DOT examinations: practical aspects and regulatory review.

Most certification examinations of commercial drivers are simple, and relatively few drivers are disqualified. If these examinations are not done properly, however, the public can be exposed to potentially unqualified drivers. Should an accident occur, the physician who examined the driver may be found liable. In performing driver certification examinations, the physician's primary responsibility is to the public. The Federal Motor Carriers Safety Regulations and supporting documents provide guidelines for the conditions that may be disqualifying and the conditions that may allow only temporary certification until better medical control is achieved. Some medical diagnoses, such as insulin-requiring diabetes mellitus, are automatically disqualifying, no matter how well the disease is controlled. Other conditions may require documented clearance from a specialist before certification is granted.

Geographic, age, and racial variation in the treatment of local/regional carcinoma of the prostate.

PURPOSE: Prostate cancer is one of the most common cancers in men. Incidence rates increase with age and are substantially higher in black men than white men. This study examines the variations in the use of radical prostatectomy and radiation by geographic area, age, and race. MATERIALS AND METHODS: Data from the National Cancer Institute's Surveillance, Epidemiology, and End-Results Program (SEER) were used to examine treatment differences. Current treatments generally consist of prostatectomy, radiation, or careful observation for clinically localized or regional disease. RESULTS: The age-adjusted proportion of men, age 50 and older, who received radical prostatectomy increased sharply between 1984 and 1991, from 11.0% to 32.3% among men with local/regional disease. The choice of treatment varied widely by geographic regions. In 1991, the proportion that received prostatectomy was highest in Utah (47.8%) and lowest in Connecticut (22.5%) among men with localized and regional disease. The increase in radical prostatectomy was not limited to younger men. Although the rates increased for blacks, black men had lower age-adjusted rates of prostatectomy than whites in all years of the study. CONCLUSION: The SEER data show a clear trend toward more aggressive treatment, especially prostatectomy. However, the proportion of black men who received prostatectomy was substantially lower than that of white men and this disparity does not appear to be changing.

[Autopsy-confirmed nosocomial infections among the deaths in an intensive care unit].

In a 6-year retrospective study the occurrence of autopsy-confirmed nosocomial infections in 282 deceased patients of an intensive care unit was investigated. The autopsy rate was 100%. In 107 deceased (37.9%) 126 nosocomial infections were confirmed. The most common nosocomial infection was bronchopneumonia (54%). The main ailment was non-tumorous diseases of the digestive system (43.9%). In nearly 60% of the 107 deaths, nosocomial infections were the immediate cause of death. In all 282 deceased, hospital infections were the direct cause of death in 22.7% of these cases. The study emphasizes the importance of nosocomial infections as a causal or contributory factor in the death of patients in an intensive care unit.

[Umbilical cord compression by an amnionic band].

A case report is given from personal observations of an intrauterine death due to constriction of the umbilical cord by amniotic band. Previously reported cases and current hypotheses for the etiology of amniotic band are reviewed and merit clinical attention.

Stimulating effect of tongue on craniofacial growth.

The influence of the tongue on craniofacial growth was studied in 96 Mini-Lewe miniature pigs. The animals were partially glossectomized at different ages and slaughtered at various intervals after operation. The skulls were macerated for biometric analysis. Mandibular growth was significantly reduced lengthwise in animals glossectomized at age 12 weeks. The role played by the tongue in orofacial growth was also indicated by the reduced width of the lower jaw. In pigs partially glossectomized at age 12 weeks, lateral growth of the entire lower jaw was reduced after eight weeks. In animals glossectomized at age six weeks, lateral growth of the lower jaw was reduced in the region of the 1st deciduous molars and the canines after glossectomy. Partial glossectomy had no significant effects on vertical growth of the lower jaw, growth of the upper jaw or overall skull growth. Shortening of the tongue in miniature pigs six weeks old resulted in no measurable jaw changes 23 weeks after surgery.

[Endometrium metastasis of a malignant melanoma of the skin].

There is given a case report from personal observation of a metastasis in the endometrium from a malignant skin melanoma. Difficulties for the differential diagnosis of the tumor can arise during the histo- and cytodiagnosis of adequate currettage and smear. These problems are discussed.

[The tongue as a factor in craniofacial growth. 4. Results of animal experiments].

Post-natal craniofacial growth in the miniature pig MIN-LEWE is influenced by the tongue in various ways. Periodic and locally differentiated intensive craniofacial growth explains why the tongue has a measurable effect on jaw growth only at certain times. This is the situation in animals glossectomized at an age of 12 weeks. Shortening of the tongue in miniature pigs aged six weeks resulted in no measurable changes in the jaw after a prolonged post-surgical interval (23 weeks), which indicates that the capacity of the orofacial system during post-natal development is considerable.

[The tongue as a factor in craniofacial growth. 1. Modification of the linear dimensions of the lower jaw].

The influence of the tongue on craniofacial growth was studied in 96 miniature pigs MINI-LEWE. The animals were partially glossectomized at different ages and slaughtered at various post-operative intervals. The skulls were macerated for biometric analysis. Mandibular growth in length was significantly reduced in the animals glossectomized at an age of 12 weeks. Both the overall length and that of the tooth-bearing portion were affected.

[The tongue as a factor in craniofacial growth. 3. The influence of the height and angle of the lower jaw].

In the miniature pig MINI-LEWE partial glossectomy had no significant effects on vertical growth of the lower jaw, growth of the upper jaw or overall skull growth. No relationship was found between age at glossectomy or post-surgical interval and the size of the gonion angle. The tongue, peri-oral muscles and the muscles of the floor of the mouth are in a state of functional equilibrium which changes in the course of development, the influence of either the tongue or the peri-oral muscles increasing as a result. Partial disruption of this state of equilibrium leads to dysgnathia.

[The tongue as a factor in craniofacial growth. 2. The influence of the width dimension of the lower jaw].

The role played by the tongue in orofacial growth is also indicated by the reduced width of the lower jaw. In miniature pigs MINI-LEWE that were partially glossectomized at an age of 12 weeks lateral growth of the whole lower jaw was reduced after eight months. In animals that had been glossectomized at an age of six weeks lateral growth of the lower jaw was reduced in the region of the 1st deciduous molars and the canines right months after glossectomy.

[Mature cystic teratoma of the uterus].

An account is given of the authors own findings from a mature cystic teratoma in the uterus of a woman aged 33 years. Reference is made to analogous cases reported in earlier literature as well as to the two theories at present on teratoma genesis.
