Partial hepatectomy for Caroli's syndrome. Report of two patients with long-term follow-up.

Caroli's syndrome comprises cystic dilatations of the intrahepatic bile ducts, which may be focal or diffuse. Partial hepatectomy, when possible, is the best current treatment. We report on two patients with Caroli's syndrome confined to one side of the liver, who were subjected to partial hepatectomy. Both patients had a previous history of biliary tract surgery. Transhepatic percutaneous cholangiography and abdominal CT scan were helpful in establishing the diagnosis and the extent of the disease. After hepatectomy, one patient developed an abdominal abscess that required surgical drainage, the other developed persistent fever, which resolved spontaneously. Both patients are asymptomatic three and five years post-hepatectomy, respectively, and there is no clinical evidence of recurrence of the disease. The diagnostic work-up and therapeutic choices for this rare entity are discussed.

Diagnosis of ectopic pregnancy: endovaginal vs transabdominal sonography.

During a 25-month period, 193 women with the clinical diagnosis of suspected ectopic pregnancy had transabdominal and endovaginal sonograms. Most had quantitative determinations of serum human chorionic gonadotropin (HCG). Endovaginal sonography was diagnostic of ectopic pregnancy in 23 (38%) of the 60 patients with surgically proved ectopic pregnancies: transabdominal sonography was diagnostic in 13 patients (22%). All 83 intrauterine pregnancies were identified with endovaginal sonography, compared with 34 identified with transabdominal sonography. Endovaginal sonography was somewhat more helpful in the diagnosis of missed abortion and blighted ovum. Eighty endovaginal sonograms were classified as indeterminate as compared with 141 transabdominal studies. This indeterminate group included patients with complete abortions, ectopic pregnancies without sonographic evidence of an extrauterine gestation, incomplete abortions, and patients with subsequent negative serum levels. As in prior reports, endovaginal sonography was superior to transabdominal sonography in the evaluation of suspected ectopic pregnancies. Overall, endovaginal sonography was diagnostic in 113 patients, whereas transabdominal sonography was diagnostic in 52 patients. The finding of an extrauterine fetal pole or embryo was diagnostic for an ectopic pregnancy. Pelvic fluid, the appearance of the endometrium, and a single positive serum HCG determination were not helpful in making the diagnosis of ectopic pregnancy.

Primary biliary carcinoma: CT evaluation.

Fifty-three patients with documented primary biliary carcinoma were studied with computed tomography. Twenty-six patients had gallbladder carcinoma and 27 patients had carcinoma of the biliary ductal system. Ninety percent of patients with gallbladder cancer had an intraluminal mass. Local invasion into the liver was common. The majority of patients with biliary ductal carcinoma had dilated bile ducts, while only 50% of patients with gallbladder cancer had biliary ductal dilatation. The most common location of tumor involving the extrahepatic biliary ductal system was the distal common bile duct. This occurred in eight patients out of 27, or 30% of the cases.

Sagittal and coronal image reconstruction: application in pancreatic computed tomography.

The application of coronal and sagittal image reconstruction in the display of the normal pancreas and peripancreatic region is illustrated. The use of standard reproducible reference points for multiplanar reconstruction is suggested. Reconstruction computed tomography displays more anatomic structures in the coronal and sagittal planes than ultrasonography and accurately displays the craniocaudad extent of pancreatic pathology.

Ultrasonic distinction of abscesses from other intra-abdominal fluid collections.

The nature of 103 intra-abdominal fluid collections examined by ultrasound was determined by autopsy, needle aspiration, spontaneous drainage or surgical exploration. Abscesses, chronic hematomas, lymphoceles, urinomas, cysts and fluid-filled bowel loops are all approximately elliptical or circular in cross section. Collections of ascites have a very irregular outline. Bowel loops appear as multiple circles, all of similar size. Chronic hematomas contain strongly echoing material, while lymphoceles, urinomas, renal and hepatic cysts and collections of ascites are echo-free. Ovarian cysts into which there has been bleeding may contain echoes. The walls of abscesses, chronic hematomas, and bowel loops are less sharp than the walls of other collections. Intrahepatic abscesses may have more ragged walls than abscesses elsewhere. Ultrasonic characteristics of abscesses and hematomas overlap. It is sometimes possible to distinguish the nature of the fluid by its ultrasonic properties, but atypical appearances are not rare.

[Hepatic fibropolycystic disease in Mexico. Study of 82 cases].

We analyzed 82 cases of hepatobiliary fibropolycystic disease (FPD) that were seen at the Instituto Nacional de la Nutrición "Salvador Zubirán" in Mexico City in the thirty-year period comprised from 1956 to 1986. The different entities that compose FPD were distributed as follows: 61 (74%) cases of polycystic liver disease, 13 (16%) cases of choledochal cyst, and 8 (10%) cases of congenital hepatic fibrosis; there were 5 (6%) cases of Caroli's disease, 3 associated with congenital hepatic fibrosis and 2 with choledochal cyst. Polycystic liver disease predominated in females (67%) and presented at 54 +/- 12 years (mean +/- SEM) with pain, a mass, symptoms related to renal insufficiency or incidentally; polycystic kidneys were present in 61%. Liver function tests were normal in 94%. Choledochal cyst also predominated in females and presented at a mean age of 19 years with cholangitis. Liver function tests were abnormal in 69%. Congenital hepatic fibrosis (50% male) presented with variceal hemorrhage or cholangitis (in 3 patients associated with Caroli's disease). Polycystic kidneys were present in five patients. Four of the five patients with Caroli's disease were female and presented at a mean age of 19 years with cholangitis. It never presented as an isolated disease, but was associated more frequently to congenital hepatic fibrosis. The diseases that are part of the hepatobiliary polycystic disease vary in severity and thus the prognosis in an individual patient is determined by the type of fibropolycystic disease present. This is the largest series of this disease published in our country.

Meningioma-like intracranial granulocytic sarcoma (chloroma). Radiologic and surgical findings.

We present the case of a 23 years old female with chronic granulocytic leukemia and two intracranial tumors with all the radiological characteristics of intracranial meningiomas. Successful removal of the largest tumor was accomplished using the technique described for meningiomas. The recovery was very good and the histopathologic diagnosis was of granulocytic sarcoma. Radiotherapy of the second tumor reduced the tumor mass within one month leaving the patient neurologically intact. We recommended surgery in the future treatment of large chloromas.
