[Terminology of ultrasound vascular diagnosis].

There is at present no accepted nomenclature regarding diagnostic vascular ultrasound. At two scientific meetings, the Study Group for Vascular Diagnosis of the German Society of Ultrasound in Medicine (DEGUM) developed a terminology for Doppler and duplex sonography of the arteries and veins supplying the brain and of peripheral vessels. The most important of these sonographic terms and their synonyms are presented. Recommendations as to which of the presently employed terms should be avoided, are given.

[Carotid endarterectomy. Risks and benefits of treatment].

From 1980 to 1985, 210 endarterectomies of the carotid artery were performed in 187 patients (50 female, 127 male). In 55 cases there were asymptomatic filiform stenoses with concomitant high grade stenosis or occlusion of the contralateral artery. 155 stenoses of the carotid artery of mostly the highest degree were symptomatic. Patients were examined by means of the direct CW-doppler ultrasound immediately postoperatively and every six months up to six years. In 9 cases (4.3%) there was a perioperative (central) neurologic incident, which in 5 (2.3%) cases was followed by permanent neurologic deficits. Three of these incidents were caused by an early occlusion of the reconstruction. The stroke rate (including perioperative stroke rate) determined by life table analysis for the whole group after 6 years was 5.6%. The rate of TIA's was 4.5%. The rate of local recurrence (stenosis greater than 50% or occlusions) after six years was 15.3%. Except for the above mentioned early occlusions only one recurrent stenosis was symptomatic. We believe that asymptomatic, non multimorbid patients with doublesided high grade stenoses and symptomatic patients profit from an endarterectomy of the carotid artery, as long as the perioperative stroke rate and mortality do not exceed 3-4%.

The influence of low arylsulfatase A activity on neuropsychiatric morbidity: a large-scale screening in patients.

A total of 1728 patients consecutively admitted to a neuropsychiatric hospital and 379 chronically ill inpatients were examined for activity of arylsulphatase A (ASA) in leucocytes. A further 519 healthy individuals served as controls. We did not find evidence for the involvement of low ASA activity in chronic patients. The consecutive admissions showed a slight preponderance in the lower ASA activity classes. This activity range covers persons heterozygous for ASA deficiency alleles. The data are compatible with the hypothesis that carriers of low ASA activity alleles are at a slightly higher risk for neuropsychiatric disorders.

[Extracranial carotid stenosis and its sonographic diagnosis].

The direct Doppler examination of the carotid arteries as a method to detect and distinguish stenoses and occlusions of the common, internal, and external carotid arteries allows to decide on operability and indication for surgical treatment. The exact observation of the acoustical flow signals permits to a high degree an identification of even small stenoses and arteriosclerotic plaques. In 125 angiographical controls 95% of the sonographic diagnoses, in stenoses larger than 50% correct diagnosis could be established even up to about 99%. There were no false positive sonographic findings. Because of our good results we did not consider it necessary in recent time to perform an angiography before surgery in 25 patients. These patients had a high angiographic risk and there was a clear cut Doppler finding of an extreme stenosis in the internal carotid artery.

[Treatment of alcoholic toxic fatty liver. A placebo-controlled trial with hepavis (author's transl)].

A placebo-controlled double-blind study of 30 patients with advanced chronic fatty liver was intended to show how far treatment with a liver preparation, in this case Hepavis, is superior to alcohol abstinence alone. Examination of the laboratory parameters, especially of gamma GT shows that treatment with Hepavis with simultaneous withdrawal of alcohol produces a rapid normalization or improvement of the laboratory findings and consequently an accelerated improvement in the course of the disease. This leads to the conclusion that the pathological activity of the liver cell is reduced more quickly by the constituents of Hepavis than without suitable therapy, and that a more favorable prognosis for the fatty liver as a potential precurser of cirrhosis is to be attained. Not only is elimination of the lipogenic factor, alcohol, essential in the treatment of fatty liver, but also treatment with hepatotropic substances.
