Determination of serum diclofenac by high-performance liquid chromatography by electromechanical detection.

Diclofenac was isolated from plasma by double extraction with organic solvent. Chromatography was performed on Lichrosorb NH2 column with acetonitrile and perchloric acid (0.0025 mol/l; 35:65, v/v) as the mobile phase (flow rate 0.9 ml/min). The electrochemical detector was operated at +900 mV and sensitivity of 5-10 nA. The sensitivity of the method was 10 micrograms/l and the coefficients of variation below 10% in the entire range of the concentration in a dose interval.

Electrocardiogram and high plasma digoxin concentration.

In 48 patients a significantly elevated plasma digoxin concentration (2.5 micrograms/l or more) correlated with the clinical symptoms and the electrocardiogram. The rhythm and conduction disturbances as well as the corrected Q-T interval and the P-T-Q index in the electrocardiogram were analyzed. In 23 patients rhythm and conduction disturbances were found. The clinical symptoms of digoxin overdose were found in 23 patients. There was no significant relationship between the corrected Q-T interval in the electrocardiogram and digoxin concentrations (r = 0.19); P-T-Q index and plasma digoxin concentrations were found to have similar values (r = 0.27), as did prolonged P-Q interval and plasma digoxin concentrations (r = 0.32). A total of 17 of 48 patients had had chronic renal failure. An S-T segment depression mostly of decreasing type was found in the whole group, suggesting that this sign was more reliable as an indicator of digitalis effect than corrected Q-T interval, P-T-Q index, or prolonged P-R interval.

Correlation between fluorimetric and microbiological methods for determination of cephalexin in urine and serum.

The fluorimetric and microbiological methods for the determination of serum and urine cephalexin were compared with the aid of coded samples in two different laboratories. The coefficient of correlation between the two methods for serum cephalexin was 0.94. At urine cephalexin concentrations above 0.5 g/l, the coefficient of correlations between two methods was 0.83. At lower concentrations (0.5 g/l), the coefficient of correlation was 0.72. It was concluded that the tested methods are comparable, equally sensitive and precise.

Aminophylline vs. salbutamol in exercise-induced asthma.

A comparative, double-blind, cross-over trial of aminophylline (6 mg/kg i.v.) and salbutamol (200 mcg per inhalationem) was conducted in six male patients (20 to 46 years) with exercise-induced asthma (EIA). Both aminophylline and salbutamol successfully prevented EIA. The serum concentration of theophylline was within the normal range (9.8--20.3 mcg/ml). The effect of salbutamol was slightly more pronounced than the effect of aminophylline but not statistically significant. Because of the more convenient method of application, the authors give priority to salbutamol in the prevention of EIA.
