteins that activate or inhibit cell-cycle progression. MATERIALS AND METHODS: Six patients treated with VGB for partial epileptic seizures refractory to classic anticonvulsant treatment were studied. Gingival biopsies were taken from four of these patients for immunohistochemical studies; 10 control biopsies from individuals with healthy gingiva and 10 from patients with periodontal disease were also evaluated. RESULTS: Four of the six patients presented some degree of gingival enlargement (mild or moderate). Nuclear expression of Ki-67 was elevated (mean of 894 positive cells/mm2 in VGB-induced gingival enlargement vs. 391 cells/mm2 in controls with healthy gingiva and 425 cells/mm2 in controls with periodontal disease) (p < 0.01, analysis of variance: anova), and nuclear expression of cyclin-dependent kinase (cdk) inhibitors p27KIP1 and p21WAF1 was reduced. The patients with gingival enlargement presented inflammatory infiltrate in lamina propria, mainly composed of T lymphocytes (CD3+) and plasma cells (CD38+), which was even more intense than in the biopsies of patients with periodontal disease. CONCLUSION: The overexpression of antigen Ki-67 and slight underexpression of cdk-inhibitors p27KIP1 and p21WAF1 suggest that VGB induced an increase in cell proliferation and contributed, together with concomitant periodontal disease, to the gingival enlargement.

Left-handed-media simulation and transmission of EM waves in subwavelength split-ring-resonator-loaded metallic waveguides.

At microwave frequencies, hollow metallic waveguides behave in certain aspects as a "one-dimensional plasma." This feature will be advantageously used for simulating the propagation of electromagnetic (EM) waves in left-handed metamaterials provided the hollow waveguide is periodically loaded with split ring resonators. It will be shown that EM transmission in this structure is feasible within a certain frequency band even if the transverse dimensions of the waveguide are much smaller than the associated free-space wavelength. This effect can be qualitatively and quantitatively explained by the left-handed metamaterial theory, thus providing a new experimental validation for such a theory.

[Meningitis caused by enterovirus in a pediatric hospital: experience in 1996].

BACKGROUND: To know the incidence, etiology to the serotype level and epidemiological characteristics of cases of enteroviral meningitis diagnosed in a pediatric emergency service in 1996. METHODS: Inoculation of conventional cell cultures with samples of cerebrospinal fluid (CSF), throat swabs and stools taken from children with clinically suspected meningitis. RESULTS: Enterovirus was cultured from 17 of 65 (26.2%) CSF samples with pleocytosis (more than 10 cells/mm3) and from 12 of 268 (4.5%) biologically normal samples. The identified isolates of CSF (20 of 29) were: ECHO 30 (9), ECHO 11 (7), ECHO 9 (3) and ECHO 7 (1). Most (79%) isolates were obtained between March and June. CONCLUSIONS: Enteroviral meningitis was apparently more common in spring, although the number of samples was significantly lower during the summer months. The clinical cases were sporadic, due to at least four echoviruses serotypes. The isolation of enterovirus from CSF without pleocytosis is not exceptional (41% of our isolates).

Vascular cryopreservation in microsurgery.

Cyropreservation of blood vessels has been carried out for some decades with variable results. A rabbit model was used to compare cryopreserved femoral artery allografts (n = 12 arteries), fresh autografts (n = 15 arteries), and fresh allografts (n = 16 arteries) at 1 and 3 months postoperatively. Patency rates were highest in the fresh autografts (86.7 percent), followed by the cryopreserved allografts (66.7 percent at 1 month and 83.3 percent at 3 months) and fresh allografts (62.5 percent at 1 month and 75 percent at 3 months). The fresh allografts showed the greatest alterations in endothelial cells and intima and muscle layer, followed by cryopreserved allografts, and then fresh autografts. Changes observed included pseudoendothelium formation, thickened intima, and thinner muscle layer. Cellular infiltrate appeared on the vessel walls only in the cryopreserved allografts (25 percent), but this did not have an effect on vascular patency. Fresh autografts remain the graft of choice for vascular defects, but cryopreserved allografts serve as the most appropriate option when the former are unavailable.

[Rotavirus infection: clinical characteristics and time of elimination of the rotavirus antigen in the feces].

BACKGROUND: A prospective study was performed during three years with 76 children hospitalized with the diagnose of rotaviral gastroenteritis. The aim of our study were to evaluate the clinical features of the rotaviral gastroenteritis, the excretion time of rotavirus in stools and the nosocomial rotavirus infection. METHODS: The detection of the rotaviral antigen in stool was performed using a rapid test based on latex aglutination (Rotalex). For the determination of the excretion time of rotavirus in stools the test was performed daily in 69 children until the results was negative. Furthermore, a clinical study of the patients was carried out. RESULTS: In 39 cases (51.3%) the gastroenteritis was acquired in the community and 37 cases (48.7%) were nosocomially acquired. The most frequent clinical presentation was an acute diarrhea (96%) with vomiting (63.2%) and fever (63.2%). Only three patients did not have diarrhea. The mean duration of diarrhea was between 3-4 days with a highest duration of 10 days. Most of the patients (58%) had an excretion time of rotaviral antigen lower or equal than 3 days, with an extreme value of 14 days. CONCLUSIONS: In our study rotaviral infection occurred mainly in the cooler winter months and in children younger than 6 months of age. A high nosocomial infection level (48.7%) was detected. Vomiting often preceded the onset of diarrhea and fever. The excretion time of rotaviral antigen was independent of the clinical course.

Effect of L-ornithine 8-vasopressin on blood loss during liposuction.

In this comparative study, we carried out liposuction on 20 patients randomly divided in two groups to find an alternative medication to epinephrine that would not result in secondary effects at the cardiovascular level but would offer a similar vasoconstricting capacity. Also, a variation of the wet technique is described that decreases blood loss secondary to liposuction. The area to undergo liposuction is infiltrated with a cannula of our own design. Epinephrine is not used as a vasoconstrictor but rather L-ornithine 8-vasopressin at a concentration of 0.01 IU/ml chilled saline. With this new technique, the amount of blood removed is minimal, even in the case of extraction of large volumes of fat.

Gingival overgrowth induced by nifedipine and cyclosporin A. Clinical and morphometric study with image analysis.

In this study, we developed a quantitative method with digital image analysis to evaluate the degree of gingival overgrowth (GO), and compared GO in kidney transplant patients treated with cyclosporin A (CsA) (n = 21) or CsA+nifedipine (n = 8) and a group of healthy controls (n = 30). The method was reproducible and reliable. Our findings showed significant differences in papillary and gingival surface between controls and transplant patients treated with GO inducers. Gingival overgrowth index also differed significantly between controls and each patient group (p < 0.01, Kruskal-Wallis test). The administration of the calcium channel blocker nifedipine potentiated the adverse effect of CsA: comparison of the morphometric findings revealed significant differences between patients treated with CsA alone and CsA+nifedipine in papillary area, dental area, and GO index (p < 0.01, Mann-Whitney U-test). We conclude that the method of image analysis we developed is useful in assessing the degree of GO.

The use of latissimus dorsi muscle flap in reconstructive heart surgery.

Dynamic cardiomyoplasty is a technique for ventricular assistance indicated for the correction of left ventricle aneurysm and for cardiomyopathies in which heart transplantation is contraindicated. In this article, our first four patients to undergo cardiomyoplasty (from February 1991 until April 1992) with a left latissimus dorsi island flap, rotated into the thorax after a 3-week training period, are reviewed. Therapeutic indications were cardiomyopathy with contraindication for heart transplant in patients 1 and 4 and associated to surgery for left ventricle aneurysm for patients 2 and 3. Patient 1 died 4 months after surgery because of a cerebral embolism when he was at functional class II. The other three patients remain at functional class I, carrying out normal activities. All the data were evaluated with the paired t test. Ejection fraction values (obtained through echocardiography and scintigraphy studies) and maximum minute flow rate of blood (measured with an echo-Doppler) have increased significantly (p < 0.001, p < 0.05, and p < 0.01, respectively) after heart surgery. Clinical improvement has been found after cardiomyoplasty, which correlates with an increase in ejection fraction and maximum minute flow rate of blood.

Surgical treatment for congenital absence of the vagina using tissue expansion.

A modification of the Abbe-McIndoe technique for the treatment of congenital absence of the vagina, using a cylindrical tissue expander wrapped in a mesh split-thickness skin graft, instead of a rigid silicone mold, is presented. The expander was kept in place for a period of two to six months and the functional result was 100 percent in all the six patients treated. Our experience led us to change the design of the expander (cylindrical, without ring, 10X4 centimeters) and to place the valve in the inner thigh as the best site for the valve. The age range of the patients was 18 to 27 years, and the final result was good or very good in five of the six women. Only one patient has occasional dyspareunia, which does not prevent sexual intercourse.

Use of a calf prosthesis and tissue expansion in aesthetic reconstruction of the leg.

We present a technique for reconstruction of the legs in patients with soft-tissue loss and formation of large scars with retraction of this tissue in the pretibial region. In such patients, a subcutaneous tissue expander is placed in the region adjacent to the scar tissue. With expansion, we obtained sufficient skin for use in the reconstruction, and the resulting asymmetry in leg diameter was compensated for by means of one or two calf prostheses, depending on the patient.

The vascularized fibula graft in mandibular reconstruction.

The use of a vascularized graft from the fibula for mandibular reconstruction is presented. The great strength, pliability, and long vascular pedicle that characterize such grafts make them particularly suitable for this type of repair. The results in six cases were highly satisfactory. Masticatory function was well restored, without alteration of the facial contour, and with minimal sequelae at the donor site.

Simultaneous reconstruction of both feet with a vascularized latissimus dorsi free flap.

Simultaneous reconstruction of both feet with a single vascularized latissimus dorsi free flap is reported. The authors describe the repair of extensive dorsal and plantar defects, as well as of heel lesions secondary to osteitis of the calcanei. The latissimus dorsi free flap serves well in combining the classic cross-leg procedure with microsurgical techniques.

[Bone gammagraphy for evaluating free vascularized bone grafts in mandibular reconstruction].

In oncology jaw reconstruction, the probability of infection, sequestration and failure in scarring is high. For this reason, we use a vascularized free bone graft, whose direct blood flow makes it much more resistant to support the attendant properly. The use of bone scintigraphy by means of three phases acquisition technique is very effective to evaluate the viability of these vascularized grafts during the first days after surgery, in order to adjust the attendant treatments and to prevent the possible complications.

[Nifedipine induced gingival hyperplasia].

Nowadays we have a whole of drugs, Calcium antagonists (AC), like Verapamil (VE), Nifedipine (NI), Diltiazem (DI), used in cardiovascular therapy. One of them, Nifedipine, has several side effects, out of which is gingival overgrowth. In the present article we present four cases, in which have been analyzed this effect and its resolution in one of them, after drug suppression and patient's hygienic habits correction.
