HIV seroprevalence among military blood donors in Manica Province, Mozambique.

HIV seroprevalence data show an alarming HIV situation in central Mozambique, but little is known about the situation of HIV in Mozambican military personnel. This study is a retrospective analysis of laboratory records for voluntary blood donors at a rural hospital from January 1997 through December 1999. The hospital screened blood samples with HIV SPOT rapid test for HIV and rapid plasma reagin (RPR) serological test for syphilis. Of the 797 blood donors during this period, 110 (13.8%) were military personnel of whom 39.1% were HIV positive (35.0% in 1997, 33.3% in 1998 and 48.7% in 1999). Among the 687 nonmilitary donors 15.3% were HIV positive (P<0.0001 vs military). 74.4% of HIV-positive military personnel were also RPR positive. Conversely, only 3.0% of HIV-negative military donors were RPR positive. In light of the high rates of HIV and syphilis in military personnel, aggressive intervention measures must be taken to prevent and treat HIV and STDs in this population.

[Intra-abdominal abscess as presentation of colonic cancer].

Splenic abscess and retroperitoneal abscess are uncommon, although severe diseases, with a high mortality rate that has been attributed to delayed diagnosis, due to the unspecificity of clinical symptoms. We report two patients with a splenic and a retroperitoneal abscess, respectively, in both cases as an onset of colon cancer. The two patients complained of abdominal pain and fever as onset symptoms. Abdominal ultrasonography was normal in the case of retroperitoneal abscess and abnormal in the case of splenic abscess. CT Scan was diagnostic in both cases. In the patient with splenic abscess. CT Scan established a further diagnostic suspect of colon cancer, which was confirmed by colonoscopy. In the patient with retroperitoneal abscess, diagnosis of colon carcinoma was made during the surgical act. In spite of an adequate, combined medical and surgical therapy, both patients died within a short time after surgery.

[Pulmonary epidermoid carcinoma in a 22-year-old patient with multiple cutaneous metastasis].

The authors present one clinical case of lung epidermoid carcinoma with several concurrent features which are uncommon in relation to this type of neoplasia. Consequently, the age of the patient (22), the lack of known risk factors, the histologic type (little differentiated squamous) and an aggressive biologic behaviour of the tumour, preceded of multiple cutaneous metastasis, are circumstances that make this clinical case exceptional. The more significative aspects present at lung carcinoma included in this age group respect to others are analyzed.

Pollen allergy in the Bilbao area (European Atlantic seaboard climate): pollination forecasting methods.

Forecasting pollination can help the allergist to establish the appropriate treatment and advice for patients. Based on previous studies, we have related the climate variables with the grass pollen counts in the search for pollination predictors. By relating the meteorological data of the temperature recorded every 6 h and of the rainfall in hourly periods, together with the daily pollen counts obtained by the Hirst volumetric system, over a period of 3 years, we have tried to predict the start, duration and severity of the grass pollination, as well as the days of peak pollination. We have established a relationship by means of a polynomic regression originating from the mean cumulated temperature higher than 9 degrees C [R2 = 0.927 (P = 0.0001)], with the pollination season starting from 300 degrees C and the maximum peak at 356 degrees C, in the 3 years of the study. During the days of pollination, peaks higher than 50 grains/m3 coincide with average daily temperatures of 18.7 +/- 3 and lower than 50 grains/m3 with temperatures of 16.8 +/- 3 (significant to 95%). The duration of the pollination is influenced by the cumulated average temperatures (from 800 to 900 degrees C) and especially by precipitation at the start of and during pollination. In order to forecast grass pollination, the cumulated average temperatures are useful, starting from a basal (9 degrees C), pollination begins when this sum is greater than 300 degrees C, whereas when 800 degrees C is reached and depending on the rainfall during the season, pollination will end.(ABSTRACT TRUNCATED AT 250 WORDS)

P300 latency and amplitude in the diagnosis of dementia.

P300 latency and amplitude were investigated in 10 normal elderly individuals, 10 institutionalized elderly persons with dementia not of the Alzheimer's types, and 10 elderly people with Alzheimer's disease. Significant differences between control and dementia groups (but not between dementia groups) were noted in the P300 latency, but not amplitude. Such differential latencies were observed in the anterior and in the left temporal and parietal areas of the brain.

CIRCADIAN LOCOMOTOR ACTIVITY AND ITS ENTRAINMENT BY FOOD IN THE CRAYFISH PROCAMBARUS CLARKI

The aim of our experiments has been to study the effect of light and food in the locomotor activity rhythm of the crayfish Procambarus clarki. Experiments were carried out under light:dark (LD) cycles of 12 h:12 h, under continuous darkness (DD) and under continuous light (LL). Under LD cycles, two peaks of activity were observed during the night phase of the cycle, while resting was characteristic of the day phase. Under DD or LL, it was possible to follow a free-running rhythm with a periodicity of 22.3&plusmn;0.84 h in DD and 24.8&plusmn;0.27 h in LL, typical of circadian rhythms of nocturnal species. A single delivery of food in the day phase of the LD cycle resulted in an outburst of locomotor activity that lasted for several hours. In the ensuing days, an activity peak appeared in phase with the time of food delivery. The food-related activity peak could be followed for up to 2 weeks without food reinforcement. Under DD and LL, food induced an activity rhythm in previously arrhythmic animals. Here the period was longer than 24 h in DD (26.2&plusmn;0.12 h) and shorter in LL (22.5&plusmn;0.46 h). Together, these results strongly suggest that light and food may play a role entraining a locomotor activity rhythm in crayfish.

[Hepatic disease associated with human immunodeficiency virus infection: anatomo-clinical study].

OBJECTIVE: To study the spectrum of liver disease in the infection by the human immunodeficiency virus (HIV) and to assess the usefulness and indications of liver biopsy. PATIENTS: Fifty eight HIV seropositive patients (48 intravenous drugs users) were prospectively studied by means percutaneous liver biopsy, because of hepatic biological alterations, hepatomegaly or fever of unknown origin. RESULTS: Chronic hepatitis was the most common diagnosis, which was found in up to 20 patients (34%) (12 had chronic active hepatitis). Most of them were caused by hepatitis C virus (90%). Hepatic granulomatosis was diagnosed in nine patients (15%), all of them among the subgroup of 19 patients (47%) studied because of fever of unknown origin. Granulomas were thought to be caused by mycobacteria in eight cases (seven tuberculosis, one Mycobacterium avium-intracellulare. Mycobacteria were isolated in culture in four patients whose histologic findings were inspecific. Other representative diagnosis were liver cirrhosis in eight patients, lymphoma in two, alcoholic hepatitis in one and candidiasic hepatitis in one. The diagnostic yield of liver biopsy was 79%. CONCLUSIONS: Liver biopsy is a useful method in the diagnosis of these patients, specially if they present with fever of unknown origin or hepatic biological alterations. In this study chronic active hepatitis was the most common finding, while hepatic granulomatosis was so in patients with fever of unknown origin.

Brain mapping in dysphonemic dyslexia: in resting and phonemic discrimination conditions.

Topographic mapping of EEG bands and alpha asymmetry were analyzed during resting conditions and an auditory phonemic discrimination task (APD) in carefully screened samples of 9.0- to 12-year-old control and dysphonemic-sequencing dyslexic children. The EEG was recorded over the entire scalp, with a linked ears reference. We found: (1) no differences between groups during resting conditions in any frequency band analyzed; (2) a left hemisphere alpha responsiveness in dyslexic children during the APD condition; and (3) a beta 2 decrease in the left posterior quadrant in dyslexics during the APD condition. We conclude that the lower alpha and beta 2 responsiveness may be due to a diminished cortical reactivity and may be related to intrinsic disorganization of the neural processors associated directly with the APD task demands, the alpha activity reflecting poor attention to an external stimuli source and the beta 2 decrease reflecting an unusual processing associated with the linguistic cognitive requirements.

[Acute symptomatic HIV infection. Report of 10 cases].

BACKGROUND: The diagnosis of acute HIV infection must rely in the demonstration of a significant increase in antibody titers against HIV together with the timely development of clinical symptoms. The percentage of cases with symptomatic acute HIV infection is currently unknown. METHODS: We performed a retrospective review of all patients admitted to Internal Medicine and Infectious Diseases department of a General Hospital in a three-year period. The presence of HIV-antibodies was assessed by means of ELISA, indirect immunofluorescent test and Western blot technique. RESULTS: Among a total number of 1110 patients, we found 10 (0.9%) that were considered as having an acute symptomatic HIV infection. Clinical presentation was as follows: mononucleosis-like syndrome, 7 patients; aseptic meningitis, 1 patient; polirradiculoneurophaty and coreoatetosis, 1 patient and esophageal candidiasis, 1 patient. Transmission of infection was heterosexual contact in 60% of cases. Four patients developed a transient skin rash, four had oral candidiasis and low platelet count was detected in 6 cases. The presence of circulating HIV antigen could be demonstrated in 7 patients, and the change in antibody titer was recorded with an 11 +/- 2 weeks span. CONCLUSIONS: The incidence of symptomatic HIV acute infection was only 0.9% in our study. The high number of patients with heterosexual transmission and low platelet counts in our series is remarkable.

Topographic maps of brain electrical activity in primary degenerative dementia of the Alzheimer type and multiinfarct dementia.

The topography of the electroencephalographic (EEG) pattern of ten patients with primary degenerative dementia of the Alzheimer type, ten multiinfarct dementia patients, and ten age-matched controls was compared during three different behavioral conditions: resting condition with eyes open (EO), memorizing a list of words (M), and recalling the same list of words (R). Results indicate that the alpha frequency band does not show significant changes. On the other hand, the theta band could be considered an important factor in the differential diagnosis of the primary degenerative dementia of the Alzheimer type, showing a higher power over right posterior regions in this group of patients compared with the multiinfarct dementia patients under different behavioral conditions.

[Isotopic radioangiography in hepatic cirrhosis. Changes induced by somatostatin].

The value of first step isotopic angiography (FSIA) with 99-TC in the study of altered liver kidney and spleen circulation in patients with liver cirrhosis and alcoholic hepatitis is analyzed. This technique is used to evaluate the effect of a 250 mcg IV bolus of somatostatin on the activity/time curve at the three circulatory levels. The ratio of basal hepatic artery/portal flow in cirrhotics was 59 + 11/41 + 11% and 39 + 6/61 + 4 in alcoholic hepatitis, significantly different from normal controls (p less than 0.001). In cirrhotics the degree of alteration in FSIA correlated with the severity of liver disease. Somatostatin significantly improved the ratio of basel hepatic artery/portal flow and diminished the pressure at the root of suprahepatic veins (18.5 +/- - v.s. 16 +/- mmHg, p greater than 0.001). Neither the slopes nor the time of maximum isotopic activity of renal and splenic activity/time curves were modified. We concluded highlighting the clinical value of FSIA as a non invasive test in the study of chronic liver diseases.
