Possible association of temporomandibular joint pain and dysfunction with a polymorphism in the serotonin transporter gene.

The purpose of this study was to evaluate the relationship between temporomandibular joint pain and dysfunction and serotonin transporter (5-HTT) gene polymorphism. Forty-eight patients with temporomandibular joint pain and 111 healthy control subjects were examined. The results for the patients and control subjects were not significantly different (P >.05). The analysis of genotype distribution (homozygous for STin 2.10 genotypes of the variable-number tandem-repeat polymorphism) showed significant differences between the patients and control subjects (P =.003). ST 2.10 allele was more frequent in the patients with temporomandibular joint pain and dysfunction. In the control group, however, STin 2.12/12 genotype was significantly higher (P =.017). In the patients who were homozygous or heterozygous for variable-number tandem-repeat variants of 5-HTT STin 2.12 copies, the average scores of somatization and anger were significantly higher than those who were homozygous for STin 2.10 variant (P <.05). The patients who were homozygous for STin 2.10 genotype were also homozygous for "L" genotype (P =.019). However, this was not the condition in the control subjects. This study does not provide evidence to support the involvement of 5-HTT gene-linked polymorphic region in temporomandibular joint pain and dysfunction. Our findings indicated that only the presence of the homozygous STin 2.10 genotype of variable-number tandem-repeat is likely to play a substantial role in the genetic predisposition to temporomandibular joint pain and dysfunction and that the STin 2.12/12 genotype may have a protective role against temporomandibular joint pain and dysfunction.

Impact of training on empathic communication skills and tendency of nurses.

The study was done as a quasi-experimental investigation with the purpose of measuring the effectiveness of training on teaching the nurses empathic tendency and skills for their interactions with patients. The study setting was that of 260 nurses who work at Hacettepe University Adult Hospital; but only the nurses (150) who work in the medical and surgical units were included in the sample. Nurses who work in the other intensive care units, outpatient clinics, operating rooms, obstetric unit, and psychiatric unit were excluded from the sample because they care for unconscious or permanently bedridden patients, so it is not possible to communicate with them as the study required. After the pretest questionnaire, nurses in the sample were distributed randomly to intervention and control groups. To lower interactions between nurses, clinics in which the nurses work were selected randomly, to keep the nurses far from each other physically. Data were collected through a questionnaire with three sections. The first section asked for information about the background characteristics of nurses. The second section consisted of sample cases very common in surgery and medical units and prepared to be appropriate to nurses, by using Dökmen's Scale of Empathic Skills. The third section of the questionnaire included the Empathic Tendency Scale. The validity and reliability studies of this scale were performed by Dökmen (Dökmen, 1989; Dökmen, 1990). The questionnaire was used for both groups, and then the intervention group was educated about empathic communication. After that, the same questionnaire was again used for the two groups. There were 43 nurses in the intervention group and 70 nurses in the control group. The data from this research were evaluated using percentage distributions and statistical tests such as Chi-square and paired T-test. According to the results of the research, empathic communication skills were developed in the intervention group as a result of empathy training. However, this training did not make any changes in empathic tendency levels.

Nurses' and physicians' views about euthanasia.

Nurses' and physicians' views about euthanasia were investigated using a questionnaire that was answered by 113 nurses and 83 physicians in the internal medicine and surgery inpatient services of Hacettepe University Adult Hospital, Ankara, Turkey. The data were evaluated with Chi-square statistical test and percentages. The study results showed that both nurses and physicians define euthanasia as "a person's right to die." Nurses responded that euthanasia may be appropriate with terminally ill elderly patients; however, physicians responded that it may be appropriate with patients at any age who are in the last stage of a terminal illness. Although both groups accepted euthanasia conditionally, they stressed that, in practice, the primary responsibility should be with patients and their relatives. When faced with a patient who wishes a painless death, nurses said that they feel helpless, anxious and guilty, and find themselves in a dilemma. However, physicians responded that they feel satisfied that they are doing the right thing in continuing the patient's medical therapy.

Tracheal reconstruction using alcohol-stored homologous cartilage and autologous cartilage in the rabbit model.

OBJECTIVE: Reconstruction of laryngotracheal stenosis continues to pose a significant challenge. Cartilage grafts have been in use for almost a century, but despite good clinical results, many questions concerning the survival and growth of implanted cartilage persist. To reduce donor site morbidity, the use of homologous cartilage has been investigated. This study compared alcohol-stored homologous auricular cartilage with autologous auricular cartilage for anterior graft laryngotracheal reconstruction in a rabbit model. METHODS: Autologous and alcohol preserved homologous auricular cartilage was transplanted to the resected anterior tracheal wall of the twenty New Zealand rabbits. Rabbits were sacrificed 6 weeks after surgery and histologic analysis was performed on the implanted cartilage grafts. RESULTS: The autografts were significantly more likely than the homografts to demonstrate viable cells (95% vs. 30%, P<0.05) and less likely to exhibit significant resorption, fibrosis or necrosis (P<0.05). Resorption and necrosis were most common in areas of trauma to the graft. Complete epithelialization occurred in all of the autografts but in only 65% of the homografts (P<0.05). New cartilage formation and integration of the implanted grafts was poor with both types of grafts. CONCLUSION: Autologous cartilage appears to have better survival than alcohol preserved homologous cartilage when used for anterior graft laryngotracheal reconstruction in a rabbit model.

Impact of initial surgical treatment on survival of patients with differentiated thyroid cancer: experience of an endocrine surgery center in an iodine-deficient region.

This retrospective clinical study was designed to analyze the impact of the initial surgical procedure on the survival of 1000 patients with differentiated thyroid cancer of follicular cell origin who had a thyroid operation and were followed for the 30 years between 1968 and 1998 (median 14 years) in an iodine-deficient region where goiter is endemic. There were 753 women and 247 men with a mean age of 42.8 +/- 6.7 years (range 17-86 years). Patients were divided into three groups. All patients had undergone thyroxine treatment and thyroid-stimulating hormone (TSH) suppression, and most had had iodine-131 treatment postoperatively. Group A consisted of 336 patients with differentiated thyroid cancer (DTC) who were treated with bilateral subtotal thyroidectomy in our institution or elsewhere. Group B consisted of 158 patients with DTC who were treated initially with unilateral total lobectomy and contralateral subtotal lobectomy in our institution or elsewhere and underwent reoperation in our department. Group C consisted of 506 patients with DTC who were treated initially with total or near-total thyroidectomy in our department. Kaplan-Meyer survival analysis was used. Recurrence was seen in 23% and death in 8% of the patients. The 20-year survival rates were 76%, 85%, and 92% for groups A, B, and C, respectively. The survival difference among the patients of group A and groups B and C was found to be statistically different (p < 0.001). Long-term survival of patients with differentiated thyroid cancer living in endemic areas for goiter can be influenced by the initial surgical treatment. Patients treated initially with total or near-total thyroidectomy appear to have a better prognosis.

Ectopic thyroid in the abdomen: report of a case.

Ectopic thyroid tissue can be seen anywhere along the path of the descending glands, but it is rarely seen in the abdominal cavity. An ectopic thyroid was encountered incidentally in the pancreas of a 50-year-old woman who underwent a bilateral truncal vagotomy and pyloroplasty for a duodenal ulcer. There were no signs or symptoms of a thyroid tumor.

Thymosin alpha 1 protects liver and aorta from oxidative damage in atherosclerotic rabbits.

The thymus hormones were reported to be effective on lipid peroxidation and the antioxidant system. Thymus plays a broader role than just regulating the immune system. Thymosin alpha 1 is the first subgroup extracted from thymosin F5 and has higher biological activity than thymosin F5. In the present study, we have examined the effects of thymosin alpha 1 on lipid levels and lipid peroxidation and glutathione (GSH) content in the plasma, liver and aorta tissues of atherosclerotic rabbits. At the end of thymosin alpha 1 treatment, we determined the lipid levels and lipid peroxidation of the plasma, liver and aorta tissues and hepatic subcellular fractions in these rabbits. Our results demonstrated that thymosin alpha 1 might normalize changed lipid levels and increased lipid peroxides and also elevate decreased GSH in the plasma, liver and aorta tissues of atherosclerotic rabbits. Results of this study suggest that thymosin alpha 1 may be beneficial to prevent and/or to treat atherosclerosis.

Persistent vacuoles in leukocytes: familial Jordans anomaly.

Multiple persistent vacuoles were seen in the neutrophils, monocytes and eosinophils of a 9 year old boy and his 10 year old sister. The siblings were both asymptomatic. In the bone marrow, the cytoplasmic vacuoles were also present in the promyelocytes, myelocytes and metamyelocytes, but not in the myeloblasts and they tended to be single and large in immature cells. The cytoplasmic vacuoles did not stain with PAS, Sudan Black or Oil Red O; Sudan III positivity of the vacuoles was found only in a very small number of granulocytes. The vacuoles appeared as round and bright bodies with phase contrast microscopy. By electron microscopy, the vacuoles contained material of low electron density and had no surrounding membrane. Granulocyte functions were unimpaired. Muscle biopsy showed normal morphology. This anomalous vacuolization of the leukocytes is consistent with familial Jordans anomaly.

Cytologic changes induced by 131I in the thyroid glands of patients with hyperthyroidism; results of fine needle aspiration cytology.

Cytological changes in the thyroid glands of 120 patients who had received 131I treatment for hyperthyroidism were examined in detail. The cytological material was obtained by fine needle aspiration cytology. The patients were divided into four groups according to the amount of time elapsed after the 131I treatment. The control groups were selected from euthyroid patients with hypoactive or non-functional solitary nodules and patients with Graves' disease who had not received 131I therapy. The thyrocytes revealed degenerative changes in the immediate period after cessation of treatment. One year after the treatment some non-specific changes, such as oxyphilic metaplasia and regeneration, were seen. No malignant change has been observed clinically or cytologically during up to 20 years of follow up since receiving 131I.

Fetus in fetu or giant epignathus protruding from the mouth.

A huge and extremely organoid mass, protruding from the mouth of a newborn and causing respiratory embarrassment, was resected. It was difficult to make a clear distinction whether this mass was fetus in fetu or a highly organoid epignathus dealing with the findings obtained from its gross and histological examination of the mass.

Wilms' tumor: prognostic factors and survival.

A statistical analysis of 38 cases of Wilms' tumor treated and followed by the Pediatric Oncology Group of Cerrahpaşa Medical Faculty, from 1977 to 1984, is presented. The two-year survival rate was 67% and the five-year survival rate was found to be 63%. There was a strong correlation between the stage and age of the patients and prognosis. The median age was 16 months in stages I and II of the disease and the two-year survival rate was 87%. The median age was found to be 5 years in advanced stages and the survival rate at 2 years was 43%. No correlation was found between survival and initial symptoms of the tumor or its localization. The effects of histological grade on prognosis of disease were also investigated.

Idiopathic ulcerative colitis in Istanbul: clinical review of 204 cases.

A clinical review of 204 cases of idiopathic ulcerative colitis (IUC) seen in an insurance hospital in Istanbul during a period of nine years is presented. The criteria of diagnosis were based on history and results of rectosigmoidoscopic, biopsy, and barium-enema examinations. In the majority of cases (74 per cent), the disease manifested in the mild form, despite a history of one to five years' duration at the time of diagnosis. These patients usually had single attacks, with recovery in reponse to treatment. The mortality rate for the whole series was 3.9 per cent. Death occurred in eight cases of patients who had the severe form of the disease involving the entire colon, with fulminating courses. One additional death was due to an accident. Treatment consisted of a high-protein, low residue diet, vitamins, rest, sulfa drugs and steroids. Azathioprine, in addition, was used in fulminating cases. Surgery supplemented medical therapy in 19 cases. Indications for surgical treatment were resistance to medical management, cancer, and polyposis. Only two patients (1.0 per cent) were found to have cancer during the follow-up period. These two were operated upon. Idiopathic ulcerative colitis is not a rare entity; with awareness and use of appropriate diagnosis facilities, more cases are being discovered.
