Mothers' benefit of a self-care booklet and a self-care educational session at child health centres.

Mothers' benefit of a mass-distributed self-care booklet and a self-care educational session about young children's minor illnesses were investigated. The following questions were studied: (1) Do mothers of infants read and benefit from a self-care booklet? (2) Is the mothers' knowledge about self-care improved more if the booklet is supplemented by a self-care educational session? 265 (92%) of 288 randomly sampled Swedish mothers whose first child was 6-12 months of age were interviewed regarding their knowledge about self-care. Shortly thereafter a self-care booklet was mailed to all households in Uppsala county and 6.5 months later a new group of 572 mothers was randomly sampled. Among the 572 mothers, 332 (58%) were randomized to a self-care session organized at child health centres and 240 (42%) were not invited to a session. Of the 572 mothers, 530 (93%) were later interviewed about the booklet, the session and their knowledge about self-care. 69% participated among the mothers randomized to the session. Among these mothers, 99% recalled the booklet and 96% had read the child care section. Among the mothers not invited to the session, 68% recalled the booklet and 60% had read the child care section. The mothers who had read the child care section would follow the recommendations about when to seek (and not to seek) medical care significantly better than those who had not read it (P less than 0.003).(ABSTRACT TRUNCATED AT 250 WORDS)

Disability in children with myelomeningocele. A Nordic study.

This study was carried out to evaluate the medical impairments and disability among 527 children with myelomeningocele living in four Nordic countries. The information was extracted form questionnaires and data based on case records. The level of intellectual functioning was fairly high with 76% within normal range (IQ greater than 90). Shunt operations were performed in 68% of the children and 1/3 had had four or more shunt revisions. Twenty-three per cent walked without support. Bladder incontinence was found in 60% of the children whereas 4% were entirely continent. Intermittent catheterisation was used in 36% with higher frequency among girls (48%) than boys (24%). We found a higher incidence of bacteriuria among those who used catheterisation than among those who used manual compression. Bowel control was experienced by 112 (21%) of the children. The overall disability is classified according to Lorbber's 5-graded handicap scale and Lagergren's 3-graded scale. According to Lagergren's scale, 70% suffered form severe overall disability.

Life table methods applied to use of medical care and of prescription drugs in early childhood.

Life table methods were applied to analyses of longitudinal data on the use of medical care during the first 5 years of life among all 1701 children born in a Swedish semirural municipality. Cumulative proportions of the children who had used particular types of medical care or prescription drugs at least once by certain ages were estimated. By the fifth birthday, 98% had made at least one visit to any physician and 82% at least one visit to a paediatrician. By the fifth birthday at least one prescription for antibiotics had been purchased at a pharmacy by 82%; and 33% had been admitted to inpatient hospital care at least once (excluding immediate postnatal care). Acute conditions and more chronic diseases were also studied using these methods. At least one visit to a physician at a primary health care centre had been made for acute otitis media in 65% of 5 year olds and for atopic dermatitis in 8%.

Treatment of advanced prostate cancer.

In the management of advanced prostatic cancer, available treatments are reviewed. Preliminary results of several studies show a better response to complete androgen blockade than to conventional treatments. Prostatic tumour consists of androgen-dependent and -independent cells, and the continued growth of the latter, following androgen ablation, results in relapse.

Ofloxacin compared with ciprofloxacin in the treatment of complicated lower urinary tract infections.

In a double-blind, double-dummy study 61 patients with structural or functional abnormalities of the urinary tract and verified infection were treated for 7 days with ofloxacin 100 mg bd or ciprofloxacin 250 mg bd by mouth. Of 62 isolated strains none was resistant to ofloxacin or ciprofloxacin in vitro. Nineteen patients (63%) in both groups were free from infection ten days after therapy. Clinical resolution of symptoms occurred in 83% in the ofloxacin group and 68% in the ciprofloxacin group. Adverse reactions were reported in four patients (6.5%), two with skin rash, one with gastrointestinal disturbance and one with influenza-like symptoms. This study demonstrates that ofloxacin and ciprofloxacin are effective in the treatment of complicated lower urinary tract infection.

The effect of radiographic contrast media on the chemotaxis of granulocytes.

The direct in vitro effect of radiographic contrast media (RCM) on granulocyte locomotion was assayed using an under agarose technique. All RCM tested inhibited the chemotactic response to N-fmlp, a synthetic chemotactic N-formylated tripeptide, which is an analogue of natural bacterial products. The inhibition was most pronounced for diatrizoate, the most hyperosmolar of the RCM, but hyperosmolality per se was not the only inhibitive factor, since 0.5 M NaCl, which has an osmolality comparable to the RCM used, did not inhibit chemotaxis significantly. Furthermore, the inhibition was not caused by a nonspecific sticky effect of RCM on granulocytes, since spontaneous migration was normal. We propose that the inhibition might be caused by competitive binding of RCM to the N-fmlp receptor sited on leukocytes because the third amino acid in N-fmlp is phenylalanine, which, like the RCM, contains a benzene ring.

Use of antibiotics and prescription drugs in general during the first 9 years of life in a Swedish community.

The use of antibiotic drugs and of prescription drugs in general during the first 9 years of life was studied among all 1701 children born in a Swedish municipality. Cumulative proportions of children who had received one or more prescriptions for any type of drug and for different groups of antibiotic drugs were estimated by life-table methods. The effects on drug use of the gender and the birth order of the child along with the age, citizenship and marital status of the mother were analysed by Cox's proportional hazards regression model. Half of the children had received at least one prescription for any drug after 0.6 years and at least one prescription for any antibiotic drug after 1.8 years. Higher proportions of children with prescriptions for all drugs and for all antibiotic drugs were found among males than females, among those with older siblings compared with first-borns and among children of younger mothers compared with those of older mothers.

Flutamide versus stilboestrol in the management of advanced prostatic cancer. A controlled prospective study.

In a prospective randomised study the effect of flutamide 750 mg daily was compared with that of stilboestrol 3 mg daily in the treatment of 40 previously untreated patients with advanced prostatic cancer. There was a good subjective response to both treatments. After 12 months, a response was demonstrated in 13 of 20 patients treated with flutamide and 8 of 20 patients treated with stilboestrol. The difference was not statistically significant. Treatment with stilboestrol caused more frequent, and more severe, side effects than flutamide.

Aged

Good prognosis after calcaneal fracture in childhood.

Fifteen children who sustained a displaced intraarticular calcaneal fracture, treated closed, were reexamined after 12 (1-29) years. All were satisfactory, although slight inconvenience was encountered as a result of the fracture. Immobilization and nonweight bearing from 4 to 6 weeks seem sufficient to treat a calcaneal fracture in children.

Hypotensive shock associated with bradycardia after intravenous injection of contrast medium.

A case of hypotensive shock associated with bradycardia that occurred after injection of nonionic monomeric contrast medium is described. Whether the pathogenesis of this type of reaction is solely vasovagal or is directly caused by the contrast medium is not known. The number of occurrences after administration of nonionic monomeric versus conventional contrast agents should be compared to solve this problem.

Unique etiology of tubal ectopic pregnancy.

We report a case of hysterosalpingography inadvertently performed during early intra-uterine pregnancy. The radiologic procedure seems to have displaced the amniotic sac from the uterine cavity into the Fallopian tube, and thus has been the etiologic factor underlying a tubal ectopic pregnancy.

Physician visits and prescribed drugs among young children and their mothers.

The extent to which use of medical services by young children could be predicted by mothers' use of medical services and by certain family characteristics has been studied. The study population comprised 591 children two to four years of age in a Swedish rural municipality. Register data on physician visits and prescription drug purchases during 1979 constituted main sources of information. Statistically significant positive associations were found between number of physician visits made by the children and by their mothers and between number of prescription drug purchases made for the children and for the mothers. More physician visits and prescription drug purchases were made for the children of younger mothers than for those of older mothers. Six to eight per cent of the variation in the use of medical services by the young children was explained in multiple regression models including mainly mothers' use of medical care, mothers' age and children's age.

Lunatomalacia in a child.

A case of advanced lunatomalacia in an 8-year-old girl treated by immobilization is described. After 2 years of treatment, the girl had a normal wrist, and radiographically the condition had improved, albeit with some collapse of the radial part of the lunate. Our case suggests that the natural course of lunatomalacia is favorable in children as compared with adults.

Calcaneus fracture in the child.

We reviewed 80 fractures of the calcaneus in 78 children through a 30-year period. The fractures were more equally distributed between the sexes than in adults. In contrast to adults, intraarticular fractures of the calcaneus in children constitute less than half, probably because a lower proportion of children sustain the fracture as a result of a fall. Associated injuries were mainly encountered by children who were traffic accident victims. It is disputable that the calcaneus in children is more resistant to fracture than in adults. The rarity of the fracture in children is more likely the result of less exposure to trauma leading to calcaneal fractures and also that the fracture is often overlooked.
