Expression of apoptotic and cell proliferation regulatory proteins in actinic cheilitis.

BACKGROUND: Actinic cheilitis (AC) is a pre-malignant lesion caused by ultraviolet (UV) radiation. The apoptotic proteins p53, bax, bcl-2, and the proliferation marker Ki-67, are known to play an important role in UV-exposed skin and carcinomas, therefore, these markers were assessed in AC and compared with normal lip and oral mucosa. METHODS: AC (n = 13), normal lip (n = 7) and oral mucosa (n = 6) biopsies were stained immunohistochemically for p53, bax, bcl-2 and Ki-67, to determine their expression and distribution. RESULTS: p53 was over-expressed in AC as compared with normal lip and oral mucosa (P < 0.003). Although bcl-2 expression was higher in AC than in oral mucosa (P < 0.002), it was significantly reduced as compared with normal lip (P < 0.04). Bax expression remained unchanged, and Ki-67 was significantly increased in AC and normal lip as compared with oral mucosa (P < 0.05). CONCLUSION: The results suggest that DNA-damaged cells by UV radiation in AC are eliminated by apoptosis.

[Results of combined treatment of early cervicouterine cancer].

Forty nine patients with carcinoma of the cervix treated with a combined method of preoperative radiation therapy (telecobalto and intracavitary) followed by extrafascial hysterectomy are presented. The average age was 39.6 years old. All tumors measured more than 4 cm, in diameter. The staging was lb = 40; lla = 6; llb = 3. The hysterectomy was done average 7.3 weeks after radiation therapy. Postreatment complications were seen in 32.4% and long term complications were found in 8.1% of the cases. After three years of follow up 91% of the patients were free of neoplastic disease. The 40% of the cases has more than three years follow up. There was 14% of total mortality which includes 10.2% of mortality by cancer. The size of the tumor (initially more than 7 cm, in diameter) and the finding of the tumor tissue in the surgical specimens were found of poor prognosis. There was significant statistical difference in survival between the groups with and without cancer cells in the uterus after hysterectomy (p < 0.01).

[Uterine leiomyoma: pathology and epidemiology].

Two hundred biopsy reports with uterine myoma histopathological diagnosis were studied 67% of the cases were between 41-50 years of age. Premenopausic patients showed an average number of myomas almost three times higher than the postmenopausic women. The most frequent myoma were those within the walls with a size less than 2 cm. Chronic inespecific cervixitis was the most frequent finding in the cervix pathology, with in 1.5% of the cases. In 2 cases (1%) endometrial adenocarcinoma was found. In 60.5% of the cases one or both tubes and ovaries were removed neither case had malignant alterations. In 51 cases (25.2%) the myoma presented some type of degeneration, the most frequent being the cellular type, 16 out of 200, which represented 8% of the cases; it was considered as presarcomatose type. Presurgical clinical behavior is discussed considering the analysis of these figures and the perspectives of the conservative treatment with the use of new no ablative technique.

[Minimal deviation adenocarcinoma of the uterine cervix].

A 33 year-old female with minimal deviation adenocarcinoma of the endocervix is presented. In two years five biopsies were needed to make the diagnosis. There was association with bilateral mucinous cystadenomas of the ovary. Even under radiotherapy and surgical resection the clinical outcome was fatal with 2 years of survival rate. The tumor shows fairly well differentiated endocervical glands which may be confused with different forms of endocervical hyperplasia but the clinical course is aggressive and fatal.

[Single umbilical artery: review of 12 cases].

The umbilical cord normally consists of three vessels, two arteries and one vein. The absence of one umbilical artery or single umbilical artery (SUA) has been associated with some adverse perinatal events, such as low birth weight, prematurity, congenital malformations and perinatal mortality. The authors present 12 cases of SUA confirmed by histologic examination and the principal maternal and perinatal features of these cases are analyzed. In our study there were a prematurity rate of 58%, a low birth weight rate of 66% and a perinatal mortality rate of 580/1000. Among our patients, 42% of the infants with SUA had major congenital malformations. An accurate examination of the umbilical cord at delivery is suggested, mainly because SUA is an important index to detect associated congenital malformations.

Effects of sodium tetradecyl sulfate endoscopic variceal sclerotherapy on the esophagus. A prospective clinical and histopathologic study.

A prospective clinical, endoscopic, and histopathologic study of the esophagus was carried out in 24 patients with advanced liver disease who underwent esophageal variceal sclerotherapy (EVS) and who eventually came to autopsy. Patients were arbitrarily divided into three groups: acute (group I), intermediate (group II), and chronic (group III) based on the interval between the first EVS and death. EVS with sodium tetradecyl sulphate (STS) initially produced thrombosis with varying degrees of necrosis and inflammation followed by ulceration, recanalization, and eventually fibrosis with obliteration of varices. Recurrent variceal hemorrhage (VH) leading to death was highest in the acute group since all patients died of uncontrollable VH (100%); it ranged between 50-60% in both the intermediate and chronic groups. Despite variceal obliteration, recurrent hemorrhage developed in the chronic group due to gastric varices or other venous channels in the esophagus or stomach. Additionally, we describe findings not previously reported, such as the presence of sclerosant outside the varices after intravariceal injection, thrombosis of gastric varices after esophageal injections, and the development of muscular wall thickening.

Immunoperoxidase study on adenomatoid tumor of the epididymis using anti-mesothelial cell serum.

The mesothelial origin of epididymal adenomatoid tumor is supported by the current study for the presence of mesothelial antigen in the tumor cells. By an indirect immunoperoxidase technique, anti-mesothelial cell serum showed positive cytoplasmic staining in five of the six adenomatoid tumors studied. Previous findings of the presence of strong cytoplasmic keratin and the absence of carcinoembryonic antigen and Factor VIII-related antigen in these tumor cells are also confirmed by this study.

Adenomatoid tumor of the epididymis. Ultrastructural study of three cases.

Three cases of Adenomatoid Tumor of the Epididymis are presented. In these cases the ultrastructural and histochemical study is similar. The adenoid structures of the tumor were lined by epithelial cells with well differentiated microvilli, junction complexes and intercellular channels. The stroma was formed of bundles of collagen and elastic fibers. Acid mucopolysaccharides were found in the tubular structures of the tumor. This study supports the mesothelial origin of the Adenomatoid Tumor.
