[Ultrasonics in the diagnosis of choledocholithiasis].

The value of ultrasound in the diagnosis of choledocholithiasis is controversial, sensitivity figures ranging from 22 to 75%. We correlated the ultrasound study with operative findings in 38 patients. A diagnosis of common bile duct stone was made upon finding an echogenic image with or without acoustic shadow in the common bile duct; no attention was paid to the dilatation of the biliary tree. The sensitivity was 68% and the positive predictive value was 85%. Therefore, our results approach the highest reported figures for the value of this diagnostic technique.

Study of the hydrolysis and ionization constants of Schiff base from pyridoxal 5'-phosphate and n-hexylamine in partially aqueous solvents. An application to phosphorylase b.

Formation and hydrolysis rate constants as well as equilibrium constants of the Schiff base derived from pyridoxal 5'-phosphate and n-hexylamine were determined between pH 3.5 and 7.5 in ethanol/water mixtures (3:17, v/v, and 49:1, v/v). The results indicate that solvent polarity scarcely alters the values of these constants but that they are dependent on the pH. Spectrophotometric titration of this Schiff base was also carried out. We found that a pKa value of 6.1, attributed in high-polarity media to protonation of the pyridine nitrogen atom, is independent of solvent polarity, whereas the pKa of the monoprotonated form of the imine falls from 12.5 in ethanol/water (3:17) to 11.3 in ethanol/water (49:1). Fitting of the experimental results for the hydrolysis to a theoretical model indicates the existence of a group with a pKa value of 6.1 that is crucial in the variation of kinetic constant of hydrolysis with pH. Studies of the reactivity of the coenzyme (pyridoxal 5'-phosphate) of glycogen phosphorylase b with hydroxylamine show that this reaction only occurs when the pH value of solution is below 6.5 and the hydrolysis of imine bond has started. We propose that the decrease in activity of phosphorylase b when the pH value is less than 6.2 must be caused by the cleavage of enzyme-coenzyme binding and that this may be related with protonation of the pyridine nitrogen atom of pyridoxal 5'-phosphate.

Influence of pH on the removal of pyridoxal 5'-phosphate from phosphorylase b.

A method to break the pyridoxal 5'-phosphate (PLP)-phosphorylase b bond using hydroxylamine and slightly acid pH is put forward and described in the present paper. This method does not involve drastic conditions or deforming reagents. The influence of pH and protein concentration on the removal of PLP from phosphorylase has also been studied, resulting in an order of -0.3 with respect to the enzyme, a value that implies a complex reaction. An additional conclusion is that an increase in the protein concentration entails better protection of the enzyme from attack by hydroxylamine.

Subdural hematomas of the posterior fossa in normal-weight newborns. Report of two cases.

Two cases are reported of subdural hematoma of the posterior fossa in normal-weight full-term newborn infants. The most salient factors in these cases were the lack of specific symptoms and signs indicating the nature and location of the lesion, the importance of computerized tomography for diagnosis, and the good results obtained with early surgical treatment.

Rate and equilibrium constants of the tetramerization process of phosphorylase b. Influence of AMP and Mg2+.

The association-dissociation equilibrium of phosphorylase b at different enzymatic concentrations has been studied at 25 degrees C in this paper, pointing out how this equilibrium is affected by both AMP and Mg2+ concentrations. It has also been proved that association of phosphorylase b in the presence of AMP and Mg2+ follows second-order and first-order rate laws in the direction of tetramerization and dimerization, respectively. Rate constants have also been calculated and their dependence upon protein, AMP and Mg2+ concentrations studied thoroughly.

Kinetic and thermodynamic study of the tetramerization equilibrium of phosphorylase b.

The association equilibrium of phosphorylase b, induced by AMP and in the presence of Mg2+, has been shown to be a reversible process that follows second order and first order reversible rate laws in the direction of tetramerization and dimerization respectively, this fact being independent of temperature and of enzyme and AMP concentrations. Moreover, rate and equilibrium constants have been evaluated and their dependence on temperature and AMP concentration studied in this work. An important role that the existence of two classes of AMP binding sites per enzymatic subunit plays in the aggregation properties of the enzyme has also been emphasized. In the presence of 0.1 and 1 mM AMP (binding to the high affinity site), the values of the change in enthalpy, activation energy of dimerization and activation energy of tetramerization are: -36 kcal/mol, +36 kcal/mol, and 0 kcal/mol respectively. Binding of AMP to the low affinity site (10 mM AMP) yields significant changes in the self-association equilibrium, since the preceding parameters reach the following values: -18, +32, and +14 kcal/mol.

Method of assay for 4-aminobutyrate-2-oxoglutarate aminotransferase.

In this paper an analytical colorimetric method is presented where 2,4-dinitrophenyl hydrazine is used to measure small amounts of succinic semialdehyde in the presence of 2-oxoglutarate. This method is applicable to the measurement of 4-aminobutyrate-2-oxoglutarate aminotransferase activity, where succinic semialdehyde formed during the enzymatic reaction has to be measured in the presence of 2-oxoglutarate.

Dose-dependent activity of albendazole against benzimidazole-resistant nematodes in sheep: relationship between pharmacokinetics and efficacy.

The relationship between the pharmacokinetic behaviour and the anthelmintic efficacy of albendazole (ABZ) against benzimidazole (BZD)-resistant nematodes was studied in sheep. A micronized ABZ suspension was orally administered at two different dose levels to sheep naturally infected with BZD-resistant gastrointestinal (GI) nematodes. The experimental animals were allocated into the following groups (n = 8): (a) untreated control; (b) orally treated with ABZ at 3.8 mg/kg b.w.; and (c) orally treated with ABZ at 7.5 mg/kg b.w. Plasma samples were obtained serially over 72 h post-treatment from both treated groups and analysed by HPLC to measure the concentrations of ABZ and its sulphoxide (ABZSO) and sulphone (ABZSO(2)) metabolites. Faecal egg counts were performed prior to treatment and at the necropsy day. All experimental animals were sacrificed 10 days after treatment to perform GI worm counts. While ABZ parent drug was not recovered in the bloodstream, ABZSO and ABZSO(2) were the molecules found in plasma. ABZSO was the metabolite measured at the highest concentrations in the bloodstream for up to 36 (treatment at 3.8 mg/kg) or 60 h (treatment at 7.5 mg/kg) post-administration. There was a proportional relationship between the administered ABZ dose and the measured plasma concentrations of both ABZ metabolites. Over a 100% increment on the plasma AUC values for the anthelmintically active ABZSO metabolite was observed at the 7.5 mg/kg compared to the 3.8 mg/kg treatment. The low efficacy patterns (< 24%) observed against the GI nematodes investigated indicate a high level of resistance to ABZ given at 3.8 mg/kg an efficacious therapeutic dose rate recommended in some countries. However, the higher and prolonged plasma drug concentration measured after the 7.5 mg/kg treatment resulted in an improved efficacy pattern (estimated by both faecal egg and adult worm counts) against most of the GI nematodes studied compared to that obtained at the lower dose rate. A direct relationship between drug pharmacokinetic behaviour and anthelmintic efficacy against BZD-resistant nematodes in sheep was shown in the current work, although individual variation precluded the observation of statistically significant differences in worm counts.

[Sublingual immunotherapy in children. Immunotherapy Committee of the Spanish Society for Clinical Immunology and Pediatric Allergology].

Sublingual immunotherapy is currently attracting growing interest because of its ease of administration and, according to previous studies, its infrequent and mild adverse effects. However, at least in children, the efficacy of this therapy has not been completely demonstrated. In addition, the mechanisms of action remain to be elucidated since few studies have been published and the results have been contradictory and sometimes inconclusive. For this reason, we performed a literature review through the MEDLINE database, selecting double-blind studies carried out in children. Only 10 studies meeting these requirements were retrieved. All the studies were performed by European researchers and nine were published in European journals. Efficacy was evaluated by clinical parameters and by reduction in medication use. The results on efficacy are not homogeneous, although most support the utility of this route of administration. Moreover, reports of allergens other than those used in these studies dust mites and grass pollens are lacking. In conclusion, further studies evaluating the efficacy of this therapy in children are required. Among the general population, if the efficacy of sublingual immunotherapy in the treatment of sensitization to hymenoptera venoms were demonstrated, as has been the case with subcutaneous immunotherapy, the utility of this route of administration would be definitively confirmed. Finally, sublingual immunotherapy could be used in children who have shown systemic reactions to subcutaneous immunotherapy or who refuse to undergo injections.

Canine distemper in a genet (Gennetta gennetta), associated with endogenous lipid pneumonia.

This report describes the histological and immunohistochemical findings in an adult male genet (Gennetta gennetta) which died a few hours after being found lying in a forest in Lugo (north-western Spain). Subpleural, yellowish, firm foci were found in the lung. Histopathological examination confirmed a diagnosis of endogenous lipid pneumonia. Microscopical lesions that gave rise to a suspicion of canine distemper virus (CDV) infection included lymphoid depletion, non-suppurative encephalitis and demyelination in the central nervous system, and the presence of inclusion bodies in renal tubules. Immunohistochemical examination was performed with the streptavidin-biotin-complex method and a monoclonal antibody against the nucleocapsid protein (NP) of CDV. Antigen was detected in epithelial, nervous and lymphoid cells in several organs. This would appear to be the first report of distemper-like infection in a genet.

Energy evaluation of extruded compound foods for dogs by near-infrared spectroscopy.

Near-infrared reflectance spectroscopy (NIRS) was used to predict the chemical composition, apparent digestibility and digestible nutrients and energy content of commercial extruded compound foods for dogs. Fifty-six foods of known chemical composition and in vivo apparent digestibility were analysed overall and 51 foods were used to predict gross energy digestibility and digestible energy content. Modified partial least square calibration models were developed for organic matter (OM), crude protein (CP), ether extract (EE), crude fibre (CF), nitrogen free extracts (NFE) and gross energy (GE) content, the apparent digestibility (OMD, CPD, EED, NFED and GED) and the digestible nutrient and energy content (DOM, DCP, DEE, DNFE and DE) of foods. The calibration equations obtained were evaluated by the standard error and the determination coefficient of cross-validation. The cross-validation coefficients of determination (R) were 0.61, 0.99, 0.91, 0.96, 0.94 and 0.92 for OM, CP, EE, CF, NFE and GE, the corresponding standard error of cross-validation (SECV) being 5.80, 3.51, 13.35, 3.64 and 16.95 g/kg dry matter (DM) and 0.29 MJ/kg DM respectively. The prediction of apparent digestibility was slightly less accurate, but NIRS prediction of digestible nutrient (g/kg DM) and DE (MJ/kg DM) gave satisfactory results, with high R (0.93, 0.97, 0.93, 0.83 and 0.93 for DOM, DCP, DEE, DNFE and DE respectively) and relatively low SECV (11.55, 6.85, 12.14 and 22.98 g/kg DM and 0.47 MJ/kg DM). It is concluded that the precision of NIRS in predicting the energy value of compound extruded foods for dogs is similar or better than by proximate analysis, as well as being faster and more accurate.

[Essential functions of public health: emerging topic in health care reform].

In the Americas, health sector reforms are facing the challenge of strengthening the steering and leadership role of health authorities. An important part of that role consists of fulfilling the essential public health functions (EPHFs) that are incumbent on all levels of government. For that, it's crucial to improve public health practice, as well as the instruments used to assess the current state of public health practice and the areas where it needs to be strengthened. For that purpose, the Pan American Health Organization has started an initiative called "Public Health in the Americas," with the objective of defining and measuring EPHFs, as a way of improving public health practice and strengthening the leadership provided by health authorities at all levels of government. This article summarizes conceptual and methodological aspects of defining and measuring EPHFs. The article also analyzes the implications that measuring performance on these public health roles, responsibilities, and activities would have for improving public health practice in the Americas.

[Diagnostic-therapeutic approach to internal jugular thrombosis].

The internal jugular vein thrombosis is a rare and critical disease. The infection of the head and neck deep spaces it is not the main cause since the antibiotic era. The ENT neoplasms and iatrogenic etiology are more frequent. Anodyne symptoms and vital complications are important to obtain an early diagnose and therapy, to improve the prognosis. The objective of this review is to obtain a certainly therapeutic criteria in the approach to this entity.
