[Review of the pathogenetic problem of hereditary ataxia].

In the inherited ataxias, a heterogeneous group of relatively rare and progressive neurological disorders, abnormalities in pyruvate metabolism have been described. Pyruvate is involved in the glycolitic pathway as an important step, and utilized in the Krebs cycle, the main energy source in the brain. Furthermore, pyruvate and other intermediates in the Krebs cycle, can also serve as a precursors of amino acids for which a role as a neurotransmitter has been shown. The explanation of the basic biochemistry may serve as a basis for a more steady knowledge of the clinical and pathological findings of such diseases.

[Alcoholic myopathy. Histochemical and ultrastructural study].

Muscles of alcoholic rats undergo histochemical and ultrastructural study. Significative alterations have not been found. The authors compare their experimental data with analogous human pathology, pointing out the possible biochemical and immunological differences.

[Enzymatic study of peripheral nervous tissue and the retinas of rats subjected to experimental hyperglycemia: effect of ganglioside administration].

The authors submitted to alloxan hyperglycemia albino rats of the Wistar strain. Alloxan is a sulphydryl poison, characterized by its selective action on the beta-cells of the islets of Langerhans. Rats were partly treated with a mixture of gangliosides both for 10 days before hyperglycemia induction and during the following experimental period. Treatment with gangliosides caused in both groups a precocious and significant normalization of metabolic variation seen in hyperglycemic non treated rats. The Authors discuss the physiopathologic significance of the enzymatic variations found in two nervous structures, retina and sciatic nerve, in which initial diabetes-like condition has been induced. Gangliosides, whose use is warranted by recent studies about their effect in cell membrane function, seem to suggest, in this case, a preeminent metabolic action, also if their role is still not completely clear.

[The surgical procedure in a case of accidental lesion due to impalement].

The authors describe a case of intraperitoneal anorectal injury. Such injuries now have an improved survival rate as a result undoubtedly of superior resuscitation, early operative management and early use of antibiotics. Civilian life rectal wounds are usually of the penetrating type. Major complications, both of intra and extraperitoneal injuries, are related to infection. The surgical management of these lesions is primarily a proximal diversion of the fecal stream. This is accomplished by a divided--end colostomy. Local wound care must be given for the inevitable infected foreign tract. When sepsis occurs, the surgeon must be prepared to handle it adequately by appropriate incision and drainage.
