Risk factors for pregnancy-induced hypertension in women at high risk for the condition. Italian Study of Aspirin in Pregnancy Group.

To analyze determinants of the risk of pregnancy-induced hypertension (PIH) with or without proteinuria, we compared characteristics of women enrolled in the Italian Study of Aspirin in Pregnancy who developed PIH and those who did not. A total of 756 women were included in the present analysis; of these, 132 women (17%) developed PIH during the trial. The risk of developing PIH tended to increase with maternal age: in comparison with women age 20-25 years, the odds ratio (OR) estimates of risk ratio were 3.5 [95% confidence interval (CI) = 1.6-7.1] in women age 26-30 years and 4.2 (95% CI = 1.9-8.8) in those age > 30 years. There was little relation between development of PIH and education. PIH risk increased according to nonpregnant body mass index; in comparison with women with Quetelet's index (kg per m2) < 25, the OR estimates were 1.7 (95% CI = 1.1-2.7) and 2.1 (95% CI = 1.3-3.6), respectively, for women with a value for Quetelet's index of > 25-30 and > 30. Parous women were at decreased risk of PIH: in comparison with nulliparas, the ORs were 0.7 (95% CI = 0.4-1.0) and 0.5 (95% CI = 0.3-0.9), respectively, in women reporting 1 or > or = 2 births. There was no important relation between previous spontaneous or induced abortion and PIH risk.

The influence of body size, smoking, and diet on bone density in pre- and postmenopausal women.

We studied the determinants of low bone mineral density, using data from a population-based screening program of osteoporosis carried out among 1,373 women (age 40-64 years) in the province of Pordenone, Italy, by means of dual photon absorptiometry of the lumbar spine. Menopause had a major effect on bone mineral density. Age had little influence before menopause. In multivariate linear regression analyses, weight was the strongest predictor of bone mineral density in pre- as well as postmenopausal women. After the inclusion in a single model of a term for current weight, weight at ages 12 and 30 years explained some additional variance, whereas high waist-to-hip ratio (an indicator of central adiposity) had no influence. Smoking 15 or more cigarettes per day entailed a small increased risk of osteoporosis, but this effect, independent of weight, appeared to be restricted to premenopausal women. No food or micronutrient that we examined was predictive of bone mineral density, nor was coffee or alcoholic beverage intake.

Reproductive factors and risk of uterine fibroids.

We analyzed the relation between reproductive history and risk of uterine fibroids using data from a case-control study. Cases were 621 women with histologically confirmed diagnosis of uterine fibroids. Controls were 1,051 non-hysterectomized patients. Compared with nulliparae, parous women had a relative risk (RR) of fibroids of 0.5 [95% confidence interval (CI) = 0.4-0.6], and the risk declined with number of births. The risk of fibroids also decreased with number of induced abortions (RR = 0.8 and 0.6 for women reporting one or two or more abortions, respectively). A total of 24 cases (3.9%) and 19 controls (1.8%) reported a history of infertility (RR = 2.0; 95% CI = 1.1-3.7).

Treatment with a gonadotrophin releasing hormone agonist before endometrial resection: a multicentre, randomised controlled trial.

OBJECTIVES: To ascertain whether treatment with a gonadotrophin releasing hormone agonist before endometrial resection reduces absorption of distension fluid and operating time and facilitates the procedure. DESIGN: A multicentre, prospective, randomised controlled study. PARTICIPANTS: Seventy-one premenopausal women with established menorrhagia. INTERVENTIONS: Eight weeks of goserelin depot treatment before endometrial resection of immediate surgery in the early proliferative phase of the cycle. MAIN OUTCOME MEASURES: Irrigation fluid deficit, operating time and degree or difficulty of the procedure. RESULTS: After randomisation eight women withdrew from the study, leaving 33 women in the goserelin arm and 30 in the immediate surgery arm. Mean (SD) operating time was 15.1 (9.0) min in the goserelin group versus 16.9 (9.5) min in the controls; mean difference + 1.8 min, 95% CI, -2.9 to + 6.4. Mean (SD) distension medium deficit was, respectively, 422 (287) ml versus 564 (291 ml); mean difference + 142 ml, 95% CI -4 to + 288. The goserelin effect was restricted to the 29 women with adenomyosis as the mean (SD) fluid deficit was considerably less in the 19 treated women than in the 10 controls (299 (206) ml versus 597 (135) ml; mean difference + 298 ml, 95% CI + 149 to + 447). The surgeons classified the intraoperative difficulties as none in 6, minimal in 20, moderate in 7, and severe in no cases in the goserelin group; corresponding figures in the group without pretreatment were 2, 14, 13, and 1. CONCLUSIONS: Goserelin administration before endometrial resection may reduce absorption of fluid at surgery in women with adenomyosis and may facilitate intrauterine operating conditions.

Menopause, menstrual and reproductive history, and bone density in northern Italy.

STUDY OBJECTIVE: To analyse the relationship between menstrual and reproductive factors and the risk of low bone mineral density (BMD). DESIGN: This was a population based screening programme carried out between 1991 and 1993 among 1373 perimenopausal women in northern Italy by means of dual photon absorptiometry at the lumbar spine. MAIN RESULTS: BMD was strongly related to the age at menopause. In comparison with women reporting menopause below 45 years of age, the odds ratios (OR) of being in the lowest compared with the highest BMD tertile were 0.7 (95% confidence interval (CI) 0.3,1.5) and 0.3 (95% CI 0.1,0.8), respectively, in those with menopause at age 45-49 and above 50 years: the trend in risk was significant. Likewise, the risk of being in the lowest tertile increased with years since the menopause. Compared with women who reported they had undergone the menopause less than two years before interview, the OR of being in the lowest BMD tertile were 2.1 (95% CI 1.1,4.3), 2.3 (95% CI 1.1, 5.0), and 5.7 (95% CI 2.5,12.9) respectively in women who reported menopause 2-5, 6-9, and > or = 10 years earlier. The protective effect on bone density of late age at menopause was observed in different strata of years since menopause. Likewise, the increasing risk of a low BMD with increasing years since the menopause was evident in strata of different age at menopause. No relationships were observed between BMD and the age at menarche, characteristics of menstrual cycles, and the duration of menses. Likewise, no association emerged between reproductive history, including parity and age at first pregnancy, and BMD. CONCLUSIONS: In this Italian population the risk of being in the lowest BMD tertile decreased with increasing age at menopause and increased with years since menopause. No relationships emerged between BMD and other menstrual characteristics or reproductive factors.

Oral contraceptives and cancer. A review of the evidence.

The following review considers epidemiological data published from 1990 onwards on oral contraceptives (OCs) and the risk of cancers of the breast, cervix uteri, endometrium, ovary, liver and skin. In several studies, breast cancer risk was seen to be elevated among women who were current users of an OC, or had recently stopped using an OC, whereas there was no residual risk 5 or more years after stopping OC use. No interaction was observed between type of OC, or with any recognised risk factor for breast cancer, or time-factor, except for some potential excess risk for women who started OC use at a young age. Most studies have confirmed that OCs moderately increase the risk of cervical cancer, particularly in human papilloma virus (HPV)-positive women, thus suggesting that OCs may act as a promoter for HPV-induced carcinogenesis. Recent epidemiological studies have confirmed that combined OCs provide substantial protection against endometrial and ovarian cancers, and results suggest that such protection is long-lasting, and may persist for 15 years or more after stopping OC use. Most case-control studies have shown a relationship between OC use and hepatocellular carcinoma. However, data from cohort studies or analysis of vital statistics indicate that the public health impact of such an association is modest, if not negligible. No association was observed between combined OC use and the incidence of skin melanoma, or any other common skin neoplasm. In terms of clinical and public health implications, the most relevant points regarding OC use are: (i) recent data confirm that OCs confer presistent protection against ovarian cancer; and (ii) any increased risk of breast cancer in OC users is moderate and is restricted to current/recent users. This is reassuring for younger women, whose baseline risk of this disease is extremely low.

Sexually transmitted diseases and risk of HIV infection.

We have analyzed the association between sexually transmitted diseases (STD) and HIV infection, using data from a cross-sectional survey of subjects attending STD clinics in Northern Italy conducted since 1988. A total of 1,711 subjects (1,259 males, 452 females), who had referred themselves to three STD clinics in Northern Italy for suspected STD or STD treatment, were included for the study. Out of these, 145 subjects (113 males and 32 females) were HIV-positive. A total of 58 HIV-positive and 368 HIV-negative subjects reported a history of STD; the corresponding odds ratio (OR) was 2.3 (95% confidence interval (CI) 1.5-3.6) for subjects reporting a history of STD. Considering various STD in details, the estimated OR was 1.8 (95% CI 0.8-3.8) for a history of gonorrhoea and 1.5 (95% CI 0.8-2.7) of syphilis, and the OR was 1.8 (95% CI 1.0-3.2) and 2.2 (95% CI 1.3-3.8), respectively, for a positive TPHA and VDRL test. The results of the test for HbsAg were available in 50 HIV-positive and 1,028 HIV-negative subjects; the OR of HIV infection in subjects with HbSAg was 3.9 (95% CI 1.7-9.0). Presence of genital ulcers at clinical examination was not significantly associated with the risk of HIV infection (OR yes vs no genital ulcers 1.5, 95% CI 0.6-2.8).

Validity of self-reported smoking habits in pregnancy: a saliva cotinine analysis.

BACKGROUND: We examined the validity of self-reported cigarette smoking during the third trimester of pregnancy using saliva cotinine as a marker. METHODS: Eligible for the study were 109 pregnant women attending the outpatient Prenatal Service of the Luigi Mangiagalli Clinic (the largest maternity clinic in Milan) for routine prenatal visits during the third trimester of pregnancy on twenty days in 1994. Women self-reporting current smoking or quitting smoking in pregnancy were asked to provide a saliva sample. Cotinine concentration was analyzed and classified as follows: cotinine not detectable, not probable nicotine use or passive exposure; cotinine <10 ng/ml, not probable nicotine use/probable passive exposure; cotinine, > or = 10 ng/m, probable occasional or regular nicotine use. RESULTS: A total of 57 (52.3%) women were non-smokers at conception and were excluded from any subsequent analysis. Of the remaining 52 women, 25 self-reported quitting smoking in pregnancy and 27 were current smokers. Saliva cotinine levels were below 10 in 20 of the 25 subjects reporting quitting smoking in pregnancy. The five cases with cotinine > or = 10 reported a husband smoking more than 10 cigarettes per day. Among the 26 current smokers, seven had a cotinine level <10 ng/ml (four reported smoking fewer then five cigarettes per day and two reported smoking five or more per day); in 20 cases the cotinine value was > or = 10 ng/ml. CONCLUSIONS: These findings provide evidence of a satisfactory validity of self-reported smoking habits in pregnancy.

Endometriosis and pelvic pain: relation to disease stage and localization.

OBJECTIVES: To determine whether prevalence and severity of pain symptoms are related to endometriosis stage and site, with particular reference to deep infiltrating vaginal lesions. DESIGN: Systematic assessment of chronic pelvic pain symptoms. SETTING: University hospital endometriosis center. PATIENTS: A total of 244 consecutive symptomatic patients with endometriosis diagnosed at laparoscopy or laparotomy. INTERVENTIONS: Assessment of dysmenorrhea and nonmenstrual pain by means of a 10-point linear analog scale, a 7-point multidimensional rating scale, and a 3-point verbal scale; evaluation of deep dyspareunia with the first and third systems only. MAIN OUTCOME MEASURES: Prevalence and severity of pain symptoms in relation to endometriosis stage and site of lesions. Correlation between revised American Fertility Society score and symptoms severity, as well as between two pain scales to assess dysmenorrhea and nonmenstrual pain. RESULTS: Eighty-eight women had stage I and II disease and 156 had stage III and IV disease. Only ovarian endometriosis was present in 108 patients, only peritoneal implants were present in 37, combined ovarian and peritoneal lesions were present in 57, and histologically confirmed vaginal endometriosis was present in 42. The frequency and severity of deep dyspareunia and the frequency of dysmenorrhea were less in patients with only ovarian endometriosis than in those with lesions at other sites. Patients with vaginal endometriosis had a significantly increased risk of deep dyspareunia compared with those whose lesions were at other sites (odds ratio, 2.55; 95% confidence interval, 1.21 to 5.39). Stage per se, independent of lesion site, was not correlated with frequency and severity of dysmenorrhea and nonmenstrual pain. The severity of deep dyspareunia was related inversely to the endometriosis score (Spearman correlation coefficients for linear analog and verbal rating scales, respectively, -0.22 and -0.20). Kendall test by ranks revealed a correlation between linear analog and multidimensional pain scales in the rating of both dysmenorrhea and nonmenstrual pain (respectively, tau-b, 0.59 and tau-b, 0.68). CONCLUSIONS: Endometriosis stage in the current classification was not related consistently to pain symptoms. The presence of vaginal lesions was associated frequently with severe deep dyspareunia. Dysmenorrhea and nonmenstrual pelvic pain were assessed with equal accuracy by a linear analog and a multidimensional scale.

Uterine myomas and smoking. Results from an Italian study.

OBJECTIVE: To evaluate the relationship between smoking and uterine myomas requiring surgery. STUDY DESIGN: We conducted a case-control study in Milan between 1986 and 1992. Cases were 476 patients under 55 years of age with histologically confirmed myomas. Controls were 1,283 women admitted to the hospital for a spectrum of acute, other-than-gynecologic, hormonal or neoplastic conditions (30% trauma, 25% nontraumatic orthopedic conditions, 25% surgical, 20% other miscellaneous). RESULTS: Cases were less frequently current smokers (22%) than controls (32%). In comparison with never smokers, the multivariate relative risk (RR) for myomas was 0.5 (95% confidence interval [CI], 0.4-0.7) in smokers. Ex-smokers were 10% of cases versus 7% of controls (RR 1.2, 95% CI 0.9-1.8). No clear trend in risk was observed with the number of cigarettes smoked per day or duration of smoking and risk of fibroids. The estimated RRs were largely consistent when separate analyses were performed in strata of age and other selected covariates, including indices of body weight. CONCLUSION: Current smoking seems to reduce the risk of myomas.

Dietary factors and the risk of psoriasis. Results of an Italian case-control study.

We have conducted a case-control study to explore the relation between the consumption of selected foods and psoriasis. The out-patient services of nine teaching hospitals and five general hospitals in northern and southern Italy took part. Cases were newly diagnosed psoriatics with a history of skin problems of no more than 2 years. As controls, we selected subjects with newly diagnosed skin conditions, other than psoriasis, who were seen in the same out-patient clinics. Inclusion of cases and controls was limited to the age group 16-65 years. A total of 316 psoriatics and 366 controls were recruited. Anthropometric measures, including height and weight, were obtained. Diet was assessed by a semiquantitative food frequency questionnaire. Psoriasis appeared to be positively associated with body mass index (chi 2(1) trend 4.3, P < 0.05). Significant inverse relations with psoriasis were also observed for the intake of carrots (chi 2(1) trend 4.8, P < 0.05), tomatoes (chi 2(1) trend 26.3, P < 0.01), fresh fruit (chi 2(1) trend 11.7, P < 0.01) and index of beta-carotene intake (chi 2(1) trend 9.1, P < 0.01). Although largely explorative, these results provide some evidence for a potential role of diet in psoriasis.

Pelvic inflammatory disease and risk of ovarian cancer.

We analyzed the association between history of pelvic inflammatory disease (PID) and the risk of subsequent epithelial ovarian cancer, using data from a large case-control study conducted between 1983-1991 in Italy. Data were collected from a network of hospitals, including the main teaching and general hospitals in the greater Milan area, Northern Italy. The cases studied were 971 women below the age of 75 years (median age, 54 years) with histologically confirmed epithelial ovarian cancer, diagnosed within 1 year before the interview. Control subjects were 2758 women admitted to the same hospitals where cases were identified for acute, nonmalignant, nonhormone-related conditions, who had not undergone bilateral oophorectomy. The median age of the control group was 52 years (range, 23-74). A total of 14 (1.4%) cases and 72 (2.6%) controls reported a history of PID/ salpingitis, the corresponding multivariate relative risk being 0.7 (95% confidence interval, 0.4-1.3). A separate analysis of the association between history of PID/salpingitis and risk of ovarian cancer in strata of parity and education confirmed the results based on the whole series. In conclusion, although based on limited numbers of cases and controls with PID, this studies was able to exclude, at the conventional 95% confidence limit, an increased risk of ovarian cancer of over 30% in women with previous PID in this population.

Risk factors for functional ovarian cysts.

We evaluated risk factors for functional ovarian cysts using data from a case-control study conducted in Milan. Cases were 68 women with histologically confirmed functional cysts. Controls were 272 women admitted to hospital for acute conditions. Compared with women who were 11 years of age or younger at menarche, women who experienced menarche at 12-13 years or at 14 years or later had odds ratios (ORs) of 0.9 [95% confidence interval (CI) = 0.5-1.6] and 0.4 (95% CI = 0.2-0.9). In comparison with women reporting menstrual cycles lasting less than 26 days, the OR was 1.8 (95% CI = 0.7-5.0) in women reporting cycles lasting 26-30 days, 1.8 (95% CI = 0.4-7.4) in those reporting cycles greater than 30 days, and 1.9 (95% CI = 0.4-7.7) in those with totally irregular cycles. Body mass was inversely related to the risk of functional ovarian cysts: compared with women with a Quetelet index below 20, the OR was 0.9 (95% CI = 0.5-1.7) in those with an index in the range from 20-24 and 0.5 (95% CI = 0.2-1.2) in those with an index of 25 or more. We found little relation between the risk of functional ovarian cysts and education, smoking, marital status, or age at first marriage. For oral contraceptive use, we found an OR of 1.3 (95% CI = 0.7-2.6).

Selected food intake and risk of vulvar cancer.

BACKGROUND: A case-control study was conducted to analyze the association between body mass, selected indicator food intake, and vulvar cancer risk. METHODS: The patients included in this report were 125 women aged 80 years or younger with histologically confirmed diagnosis of invasive vulvar cancer who were admitted to a network of general and teaching hospitals in the greater Milan area. Control subjects were 541 patients admitted to teaching and general hospitals in Milan for acute conditions. RESULTS: The risk of vulvar cancer was inversely related to green vegetable and carrot consumption, the corresponding multivariate relative risks for lowest versus highest levels of intake being 2.0 (95% confidence interval [CI], 1.2-3.4) and 1.4 (95% CI, 0.9-2.2). The trend in risk was significant for green vegetables. No consistent association emerged between milk, meat, liver, alcohol and coffee consumption and risk of vulvar cancer. In comparison with leaner women, the relative risks of vulvar cancer were 1.8, 1.9, 2.8, and 2.9 in progressively higher quintiles of the body mass index, and the trend in risk was significant. CONCLUSIONS: These data indicate that the risk of vulvar cancer is related to a number of nutritional and dietary factors. This is of particular interest, because vulvar cancer is a relatively rare neoplasm, whose etiology is still poorly understood, and on which only a few epidemiologic studies have been conducted.
