Fluorescent demonstration of the Paneth cell granules.

Blancofor BA produces a fluorescent staining of Paneth cell granules. It is very likely that this staining is related to the presence of a basic protein in the core of the granule. None of the other types of intestinal cells showed a fluorescent reaction with this substance.

[Intra-abdominal foreign bodies following laparotomy interventions: report of a case].

The authors report their own observation of a case of post-laparotomic endoabdominal foreign body, which for a long period had remained asymptomatic. Starting from the clinical case, they effect a short study of such iatrogenic pathology, and state the diagnostic and therapeutic criteria thereabout. They subsequently dissertate on the possible preventing means.

[Colo-vesical fistulas in acute pathology of the colon].

A survey of the relevant literature and a statement of the acute diseases of the colon giving rise to urological complications are followed by a short account of the subjective and objective symptoms of colo-vesical fistulae. An assessment is also made of the advantages of careful diagnostic, clinical and instrumental appraisal. The classic management of such fistulae is described, and the main features of the surgical technique required are illustrated. Successful employment of total parenteral therapy in 13 cases of enteric fistula (including 2 colo-vesical cases) at the University of Genoa "R" Surgical Clinic is reported. The indications, contraindications and complications associated with this method are explained. The modalities whereby a gradual transition is made to feeding per os are described and the results obtained are evaluated.

The rationale for a study on HIV-1 reverse transcriptase mutations and outcome of antiretroviral therapy with two nucleoside analogs.

The development of resistance to antiretroviral drugs has been recognized as an important cause of treatment failure in HIV-1-infected patients; however the correlation between emergence of resistance and treatment failure has not been yet clearly defined. The high rate of viral replication, together with the lack of proof reading activity of HIV-1 reverse transcriptase, accounts for the rapid establishment of extensive genotypic variation, resulting in the emergence of viral mutants showing primary or secondary resistance to antiretroviral drugs. In this regard, phenotyping and genotyping allow the detection of drug resistance. Both tests have advantages and limitations compared to each other. The complete suppression of viral replication seems to be the best way to avoid occurrence of drug resistance, and viral loads below the limit of detection can be usually achieved by a combination of 3 antiretroviral drugs. In this scenario, the proportion of HIV-1-infected patients on dual therapy is still relevant in Italy. We believe that the study of this subset of individuals is very important, as resistance to nucleoside analogues may impair the outcome of a future triple therapy. In addition, this study could contribute to define the role of resistance assays in the management of HIV-1-infected patients.

[Estimate of underreporting of infectious diseases through a sentinel network of pediatricinas in the area of local health unit of Florence].

The aim of this study was to evaluate the underreporting of some infectious diseases in the pediatric population in the Local Health Unit of Florence in the period 15.09.1997-14.09.1998. Data from the current notification system and from an a hoc sentinel network were used. Nine family pediatricians voluntarily participated in the sentinel network, notifying all cases of measles, mumps, pertussis, rubella, scarlet fever and chickenpox diagnosed in the population of children under their professional responsibility, in the period of the study. Chickenpox was the most frequent disease (2,043 cases equal to 73.5% of total notifications). The notification rate for chickenpox obtained with the sentinel network was 41.6 per 1,000 children, meanwhile the notification rate obtained with the current notification system was 23.7 per 1,000. The notification rate for scarlet fever was 24.1 per 1,000 with the sentinel network and 6.0 per 1000 with the current notification system. The underreporting for the two diseases was respectively 43% and 75%. Voluntary participation of physicians in sentinel network guarantees data of good quality, making these networks very useful tools for the epidemiologic evaluation of infectious diseases with benign prognosis.

[Inguinal hernia containing bladder and ureter. A clinical case].

The authors describe an uncommon case of inguinal hernia with bladder and ureter content. Bladder herniation preoperative diagnosis has been achieved by means of clinical history, objective and instrumental examination (cystography). As usually happens, ureteral herniation was a chance finding; this could involve a trick in surgery setting up and doubts in the treatment methods.
