[Investigation of understanding and willingness of organ transplantation in young people in Beijing, Shanghai, Wuhan].

OBJECTIVE: To understand the knowledge of young people in cities in China about organ transplantation and their willingness to donate organs, methods of donation, and the willingness of donators. METHODS: A questionnaire was designed in investigating 3,012 young people (999 questionnaires in Beijing, 990 in Shanghai, 1,023 in Wuhan) aged 20-45 years in the three cities. The data were processed by SPSS software. RESULTS: Over 90% of the people investigated knew that human organs can be transplanted 85% considered organ transplantation is a warefare undertaking. 85% thought organ transplantation is necessary when someone's organ is severely-impaired. If possible, 70% were willing to receive organ transplantation. With regard to forms of donation, over 70% emphasized the individual willingness combined with reimbursement. The Preferred recipient in sequence were relatives, special institutions, friends, volunteers, and others. The various factors. affecting understanding of organ transplantation were analysed. CONCLUSION: In cities of China, young people have a better understanding of organ transplantation, but conventional attitudes and feudal habits are the major obstacle to the development of organ transplantation in China.

13C NMR spectroscopic and X-ray crystallographic study of the role played by mitochondrial cytochrome b5 heme propionates in the electrostatic binding to cytochrome c.

The role played by the outer mitochondrial membrane (OM) cytochrome b5 heme propionate groups in the electrostatic binding between OM cytochrome b5 and horse heart cytochrome c was investigated by 13C NMR spectroscopy and X-ray crystallography. To achieve these aims, 13C-labeled heme OM cytochrome b5 was expressed in Escherichia coli as previously described [Rivera M., Walker, F.A. (1995) Anal. Biochem. 230, 295-302]. Assignment of the resonances arising from the heme propionate carbons in ferricytochrome b5 was carried out by a combination of one- and two-dimensional NMR experiments. Titrations of [13C]heme-labeled OM cytochrome b5 with horse heart cytochrome c were carried out in order to monitor the resonances arising from the heme propionate carbonyl carbons in OM cytochrome b5. The results from these titrations clearly show that only the heme propionate located on the exposed heme edge in OM cytochrome b5 participates in the electrostatic stabilization of the complex between OM cytochrome b5 and horse heart cytochrome c. Similar experiments carried out monitoring 13C resonances arising from several other heme substituents demonstrated that the stoichiometry of the complex is 1:1. A conditional binding constant, K which equals 3.8 x 10(4) +/- 1.4 x 10(4) at mu = 0.02 M, was obtained for the formation of the complex by fitting the binding curves obtained experimentally to a model based on this stoichiometry. The X-ray crystal structure of rat liver OM cytochrome b5 solved to 2.7 A resolution shows that the structures of bovine liver microsomal cytochrome b5 and rat liver OM cytochrome b5 are almost identical when compared at medium resolution. The similarity between the two structures, combined with the findings that only the heme propionate located on the exposed heme edge of OM cytochrome b5 participates in the electrostatic binding to cytochrome c and that the stability of this complex is similar to that measured for the association between microsomal cytochrome b5 and cytochrome c, clearly indicates that the site of interaction on OM cytochrome b5 is almost identical to the one elucidated for microsomal cytochrome b5. It is therefore possible to conclude that the large body of information gathered by many investigators for the nonphysiological interaction between microsomal cytochrome b5 and cytochrome c (recently reviewed) [Mauk, A. G. Mauk, M. R., Moore, G. R., & Northrup, S. H. (1995) Bioenerg. Biomembr. 27, 311-330] has indeed biological as well as pedagogical validity.

Structure of amylovoran, the capsular exopolysaccharide from the fire blight pathogen Erwinia amylovora.

The acidic exopolysaccharide (EPS) of Erwinia amylovora, amylovoran, was purified from culture supernatants of bacteria in minimal medium and cleaved chemically either by treatment with trifluoracetic acid or hydrofluoric acid, and enzymatically by digestion with depolymerase from E. amylovora phage phi-Ealh. Structural characterization of the resulting oligosaccharides was performed by a combination of mass spectrometric and NMR [one- and two-dimensional (1D and 2D)] spectroscopic techniques. A branched repeating unit with five monosaccharide residues and various substituents was determined: [sequence: see text] The terminal monosaccharide of the side branch, which bears a 4,6-bound pyruvate residue in the R-configuration, was found to be modified with 2-linked (26%), 3-linked (24%), 2-,3-linked (40%) O-acetyl groups, or these were absent (10%). An additional glucose residue is linked to approximately 10% of the core alpha-galactose of the repeating unit.

Immunohistochemical analysis of growth factor expression and localization in gastric coronary vein of cirrhotic patients.

This study was designed to investigate the expression and localization of PDGF, bFGF, EGF, and TGF alpha in gastric coronary vein of cirrhotic (n = 30) and non-cirrhotic patients (n = 10) using immunohistochemical technique. The strongly positive immunostaining rate were 93%, 89%, 70% and 68% respectively in cirrhotic patients. The immunostaining was negative in non-cirrhotic patients. The damage to endothelium, hypertrophy and hyperplasia of vascular smooth muscle cells and vascular remodeling were seen in gastric coronary vein of cirrhotic patients. These results suggested that gastric coronary vein could produce growth factor during cirrhosis, the growth factor can act on the vascular function and/or structure via autocrine-paracrine mechanism.

[Selective embolization of coronary vein of the stomach--a new approach for the treatment of bleeding from esophageal varices].

Selective embolization of coronary vein of stomach was employed for the treatment of 222 patients with bleeding from esophageal varices due to portal hypertension. The coronary vein was separated at middle segment and tube was inserted and clamped 5 cm below and above the cardia. The gamma-octyl-cyanoacrylate was used for embolization. 19 patients received preventive operation, 106 was subject to emergency surgery and 98 patients was treated by elective operation. The bleeding of all patients receiving emergency procedure was stopped. 3 died of liver failure after operation. 219 survived. 189 cases were followed up for 1-2 years and on varices was found. 182 cases followed up for 6 years revealed no relapse of bleeding. This technique could achieve good curative effect, which provides a new approach for the treatment of bleeding from esophageal varices of portal hypertension.

Pharmacological properties of traditional medicines. XXII. Pharmacokinetic study of mulberroside A and its metabolites in rat.

To identify the active components in Mori Cortex (Chinese medicine), Mori Cortex extracts were administered orally to rats, and then blood plasma, urine, and bile were analyzed. The results showed only a few remaining metabolites of mulberroside A. Consequently, to clarify the transitional properties of mulberroside A derivatives to tissues and the in vivo abundance ratio of mulberroside A compounds, the pharmacokinetics of mulberroside A derivatives were investigated in this study. When Mori Cortex extracts were administered orally, mulberroside A was only detected in small quantities in the plasma, and its bioavailability was about 1%. This is due to the first pass effect, by which most mulberroside A was converted into oxyresveratrol and transported into the circulating blood, and its absorption ratio was estimated at about 50%. Oxyresveratrol was found to be transported to tissues at high rates. As a result, when analyzing the pharmacological activity of the Mori Cortex in vitro, it is more useful to study oxyresveratrol than mulberroside A.

Vasoactive intestinal peptide receptors in brains of hepatic encephalopathy rats. An experimental study.

We evaluated the effect of vasoactive intestinal peptide (VIP) receptor, a brain-gut peptide receptor which is capable of exciting central neurons, on the pathogenesis of hepatic encephalopathy (HE). By means of radio-ligand binding assay, VIP receptors in the crude synaptosomal membranes of rat brains were investigated in rat models with HE induced by partial hepatectomy following carbon tetrachloride intoxication and in controls. The binding to the VIP receptor was studied, with 125I-VIP as a radioligand. Scatchard analysis of the binding data was linear, yielding a dissociation constant (Kd) of 0.28 +/- 0.01 nM and a maximal binding capacity (Bmax) of 9.56 +/- 0.29 fmol/mg of protein in HE rats. Only decreased Bmax values were observed (P < 0.002), while the Kd values were statistically unchanged (P > 0.20) in HE rats as compared with the controls. The results suggest that the changes of VIP receptors in the brains of HE rats play a significant role in the pathogenesis of HE. The mechanism of HE induced by the alterations of VIP receptors in the brains was discussed.

A new staging system for nasopharyngeal carcinoma in China.

PURPOSE: An accurate and rational nasopharyngeal carcinoma (NPC) stage based on images is proposed. METHODS AND MATERIALS: Four hundred and twenty-one cases of NPC, treated in the Cancer Hospital, Sun Yat-sen University of Medical Sciences (SUMS), with computed tomography scanning before initial radiotherapy, are analyzed. Important prognostic factors that form the basis of the new staging system are screened out by means of Cox model and clinical experiences. Survival curves of various kinds of T and N stages are compared by computer simulation. A new staging system is proposed by studying the traditional staging systems such as the AJC, UICC, Ho's and Changsha systems. RESULTS: According to the new staging criteria, the 5-year survival rates for Stages I-IV are 89.7%, 75.9%, 51.3%, and 22.2%, respectively. CONCLUSION: This new clinical staging for NPC based on a large amount of cases multivariate analysis is satisfactory and widely used in China.

Experimental study on ischemia and reperfusion injury of rat liver and effects of ligustrazine and salvia compound.

The present study was to investigate changes of free radicals in the whole rat liver, changes of ATP levels of hepatic cells, ultrastructural changes in hepatic tissue during ischemia and reperfusion in rats and the effects of ligustrazine and salvia compound. The results indicated that: 1) the free radical levels increased by 8.3-fold and 9.0-fold in the groups with 30 and 60 min ischemia after 40 min reperfusion, as compared with the group with ischemia only (P < 0.01, P < 0.01, respectively). 2) The levels of ATP returned to normal in the group with 30 min ischemia after reperfusion for 40 min and 3 d, remained low in the group with 90 min ischemia and fell again after a mild increase in the group with 60 min ischemia. 3) The hepatic and endothelial cell damage after ischemia became more severe after reperfusion, as revealed by electron microscopy. The present study also showed that ligustrazine and salvia compound have protective effects against reperfusion injury. They can be used to scavenge free radicals, improve hepatic microcirculation and alleviate hepatic and endothelial cell damage.

Guardianship networks for rural psychiatric patients. A non-professional support system in Jinshan County, Shanghai.

This paper describes the 'rural guardianship network' method of managing persons with severe mental illnesses and reports on a retrospective case-control study that compares the outcome of 561 schizophrenic patients from townships with guardianship networks to that of 103 schizophrenic patients from a township without guardianship networks. Each patient enrolled in a guardianship network is assigned a guardian--usually a family member--who ensures that the patient takes medication and gets medical follow-up, and reports any changes in the patient's condition to the village health worker or the township doctor who is responsible for psychiatric care. These guardians are supported and supervised by management groups at the village and township levels composed of government officials, medical administrators, physicians from township hospitals, and village health workers. Compared with patients who are not enrolled in guardianship networks, enrolled patients have significantly lower rates of hospital admission, lower rates of socially disruptive behaviour, and lower levels of psychopathological symptoms. The rural guardianship network is an effective and economical means of providing close supervision of mentally ill patients in their homes.

Determination of the pattern of methyl esterification in pectin. Distribution of contiguous nonesterified residues.

A method is described for determining the distribution of contiguous nonesterified galacturonic acid residues within pectins. First, the esterified galacturonic acids are converted to galactose by reduction with sodium borohydride, then the glycosidic linkages of the resulting galactose residues are cleaved selectively by liquid HF solvolysis. Separation and quantitation of the resulting galacturonic acid containing oligomers reveals the proportion of each stretch of contiguous nonesterified galacturonic acid residues in the original pectin. The distribution of nonesterified GalA in a pectin fraction obtained from cotton suspension culture cell walls with approximately 50% esterification appears to be far from random.

New techniques for the rapid characterization of oligonucleotides by mass spectrometry.

Recent advances in combined HPLC/electrospray ionization-mass spectrometry provide effective new capabilities for the rapid characterization of oligonucleotides. Accurate mass measurements with errors < 0.3 Da, and determination of base and sugar modification and of nearest neighbor identities, can be routinely carried out on 10-100 component mixtures of RNA or DNA. These procedures are widely applicable in structural and analytical studies involving mixtures of oligonucleotides.
