[1.5-T MR of the knee: the correlations with arthroscopy].

The authors report their experience of knee articular pathologies studied with MR imaging. Forty patients (36 with and 4 without articular trauma) were examined with both MR imaging and arthroscopy. Arthroscopic findings were correlated with MR results in order to evaluate the accuracy of the latter in detecting meniscal tears. MR images were obtained with a 1.5 T unit and a transmit-receiver surface coil. Spin-echo sequences and, occasionally, fast-scan sequences were used both in the coronal and in the sagittal planes. High resolution MR imaging clearly depicted knee anatomy. As far as meniscal lesions were concerned, the authors employed a grading system to classify intrameniscal signals, as proposed by Lotysch. The preliminary results are encouraging and suggest the use of MR imaging as the first imaging modality for use in knee pathology before arthroscopy is performed.

[Implantation of ultra thin naso-duodenal and naso-jejunal tubes for feeding].

Enteral nutrition through filiform naso-duodenal or naso-jejunal tubes with homogeneous low molecular nutritional solutions has a rather important place in therapy of different conditions; endoscopy can help to introduce these tubes into the intestinal tract in the following situations: A leak in the upper gastrointestinal tract, which may occur after surgical resections of the esophagus or stomach, or which may occur in rare cases after bouginage or after diagnostic procedures can be passed by with an ultrathin tube until it is healed of. Patients with stenotic lesions of the esophagus or the cardia, which can not be treated by surgery or conservative procedures like pertubation and dilation can be fed enterally if it is possible to move the tube beyond the stenosis. Patients with neurogenic swallowing dysfunction of different origine can be fed during long periods of time through such filiform naso-enteral tubes successfully and reach a perfect nutritional state. In acute treatment of Crohn's disease and other inflammatory intestinal diseases enteral nutrition through tubes is an essential therapeutic procedure.

[Changes in the atlanto-odontoid articulation in rheumatoid arthritis. Radiologic observations in 86 cases].

The authors describe the alterations of atlanto-axial joint observed in patients with rheumatoid arthritis: 16 of 86 patients = 18.6% (15 females and 1 male) showed the presence of joint luxation. The authors suggest that a "dynamic" tomographic study in lateral orthostatic projection, with indifferent position and head flexed, is essential for demonstrating the changes of this joint and that examination should be performed in all patients with rheumatoid arthritis, even if they are asymptomatic.

[Multi-institution, multidisciplinary radiology and endoscopy trial of the diagnostic accuracy of the double contrast examination of the stomach surface].

In 12 X-ray and gastroenterology departments 1570 patients, clinically suspected to have a gastric pathology, have been examined by double contrast (surface) examination of the stomach followed by gastroscopy. Diagnostic accuracy was 93% in respect of endoscopic data, assumed correct by hypothesis. On the basis of a cost and benefit evaluation, made according to the well known criteria, a wider routine use of the double contrast technique is proposed.

[Effect of 3,4,5-trimethoxy-benzoyl-aminocaproic acid on the cytolytic process in myocardial infarct. Working hypothesis].

The equilibrium of the ion pump is seriously disturbed at the cell level in acute myocardial infarct. The damaged cells are unable to take up potassium and this remains in the intercellular spaces, bound to acid substances released by the necrotic site. This results in self-perpetuating damage to surrounding tissue, together with extension and aggravation of the infarct. Early treatment with 3,4,5-trimethoxy-benzoyl-aminocaproic acid enables this secondary type of lesion to be prevented, since this substance is capable of fixing potassium ions and returning them to the cell.

Maternal plasma calcitonin gene-related peptide levels do not change during labor and are not influenced by delivery route.

OBJECTIVE: Calcitonin gene-related peptide (CGRP) circulates in maternal circulation throughout pregnancy, and specific receptors for CGRP (CGRPrs) are expressed by human myometrium. Because CGRP induces a dose-dependent relaxation of human myometrium, we examined a role for CGRP in modulation of myometrial smooth muscle contractility during pregnancy and labor. The aim of the present study was to evaluate the changes of maternal serum CGRP levels during parturition, according to the mode of delivery and in relation to cervical dilatation. METHODS: Circulating CGRP levels were measured in the following groups of healthy women: nonpregnant women, during the follicular phase of the menstrual cycle (n = 19); at term pregnancy (39-40 weeks; n = 24); after elective cesarean delivery (39-40 weeks; n = 20); and at spontaneous vaginal delivery (39-40 weeks; n = 16). In a subgroup of women, blood samples were collected longitudinally throughout labor at various cervical dilatations in the progress of labor (n = 8). RESULTS: Pregnant women at term not in labor had significantly higher CGRP levels than nonpregnant women (P =.021). No significant difference was found between women who delivered vaginally and those who had elective cesarean, and there were no correlations between CGRP plasma levels and cervical dilatation. CONCLUSIONS: Parturition is characterized by no significant changes in maternal serum CGRP levels, and no significant correlation exists between plasma CGRP levels and cervical dilatation during labor.

[Agenesis of the gallbladder. Description of 2 cases in 2 siblings].

Gallbladder and cystic duct agenesis is a rare malformation with frequent familial occurrence. We report a case in a girl, diagnosed by nonoperative means, and a second case in her brother, discovered with a screening in the same family, operated with ultrasonography.
