[Survey of activities of daily living in non-institutionalized elderly residents].

To investigate the states of activities of daily living (ADLs) in the elderly, non-institutionalized residents-aged 65 and over living in Azuchi (population of approximately 12,000) were interviewed. Data was collected from 1,287 subjects (509 men and 778 women; 92.1%) for the 3 years, 1990 to 1992. Differences in difficulties with the 6 basic ADLs (BADLs) and the 7 instrumental ADLs (IADLs) were analyzed according to gender and age. For the BADLs (eating, dressing, bathing, using the toilet, mobility, and incontinence), only the percentage of incontinence was significantly higher in women than in men (p < 0.01). The percentage of those fully independent in the 6 BADLs in men was 87%, which was higher than that in women (77%), but except for incontinence there were no significant differences between the sexes. The percentages of disability in the BADLs for women and for both sexes combined were higher according to age (p < 0.01), but for men, only those of dressing (p < 0.05) and of incontinence (p < 0.01) were related with age. The disability percentage for cooking was the only activity among the IADLs (cooking, using the telephone, cleaning house, walking, shopping, going up and down steps, and using transportation) which was significantly higher in men (p < 0.01), but those for walking, going up and down steps, and using transportation were higher in women (p < 0.01). Stimulatory release of hepatic lipase activity from liver by epidermal growth factor.

We have found that release of hepatic lipase activity is stimulated from liver slices by epidermal growth factor (EGF) into the medium in a time-dependent manner. This novel effect of EGF was markedly decreased by various tyrosine kinase inhibitors, such as alpha-cyano-3-ethoxy-4-hydroxy-5-phenyl-thiomethyl cinnamamide, amiloride and biochanin A. The stimulation by EGF was also suppressed, however, by dibutyryl cyclic adenosine monophosphate, and 3-isobutyl-1-methylxanthine, a cyclic nucleotide dependent phosphodiesterase inhibitor. These findings show that the stimulatory release of the enzyme activity by EGF is associated with the activation of tyrosine kinase activity and with the intracellular cyclic adenosine monophosphate level.

Vanadate-stimulated release of hepatic lipase activity from liver.

Vanadate stimulated the release of rat hepatic lipase activity from liver slices into an incubation medium in a time- and dose-dependent manner. Insulin, however, failed to have this stimulatory action, and the release by heparin was recognized, but was not additive to that by vanadate. Amiloride, an inhibitor of tyrosine kinase in some receptors and of the Na+/H+ exchange system suppressed the vanadate-stimulated release. Biochanin A, a different type of tyrosine kinase inhibitor than amiloride, also suppressed the effect of vanadate. The stimulation by vanadate was clearly preserved in Na(+)-, K(+)-, or Ca(2+)-free medium, suggesting that neither the Na+/H+ exchange system, Na+, K(+)-adenosine triphosphatase, nor Ca(2+)-influx into cells is involved in the action of this substance. These results suggest that vanadate-stimulated release of the enzyme activity is associated with the activation of the tyrosine kinase activity.

Kinetic studies on the cleavage of oligouridylic acids and poly U by bovine pancreatic ribonuclease A.

Kinetic parameters, Km and Vmax for the transesterification of oligouridylic acid, (Up)nU greater than p (n=0-4), by RNase A were measured spectrophotometrically at pH 7.0 and 25 degrees C. The kinetic parameters, pKm and log Vmax increased with increase in the chain length (n), and seemed to be almost constant with substrates having n greater than or equal to 2. The contribution of each subsite to the binding was estimated according to Hiromi's theory. The subsite affinities for (B1, R1, P1)+(B2, R2, P2) and (B3, R3, P3) are 8.03 kcal and 0.72 kcal/mol, respectively, and those for (B4, R4, P4) and (B5, R5, P5) are less than 0.5 kcal/mol. Therefore, we postulate that the size of the RNase A active site is about 3 nucleotides in length. Transesterification of poly U by RNase A was followed spectrophotometrically. The reaction is markedly influenced by ionic strength. At lower ionic strength, the v0-S curve of poly U cleavage was sigmoidal and cooperative, and it became less cooperative at higher ionic strength. Since the estimated Vmax value for poly U cleavage at ionic strength of 0.1 was more than 20 times larger than that of oligouridylic acids cleavage, we propose a non-specific interaction of poly U anion with cationic groups on the surface of the enzyme, modulating the conformation of active site, and thus increasing the activity at low ionic strength. The interaction decreases at higher ionic strength due to the interaction of counter anions with the non-specific sites.

Clinical indication of digital subtraction angiography.

Intravenous subtraction angiography with Technicare DR 960 has been performed in the Department of Diagnostic Radiology and Nuclear Medicine, National Medical Center Hospital. All procedures are based on the non-invasive technique, i.e. with 5 cm length and 19 G. of cannula to the ante-cubital vein, and small amount of contrast material of half ml per kg of body weight kg as maximum dosage for pediatric patients. Image quality was evaluated by visual classification such as excellent, good, fair and poor. Among 335 procedures, 303 cases have brought useful diagnostic informations. 156 cases were excellent and good (47%). There were several specialized informations which might be worthwhile for DSA evaluation, i.e. 1. overlapping images, 2. static dynamic images, 3. density controlled image analysis, 4. three vessel visualized images and 5. well demarcated tumor images of extremities. Since the halo image around the true lumen of aneurysma suggested the outline of the false lumen, this phenomena might indicate the aneurysmal wall thickness. Poor risk patients and outpatients have been selected as our DSA candidates. The patient who is indicated for catheter technique angiogram were not examined by DSA.
