The S-bladder, an ileal bladder substitution.

Low pressure continent urinary diversions with abdominal wall stomas alike the Kock-pouch have shown good results. However, if urethral function can be saved without risk, an anastomosis of a bowel bladder to the urethral stump is more favorable in regards of continence, ease of the procedure and cosmetics. The S-bladder developed on the basis of the S-pouch from Parks, is conceptualized to be a complete replacement of the lower urinary tract, offering a low pressure reservoir, safe antireflux mechanisms, continence and voiding per urethra with abdominal straining. 52 patients (46 males and 6 females) were treated with an S-bladder. Continence in daytime and nighttime was achieved in 44 patients, while 49 patients remain continent during the day. To achieve this result 16 of our patients had to be treated with an artificial sphincter implanted at the bulbous urethra. Eleven of these 16 patients suffered from neurogenic bladder disease with preoperative urethral incompetence. Indications was TCC of the bladder in 38, neurogenic bladders in 11 and low-compliance bladders in 3 patients. The complication rate of 13.5% was acceptable. Emptying by abdominal straining was possible in all patients. Residuals were less than 50 cc, even in those patients with artificial sphincters.

Inhibition of urokinase activity and prevention of urokinase receptor binding by monoclonal antibodies.

Two murine monoclonal antibodies produced against human urokinase-type plasminogen activator were characterized with respect to their antigen-binding specificity and their effects on urokinase activity and urokinase receptor binding. One of the antibodies binds to the protease domain of urokinase (Kass = 2.1 X 10(7) M-1). Antibody binding inhibits catalysis of plasminogen activation. It does not, however, affect amidolytic activity of urokinase towards the chromogenic substrate D-Val-Leu-Arg-p-nitroanilide. The antibody thus appears to interfere with plasminogen binding without directly affecting catalytically active amino acid residues of the enzyme. The other antibody binds to the aminoterminal fragment of urokinase (Kass = 1.0 X 10(7) M-1) and prevents binding of the enzyme to high affinity receptors on human granulocytes. Binding of this antibody neither influences plasminogen activation nor the amidolytic activity of urokinase. Both antibodies are potentially useful for the further analysis and manipulation of urokinase function.

Biological activities of synthetic peptides of the sequence of human interferon-alpha.

Theoretical and experimental data were obtained indicating a possible role of the amino acid sequence 69-80 of HuIFN-alpha for antiviral activity of interferon. In this paper, synthetic peptides (penta-, octa- and dodecapeptides), representing the sequence region of positions 69-80 of the primary structure of HuIFN-alpha-2 were investigated with regard to their ability to inhibit the cytopathic effect of VSV on MDBK cells and on embryonic human fibroblasts. The peptides themselves exhibit only a very weak antiviral activity; however, it was found that these peptides even increase the antiviral activity of HuIFN-alpha-2.

Meshgraft urethroplasty using split-thickness skin graft.

Meshgraft urethroplasty has become one of the standard operative procedures for the treatment of long and complicated urethral strictures. The original method used meshed foreskin for urethral reconstruction. To extend the application of the method to circumcized patients, a split-thickness skin graft was used to construct a neo-urethra. In a first stage, a split thickness skin graft is harvested from the medical part of the thigh and transplanted alongside the opened urethra. After complete healing of this transplant, the neo-urethra is formed in a second stage 12 weeks later. Since 1980 meshgraft urethroplasty using a split-thickness skin graft has been performed in 34 patients. In all patients excellent anatomic and functional results have been achieved. This technique was found to be most useful in exccedingly long or problematic strictures, e.g. in spinal cord-injured patients.

[SEM studies on IFN-receptor dynamics of cultured cells from mammals].

Visualization of rIFN-alpha 1 and -alpha 2 receptors on MDBK-cells was done by means of a two step method with gold-labelled monoclonal anti IFN-antibodies in the SEM. This binding showed a marked dependency from the temperature, concentration of the rHu-IFN, prefixation of the MDBK-cells, and time of incubation. This results are in agreement with findings of classical virological methods. By means of SEM was shown that the distribution and amount of the IFN-receptors on MDBK-cells depends strongly on the cell cycle and that not each was labelled. The results were discussed in connection with structural-functional alterations of the cytoskeleton.

Immunoelectron microscopic studies on the location of ribosomal proteins on the surface of the 40S ribosomal subunit from rat liver.

Seven ribosomal proteins have been localized by means of immunoelectron microscopy on the surface of the 40S ribosomal subunit from rat liver using monospecific antibodies. The location of ribosomal proteins S13/16, S19, and S24 is described for the first time, and that of ribosomal proteins S2, S3, S3a, and S7, which has been published previously on the basis of experiments performed with less well characterized antibody preparations [Lutsch et al., Mol. Gen. Genet. 176, 281-291 (1979) and Biomed. Biochim. Acta 42, 705-723 (1983)], is corrected in this paper. The results are discussed with respect to the involvement of these proteins in functional sites of the 40S ribosomal subunit.

Kock pouch and S bladder: 2 different ways of lower urinary tract reconstruction.

Many attempts have been made to develop a continent form of urinary diversion. Encouraging results have been obtained with the Kock pouch, which offers a low pressure reservoir, safe reflux protection and a reliable continent abdominal wall stoma. If urethral function could be saved without risk, anastomosis of a bowel bladder to the urethral stump is more favorable as far as cosmetics and continence are concerned. The S bladder, developed on the basis of the S pouch, is conceptualized to be a complete replacement of the lower urinary tract that offers a low pressure reservoir, safe antireflux mechanisms, continence and voiding per urethram with abdominal straining. We treated 78 patients with continent urinary diversion (Kock pouch in 46 and S bladder in 32). Continence was achieved in 87% of the patients treated with a Kock pouch, while 93.8% were continent in the S bladder group. In an effort to achieve continence day and night 43.8% of the patients treated with an S bladder required an artificial sphincter. The over-all complication rate (12.5 compared to 41.8%) and the operative complication rate (6.3 compared to 23.9%) were significantly lower in patients treated with the S bladder. Emptying by abdominal straining was possible in all patients with an S bladder. Residuals were less than 50 cc, even in patients with an artificial sphincter.

Mesh graft urethroplasty using split thickness skin graft or foreskin.

Long urethral strictures remain one of the hazards of modern urology. Reconstructive operations with scrotal skin suffer a high rate of recurrent stricture. To avoid complications, meshed split thickness skin graft or foreskin was used to construct a neourethra. In stage 1 split thickness skin graft is harvested and transplanted along the opened urethra. In stage 2 the neourethra is formed 8 to 12 weeks later. Since 1977 mesh graft urethroplasty has been performed in 96 patients using meshed foreskin (76) or split thickness skin grafts (23). In all but 1 patient excellent anatomical and functional results were achieved regardless of which type of graft was used. This technique was most useful in exceedingly long or problematic strictures, for example in spinal cord injury patients.

The late occurrence of urinary tract damage in patients successfully treated by radiotherapy for cervical carcinoma.

Urinary tract complications apparently resulting from radiation therapy for carcinoma of the cervix can occur as long as 30 years after cessation of such treatment. Patients generally present with urinary incontinence and often are treated by standard operative methods that usually are unsuccessful. Incontinence is related to bladder fibrosis, urethral nonfunction and vesicovaginal fistuLa formation, and may be accompanied by bilateral ureteral obstruction. Of 11 patients with late complications of radiotherapy 4 had upper tract deterioration, 4 had vesicovaginal fistulas, 5 had an incompetent urethra aNd 9 had a fibrotic, noncompliant areflexive bladder. Treatment was aimed at providing adequate low pressure storage capacity and consisted of augmentation cystoplasty in 5 patients, repair of the fistula in 4 and correction of urethral dysfunction in 5. Women who complain of incontinence and/or irritable bladder symptoms with a history of radiotherapy for cervical carcinoma should be evaluated for fistuLa formation, urethral incompetence, and detrusor areflexia and fibrosis before treatment is done.

Dipeptidyl peptidase IV in human T lymphocytes. Impaired induction of interleukin 2 and gamma interferon due to specific inhibition of dipeptidyl peptidase IV.

Specific inhibitors of the membrane-bound dipeptidyl peptidase IV (DP IV) and polyclonal antibodies against this enzyme were used to investigate the relationships between DP IV activity and the production and action of T cell-derived lymphokines. Production of interleukin 2 (IL-2) and gamma interferon by mitogen plus phorbol ester-stimulated mononuclear cells from human blood was found to be reduced in the presence of N-Ala-Pro-O-(nitrobenzoyl-)-hydroxylamine, epsilon-(4'-nitro) benzoxycarbonyl-Lys-Pro, and anti-(DP IV) immunoglobulin in a dose-dependent manner. Moreover, the proliferative response of mitogen-stimulated mononuclear cells to IL-2 is impaired in the presence of DP IV inhibitors. Therefore it is suggested that the membrane peptidase DP IV is involved in the induction and activation of cytokines controlling lymphocyte proliferation.

Immunenzymometric assay for the heart specific glycogen phosphorylase BB in human serum using monoclonal antibodies.

An enzyme-linked immunosorbent assay for the determination of the human glycogen phosphorylase isoenzyme BB (GP BB) using two murine monoclonal antibodies was developed. A series of hybridoma clones producing monoclonal antibodies to GP BB were obtained by the standard lymphocyte hybridoma technique. Two of the selected clones synthesizing monoclonal antibodies, which recognize different epitopes, were employed for the immunenzymometric assay. The first monoclonal antibody was immobilized on microtiter plates and the bound glycogen phosphorylase BB was detected with the second monoclonal antibody conjugated with horse radish peroxidase. This assay enables a specific and sensitive measurement of GP BB in the range of 0.5-150 ng/ml phosphate-buffered saline containing 0.5% bovine serum albumin in less than 3 hours. The lower limit in human serum amounts about 3 ng/ml. Preliminary data obtained with human sera from patients after aorto-coronary artery bypass surgery are demonstrated.

Production and characterization of four monoclonal antibodies specific for human interferon-alpha-1 and -alpha-2.

Four monoclonal antibodies against HuIFN-alpha-1 and -alpha-2 were produced by the lymphocyte hybridoma technique. The screening for monoclonal anti-IFN antibodies was performed by two different procedures permitting the detection of neutralizing and non-neutralizing antibodies. The first antibody screening in the supernatants of hybridoma clones was carried out by an immunodot ELISA using a novel chromogen, 2-bromo-1-naphthol, for the immunoperoxidase reaction. The second screening was performed with a solid-phase bioassay. The monoclonal antibodies IIG11 and IVH2 react specifically with HuIFN-alpha-1 independently of its origin, natural or recombinant. The mAb IIG11 recognizes also an epitope on a 27 kDa interferon, probably HuIFN-alpha-8. The mAb IIG7 binds to HuIFN-alpha-2 only, whereas the mAb IIB2 binds to HuIFN-alpha-2 and also to the 27 kDa interferon and to a 25 kDa interferon, not yet further characterized. These antibodies can be used successfully for immunopurification as well as for quantification of the corresponding interferons.

[Treatment of erectile dysfunction with a penile prosthesis].

From 1973 to February 1989 a total of 309 patients with erectile dysfunction were treated with hydraulic penile prostheses. In 169 patients a hydraulic prosthesis type AMSIPP700 was implanted; with this prosthesis the primary success rate was 91.1%. As a result of technical improvements to the device the revision rate, initially 52.3% has fallen to 3.3%. Patient acceptance is better with hydraulic prostheses, which provide optimal simulation of the physiological processes of erection and detumescence, than with any other type of prosthesis.

Intermittent catheterisation versus percutaneous suprapubic cystostomy in the early management of traumatic spinal cord lesions.

Spinal injury patients initially treated by intermittent catheterisation (IUC) and those who received a fine-bore suprapubic catheter (SPC) have been reviewed. The results show that fine-bore suprapubic catheterisation seems to be superior to intermittent catheterisation because the rate of urinary tract infections is significantly lower in the SPC-group (50%) than in the IUC-patients (71.9%), and the first infecting organisms in the SPC-group differ from those in the IUC-group and are much more easily treated by antibiotic therapy.

2-bromo-1-naphthol--a novel chromogen for the immunoperoxidase technique in immunohistochemistry.

2-bromo-1-naphthol is a new chromogen for the immunoperoxidase reaction in immunodot assays on nitrocellulose filters and on Western blots and could now also be employed in immunohistochemistry at the light microscopic level. It displays a blue-violet reaction product which is insoluble in aqueous solutions and almost insensitive to light. By means of this substrate for the peroxidase reaction in conjunction with 3,3'-diaminobenzidine it is also possible to perform double labellings.

Specific neutralizing antiserum against a polypeptide growth inhibitor for mammary cells purified from bovine mammary gland.

A recently published method for purification of a new inhibitor of growth of mammary cells in vitro from bovine mammary gland has been modified to yield several hundred micrograms of inhibitor per kg of glandular tissue. The inhibitory effect exerted by this preparation to Ehrlich ascites mammary carcinoma cells fulfilled all biological criteria of specificity established earlier for preparations obtained by other means, the most important being that the inhibitory effect is abolished by the epidermal growth factor and insulin. The preparation is shown to consist mainly of a protein of 13 kDa which appears to be not glycosylated. An antiserum raised in mice against the inhibitor is demonstrated to be specific for the 13 kDa bovine mammary gland protein. Neutralization of the inhibitory activity by the specific antiserum strongly supports the view that the 13 kDa protein is indeed the carrier of inhibitory activity. First data on tissue distribution obtained with an enzyme-linked immunosorbent assay revealed a high concentration of the anti-inhibitor-antiserum-reactive antigen in bovine lactating but not in non-lactating mammary gland tissue and in milk fat globule membranes. Some reactivity was also found in bovine lung. These data are interpreted with respect to a possible physiological significance of the growth inhibitor.

The sequential addition of ribosomal proteins during the formation of the small ribosomal subunit in Friend erythroleukemia cells.

Nucleolar '80-S' and '40-S' preribosomes (containing 45-S and 21-S pre-rRNA, respectively), as well as cytoplasmic ribosomes, were isolated from Friend erythroleukemia cells. The presence of structural ribosomal proteins in the isolated particles was studied by using antisera against individual rat liver small ribosomal subunit proteins. The analysis is based on the established crossreactivity between rat and mouse ribosomes [F. Noll and H. Bielka (1970) Mol. Gen. Genet. 106, 106-113]. The identification of the proteins was achieved by two independent immunological techniques: the passive haemagglutination test and the enzyme immunoassay of electrophoretically fractionated proteins, blotted on nitrocellulose. All 17 proteins tested are present in cytoplasmic ribosomes. A large number of proteins (S3a, S6, S7, S8, S11, S14, S18, S20, S23/24 and S25) are present in the '80-S' preribosome. Only two proteins (S3 and S21) are added during the formation of the '40-S' preribosome in the nucleolus. Four proteins (S2, S19, S26 and S29) are added at later, possibly extranucleolar, stages of ribosome formation. The results obtained provide evidence for the sequential addition of proteins during the formation of the small ribosomal subunit in Friend erythroleukemia cells.

Electron microscopic investigations on the location of rat liver ribosomal proteins S3a, S5, S6, S7 and S9 by means of antibody labeling.

The location of ribosomal proteins S3a, S5, S6, S7 and S9 on the surface of 40S subunits of rat liver ribosomes has been determined by means of antibody labeling and electron microscopy. Multiple antigenic determinants could be detected for proteins S3a, S6 and S7, whereas protein S9 shows only one and protein S5 two closely neighboured antigenic determinants. Antigenic determinants of proteins S3a, S6 and S7 have been mapped in the head and the body region of the 40S subunits. Protein S5 could be localized in the head and protein S9 in the body of the 40S subunits. These findings are discussed with regard to the location of functional sites on the 40S ribosomal subunit.
