[Asymptomatic forms of the Severe Acute Respiratory Syndrome (SARS)].

INTRODUCTION: In November 2002 an epidemic of atypical pneumonia appeared in the Chinese region of Guandong. It was subsequently given the name "Severe Acute Respiratory Syndrome" (SARS) following the occurrence of further outbreaks in Hanoi and Hong Kong in February 2003. Five cases related to the same patient source have been reported in France. CASE REPORT: We report the case of a patient of 52 years who had direct contact with a probable case of SARS. On admission the patient presented with pyrexia associated with lymphopenia and liver cell necrosis but no respiratory symptoms. The chest x-ray was normal. The thoracic CT scan showed a sub pleural ground glass appearance. Early (36 hours) PCR studies of nasopharyngeal washings were negative for the coronavirus associated with SARS. The diagnosis was confirmed serologically. CONCLUSIONS: This observation demonstrates the existence of incomplete clinical presentations of SARS. The infectivity of this asymptomatic form is unknown. Serological analyses will allow better identification. The continuing danger of seasonal recurrence, particularly in the winter, cannot be ignored. In this context the recognition of probably infectious asymptomatic forms is essential.

Dynamic properties of an oriented lipid/DNA complex studied by neutron scattering.

The formation of lipid-DNA (CL-DNA) complexes called lipoplexes, proposed as DNA vectors in gene therapy, is obtained by adding DNA to a solution containing liposomes composed of cationic and neutral lipids. The structural and dynamic properties of such lipoplexes are determined by a coupling between the electrostatic interactions and the elastic parameters of the lipid mixture. An attempt to achieve a better understanding of the structure-dynamics relationship is reported herein. In particular, an elastic neutron scattering investigation of DOTAP-DOPC (dioleoyl trimethylammonium propane-dioleoyl phosphatidylcoline) complexed with DNA is described. Proton dynamics in this oriented CL-DNA lipoplex is found to be strongly dependent upon DNA concentration. Our results show that a substantial modification of the membrane dynamics is accompanied by the balancing of the total net charge inside the complex, together with the consequent displacement of interlayer water molecules.

Co-rebinding in myoglobin as seen by time-resolved X-ray absorption spectroscopy.

The dynamics of selected conformational coordinates, key roles in the understanding of the CO-rebinding process, are investigated in horse heart carbonmonoxy myoglobin (MbCO) through time-resolved X-ray absorption spectroscopy. We present here the results obtained at 90 K in the second time scale. The approach of the CO molecule towards the Fe atom in the active site pocket is speculated to act as a natural precursor to the Fe displacement with the consequent undoming of the protein porphyrin plane. The arrangement of the Fe-C-O bonding angle geometry follows and the final MbCO active site configuration is completely reached within 1 min.

[Bronchiolitis obliterans organizing pneumonia or BOOP. Report of a new case, reflections on the concept of BOOP].

A new case of bronchiolitis obliterans with organizing pneumonia (BOOP) is reported. The clinical, radiographical and histological characteristics of the BOOP are reviewed. It is a rare affection which has been described for a long time but the nosological situation of which among diseases of bronchioles has been only recently established. It corresponds to an unspecific inflammatory answer to a pulmonary aggression and can therefore be observed in numerous circumstances. It may represent a model for the understanding of the mechanisms of pulmonary fibrosis.

Light induced states in MbCO denatured with guanidine hydrochloride.

We present the results of a comparative study of the binding of carbon monoxide to myoglobin in glycerol/buffer solution with different concentrations of guanidine hydrochloride, under extended illumination over the temperature range 30-80 K. The changes in the Soret band indicate that the folding state of the protein is a key parameter in determining the photodissociation process and the relaxation rate of the protein.

[Endobronchial granular cell tumor. A report of two cases in the course of staging of ORL cancers].

We report two new observations of endobronchial granular cell tumor discovering by bronchoscopy, during the staging of head and neck cancers. This site remains exceptional; the histologic features are well known. The ultrastructural and especially the immunohistological criteria trend to indicate a neurogenic histogenesis. Although the course is benign in this site and no malignant forms have been reported, local relapses are possible. However, some benign cases may have disturbing endoscopic and histologic features.

[Phrenic nerve malignant schwannoma: an exceptional diagnosis].

We report a case of malignant schwannoma of left phrenic nerve in a 65-year-old man. This uncommon diagnosis was suggested by radiological features and confirmed by histological findings on a needle biopsy and surgery specimens. We discuss management, follow-up and prognosis of malignant schwannomas.

[Osteochondroplastic tracheobronchopathy: 5 cases].

Five new cases of tracheobronchopathia osteochondroplastica (TBOCP) are reported and clinical, radiological and pathological features are reviewed. TBOCP is a rare disease characterized by cartilaginous and bony nodules lining the mucosa of the trachea and major bronchi. The endoscopic features realize pathological evolution by stadiums. The prognosis is usually favourable. The etiology of TBOCP remains unknown, even though some cases were reported in association with amyloidosis.

Aluminum site structure in serum transferrin and lactoferrin revealed by synchrotron radiation X-ray spectroscopy.

The Al site structure of serum transferrin and lactoferrin is investigated using X-ray absorption near edge structure (XANES) spectroscopy. Al K-edge spectra in the mono- and dialuminum forms of the proteins have been recorded for the first time. Our results show that the aluminium ion is hexa-coordinated in an octahedral-like symmetry and that the monoaluminum form, where only the C-terminal binding site is saturated, has an increased structural distortion around the metal site.

[Clinical aspects and diagnosis of sulfite intolerance. Apropos of 9 patients].

We performed an oral tolerance test for potassium metabisulfite in 33 patients and discovered sulfite intolerance in 9. These 9 patients had rhinitis including 7 with asthma. Alcoholic beverages, especially champagne, were the triggering factors the most frequently found for respiratory manifestations. Alcoholic beverages triggered bronchial or nasal reactions in 7 patients out of 9 (rhinitis in 7, asthma in 2). As in a large number of published cases, sulfite intolerance was evidenced by respiratory manifestations in our patients. Exceptional anaphylactic reactions have also been reported. Respiratory intolerance to sulfites in uncommon. A nasal or bronchial reaction occurring within minutes following ingestion of alcoholic beverages is highly suggestive of sulfite intolerance as is the development of acute asthma after administration of sulfite-containing drugs. Diagnosis is confirmed by oral tolerance tests versus placebo. The only effective preventive measure is to eliminate food and drugs containing sulfites. Such measures are justified to prevent acute episodes of asthma.
