[Long-term sedation with propofol in ICU: hemocoagulation problems].

The Authors have studied the effects of propofol on coagulation in 15 patients admitted to ICU. Propofol was used for long-term sedation (therapeutic range 3 mg/kg/h). Variables monitored included: platelets, PTT, PT, Fibrinogen, FDP, AT III. The effects on coagulation has been investigated in three groups of patients: group I) 5 patients that received propofol for 1-3 days; group II) 5 patients that received propofol for 4-10 days; group III) 5 patients that received propofol for 11-35 days. No difference were found about blood coagulation in this groups of patients before and after administration of propofol.

[Multiorgan damage in exertion heatstroke].

The paper reports a fatal case of exertion heat stroke. The etiopathogenetic, clinical and therapeutic aspects of the case are described; although rarely observed, this pathology must be taken into account by the intensive care unit in order to ensure rapid aid and treatment in an attempt to modify the severe prognosis due to multiorgan damage which increases with the intensity and duration of hyperthermia.

[Colpocystourethrorectography and echography. Comparison of 2 technics in the study of female stress urinary incontinence].

Urinary stress incontinence has been examined in 42 women with a standard radiographic technique, such as CCURG (colpocystourethrorectography) and echotomography. With these techniques 6 parameters have been established and measured both at rest and under Valsalva's maneuver. Stress incontinence has been diagnosed through variation of the normal values above physiological limits during the passage from rest to stress. In particular, reliability of echotomography in measuring the six parameters has been established and advantages and disadvantages have been evaluated in comparison with CCURG.

Vitamin D concentrations in women of postmenopausal age with fractures of the femoral neck.

Thirty-eight women of postmenopausal age, suffering from fractures of the femoral neck or vertebral bodies were studied in relation to differences in bone metabolism. The blood and urinary changes concerned in mineral and bone metabolism were recorded within 10 days of trauma, and in some femoral neck fractures, a histological study of the femoral heads removed in the course of prosthetic substitution was carried out. The patients with femoral neck fractures were older than those with vertebral fractures and had metabolic and histological findings suggestive of osteomalacia. A particularly significant difference between the two groups was the plasma level of 25 hydroxycholecalciferol, which was lower in femoral neck fractures, and the urinary excretion of calcium, which was also more reduced in femoral neck fractures. A deficit of vitamin D hepatic metabolite thus appears to be a risk factor for femoral neck fractures in old patients.

Wertheim's radical hysterectomy with pelvic lymphadenectomy in the treatment of cervical carcinoma at the Obstetric and Gynecological Clinic of the University of Pisa.

The Authors report their experience in 49 operations of radical hysterectomy with pelvic lymphadenectomy in the treatment of stage I b and stage II cervical cancer. They describe particularly the precocious and late urinary complications and the arrangements made for their prevention. We had no uretero-vaginal fistula in 21 of the 49 patients, on whom a kind of roof and floor around the terminal tract of the ureters was created by placing some surgical stitches between the posterior wall of the bladder and the anterior lateral wall of the rectum.

Brenner's tumor. Observations on six cases.

Brenner's tumor is a rare fibroepithelial ovarian neoplasia, histologically characterized by an epithelial component similar to the transitional epithelium of the urinary tract. The histogenesis of neoplasm, which only rarely secretes estrogens, is still controversial. The Authors describe the clinical and anatomopathological features of six cases come to their observation. Only one of these was malignant; the woman died 18 months after diagnosis. There is no strict correlation between histological picture and biological behaviour. Only this one can give assurance on the benign or malignant nature of Brenner's tumor.

Hepatic sinusoidal dilation in the course of histiocytosis X.

We present a case of disseminated histiocytosis X with hepatic involvement characterized by sinusoidal dilation. This histological feature regressed in connection with the clinical remission of the disease. The present report suggests that sinusoidal dilation might represent a feature of hepatic involvement in the course of histiocytosis X.

[Prolactin after repeated intravenous injections of cimetidine].

To 4 normal women a bolus of mg 400 of cimetidine was injected intravenously once an hour for three hours in succession and plasma prolactin behaviour studied. No depletion in prolactin pituitary storage was observed in all subjects. The possible usefulness of cimetidine for clinical diagnostic purposes is discussed.

[Cimetidine and hyperprolactinemia].

Plasma TSH was determined in 12 normal subjects before and after administration of mg 400 of cimetidine i.v., an H2-receptor antagonist. TSH concentration remained unchanged. In 7 normal subjects, pretreated with bromocriptine; variation of plasma prolactin were studied before and after administration of mg 400 and 800 of cimetidine. Bromocriptine inhibited the increase of prolactin secretion, induced by cimetidine. It can be assumed that: a) cimetidine doesn't release hypothalamic TRH in portal vessels; b) that drug has no direct effect on pituitary cells; c) hypothalamic H2-receptor blockade by cimetidine decreases dopamine release from hypothalamus to pituitary gland.

[Clinico-statistical review of 74 cases of subcutaneous rupture of the Achilles tendon].

The Authors report 74 cases of subcutaneous hole of Achille's tendon that were treated surgically. 52 patients were clinically controlled at a distance from 1 to 5 years. The functional result at a distance is good in 67,3% of cases, satisfactory in 25%, insufficient in 4%. They point out an hig incidence of local post-operating complications (29,8%), especially when the paratendinea medial incision has been made. The authors suggest to opt for a paratendinea lateral incision and, experimentally, to drop the use of hemostatic string.

Achilles Tendon

[The urological complications of renal ectopia in childhood].

Fifty-four cases of anomalies of position in 52 children were reviewed. Of the 52 children 24 (46%) had anomalies involving organs outside the urinary tract and contralateral renal units. Twenty-five ectopic kidneys showed major urological complications (46%). Four were treated conservatively and 21 with surgery. The postoperative results have been excellent in all the cases. Renal anomalies of position often are associated with congenital renal and extrarenal malformations. Nearly 50% of ectopic kidneys are involved by pathological process that often require surgery. Surgical treatment of urological complications of renal ectopia is discussed.
