Isoelectric focusing of serum alkaline phosphatase isoenzymes.

We present a new method for the separation of alkaline phosphatase isoenzymes based on isoelectric focusing of serum proteins. Sera of healthy subjects studied by this method present seven groups of bands within a pH range of 3-9. To date, the organ source of six out of seven groups has been identified directly or indirectly. Serum of patients with histologically proven malignant neoplasms showed an additional group in the pH range 3.6-4.2. This fraction seems to be heterogeneous with respect to the pattern and stability to heat, and to L-phenylalanine and L-homoarginine inhibition. It was present in all but two of the proven malignant tumour patients but also in 26 out of 70 with benign disorders.

[Gastro-oesophageal reflux--what if physiological? (author's transl)].

Gastro-oesophageal reflux was examined by taped long-term pH monitoring in 31 healthy subjects (16 men, 15 women; average age 28 years). The following was found: (1) reflux during the day is normal in health subjects, occurring predominantly after meals; (2) healthy persons have practically no reflux at night, after reaching the phase of deep sleep, the decisive difference between the healthy subject and those with reflux concerning the sleep phase, during which the patient with reflux has significantly increased reflux; (3) gastro-oesophageal reflux during the night is dependent upon depth of sleep, while position-dependent reflux was not observed.

[Analysis of the Luciani-Wenckebach phenomenon in the A-V node. II. Relationship between conduction and recovery times].

We have studied 20 cases Wenckebach A-V block by atrial pacing. The relationships between conduction time and recovery time of N. AV Wenckebach point, basal A-H time and A-H time of first cycle beat were analysed. No correlation was found between W point, basal A-H and A-H of the first beat by analysing the A-H = f (H-A) we found different curves which occur when the first A-H is longer or shorter than 110. These data were discussed on the basis of modern hypotheses of electrophysiological mechanism of Wenckebach periodism.

[Systolic time intervals in obstructive hypertrophic cardiomyopathy. The effects of adrenergic beta-stimulation and beta-blockade in 38 cases (author's transl)].

The authors report on the behaviour of systolic time intervals, studied with a noninvasive poligraphic technique, under normal conditions, after adrenergic beta stimuls (orciprenaline) and successive beta blocker (pindolole) in 38 patients with obstructive cardiomyopathy. Under normal conditions, the authors observed a marked dispersion of PEP and TEVS data, which have, however, average normal values. The IPEP was reduced in 29% of cases, was normal in 47.3% and raised in 23.7%; the ITEVS was reduced in 55.2% of cases, was normal in 21.1%, raised in 23.7%; the PEP/TEVS was reduced in 23.7%, normal in 31.6% and raised in 44.7%. The beta stimulation demonstrated a number of behaviour patterns: the most frequent cause was a reduction of PEP in the cases where it had been raised or where it had remained within normal limits, an increase of TEVS in the cases where it had been diminished or normal, a reduction of the PEP/TEVS correlation where these values were increased or normal. The beta blocker, followed by beta stimulus, brought on variations opposite from those of the beta stimulation. Thus, one can consider the hypothesis that the possible behaviour patterns and combination of PEP and TEVS result from different anatomical and functional expressions that can become obstructive cardiomyopathy, according to the seriousness of the obstruction, the ventricular compliance and the contractility.

[Gastroesophageal reflux and peptic esophagitis].

This review focuses on the advances made in the pathophysiology, diagnosis and treatment of gastroesophageal reflux disease. During the last five years, the factors responsible for the disease have been extensively investigated and are now relatively well understood. Little progress, however, has been made with regard to diagnostic techniques, but the roles of endoluminal pH-monitoring and esophageal manometry have greatly increased. Finally, the therapy of the disease has substantially been improved, particularly by the availability of new antisecretory and prokinetic compounds such as omeprazole and cisapride.

[Agricultural antiparasitics evaluated for their potential cancerogenic, mutagenic and toxic reproductive effects].

This paper reviews the evaluations of the potential carcinogenic, mutagenic and toxic-reproductive effects of the active ingredients and solvents contained in the pesticide products used in Italy. The information on the products (name and CAS number of the active ingredients and of the solvents, number of the products in which they are present, and the range of concentration percentages of a.i.) were obtained from the Pesticide Registry, a database operating at the Italian National Institute of Health since 1985. The evaluations of the toxic effects of these products are those which have been formulated by the Italian National Advisory Toxicological Committee, the European Union, the International Agency for Research on Cancer, and the Environmental Protection Agency of the United States. The aim of this study was to present an ample organic review of the main information issued by national and international centers on pesticides, chemicals of wide occupational use and diffusion in the environment. The presence is highlighted of some substances with carcinogenic potential (for example, 1,3-dichloropropene, alachlor, formaldehyde) for which it is therefore, necessary to carry out a careful risk assessment regarding exposure to many substances for which further study is indicated, and for others which have not, so far, produced toxic effects (bentazon, cypermetrin). Benzene "a proven human carcinogen" was present in three products. In addition, the presence was observed out of mixtures of active ingredients and solvents for which clear evidence of carcinogenic and/or reproductive toxicity exists. This information can be useful to assess the carcinogenic risk associated to pesticides in the occupationally exposed population, as also established in official regulations.
