[Behavior of alkaline phosphatase isoenzymes in liver diseases].

Levels of alkaline phosphatase isoenzymes in patients with hepatic diseases, liver tumours and in normal control groups were analysed. Attention was focused on the ALP 1 isoenzyme and its validity in the diagnosis of hepatic metastases was confirmed.

Alpha-fetoprotein and mediastinal germ cell tumors.

The Authors discuss the utility of alpha-fetoprotein (AFP) determination in mediastinal dysembryomas. AFP should be determined in all cases of mediastinal lesion. High levels of up to 500 ng/ml are a sure sign of germ cell tumors. An extremely high level (greater than 1,000 ng/ml) is an indication that the lesion is a mesoblastoma. An increase in the AFP level in the follow-up of patients affected by germ cell tumors is a sure sign of the recurrence of the disease.

Alpha-fetoprotein and tissue polypeptide antigen in non neoplastic hepatic disorders.

In order to evaluate the behaviour of alpha-fetoprotein (AFP) and Tissue Polypeptide Antigen (TPA) in non neoplastic chronic hepatic diseases 60 patients suffering from chronic hepatitis have been studied, 28 of them with different ethiology cirrhosis, 4 with primary biliary cirrhosis (CBP), 18 with chronic active hepatitis (ECA), 5 with chronic persistent hepatitis (ECP), 3 suffering from alcoholic and 2 drug-induced hepatitis. In each case the diagnosis was biopsy-proved. We have found that TPA clearly shows an increase in about 90% of cirrhosis and in about 50% of ECA. Moreover, the group with non-A, non-B (NANB) cirrhosis and chronic hepatitis has shown a statistically significant correlation between TPA and alanine aminotransferase (ALT). On the other hand, AFP hants' shown statistically significant variations. The reasons of the TPA increase must probably be looked for in the marked sensitivity of this protein to non neoplastic tissues in rapid regeneration, in addition to the sensitivity to neoplastic tissues. Further studies will be carried out to evaluate the usefulness of TPA to tracing possible cytolitic relapses or any resumption of activity in hepatic cirrhosis.

Alpha-fetoprotein as tumor marker in lung cancer diagnosis.

Authors have studied AFP serum levels in 289 lung cancer. Only 2 cases (0.7%) showed little increase of this glycoprotein and liver pathologies were present in both patients, authors believe immunohistochemical study is necessary in lung cancers that show AFP increase.

Effect of lung resection on pulmonary function. A comparative study in different surgical procedures.

The effect of lung surgery on respiratory function has been investigated in 40 patients considering separately the three main procedures (pneumonectomy, lobectomy and thoracotomy alone) to ascertain if the resected lung and the volume removal can influence this function in the immediate postoperative period. The patients were submitted to spirometry and arterial gas analysis preoperatively and during 9 days postoperation; the tests controlled were VC, FEV1, TV and RR. No significant difference has been noticed among various surgical procedures.

Clinical utility of tissue polypeptide antigen determination in lung cancer management.

In view of the fact that pulmonary malignancies still represent an important cause of tumor death and that the high rate of unsuccessful treatment may be partly due to the late clinical presentation, efforts should be spent not only to develop new and effective treatments but also to improve early diagnosis and to identify prognostic factors and parameters useful for the monitoring of the treatment. Tumor markers, if used properly, can provide a useful support for the management of patients suffering from various malignancies, including lung cancer patients. The clinical significance of one of the most widely used tumor markers, Tissue Polypeptide Antigen (TPA), has been reviewed and showed this marker to be useful to the clinician for the management of patients with pulmonary malignancy, as a complementary tool for diagnosing and staging the tumor as well as for monitoring treatment response or relapse occurrence.

[A cancer marker study in patients with neoplastic pathology of the neck and head area].

Specific markers able to detect either the presence of tumours or to identify the histological type of tumour are not currently available. Previous work yields an inconsistent picture of actual effectiveness of oncological markers, we therefore carried out a study on the cancer patients in our clinic. The present paper considered five markers (CEA, SCC, ca 19-9, TPA and Ferritin) and tested their effectiveness in diagnosing and following patients with head and neck neoplasias. The study was carried out on 102 patients, divided into three groups: 59 had been histologically diagnosed cancer but had not received any treatment (first group), the remaining 43 who had been treated (second group). We have also monitored the first group of no treated patients six months after therapy (third group). Our results suggest that while no significant conclusions may be drawn from SCC and CA 19-9, we found interesting results concerning Ferritin. Indeed Ferritin values increased by 22% in the first group, by 26% in the second one and by 19% in the third one. CEA and TPA values increased in non treated patients only by 22% and 12% respectively while they did not alter in the treated ones and they altered by 10.5% and by 10% respectively six months after therapy. Although our data are still preliminary and speculative and further evidence is needed, we can infer that the stage of the cancer and the positive markers appear to be concluded. The correlation is even stronger when all the cases are considered, without introducing the statistical and epidemiological variables.
