[Hepatitis non-A, non-B-associated DNA--demonstration of DNA in proven infectious anti-D-immunoglobulin].

An anti-D-immunoglobulin preparation implicated in a hepatitis non-A,non-B transmission was analyzed for the presence of a DNA, which was originally isolated, cloned and sequenced from feces of a patient with posttransfusion HNANB. The investigation was performed by a DNA polymerase chain reaction using synthetic oligoprimers. Commercially available immunoglobulin preparations served as controls. The demonstration of identical DNA sequences in the infectious material speaks in favour of this up to now unknown circular and partially double-stranded DNA to be a virus genome involved in hepatitis non-A,non-B.

Non A, non B hepatitis: ultrastructural findings in human liver biopsies.

Dense intranuclear particles 30 to 40 nm diameter, could be identified in liver specimens from female patients, who immediately after delivery had been inoculated with sera contaminated with non A, non B hepatitis. The same particles were described previously by KENDREY et al. (1975) in a case of acute hepatitis without clinical or serological evidence for hepatitis A and B. The joint occurrence of dense intranuclear particles with typical core particles in cases of chronic B hepatitis show that they are not specific for non A, non B hepatitis. The observed structures are likely to be nonspecific responses of hepatocytic nuclei to different hepatitis viruses. The nature of relationship to the infectious agent remains to be elucidated. Tubular structures in the endoplasmic reticulum and other cytoplasmic inclusions which had been demonstrated in liver cells from chimpanzees inoculated with non A, non B hepatitis, did not occur in our material.

[Alcohol and the liver].

Alcohol is the most significant liver poison. Its degradation takes above all place by the alcohol dehydrogenase and the microsomal alcohol-oxidizing system. In the first step of degradation acetaldehyde develops which in enrichment evokes immediately toxic defects on the mitochondrias of the cells of the liver parenchyma and thus introduces a vicious circle. Furthermore, an increased affection of pharmacometabolites as a sequel of the alcohol-conditioned enzyme induction may lead to a defect. Alcohol influences intermediary metabolic functions: the gluconeogenesis is inhibited, multi-layer disturbances in the lipid metabolism lead to fatty degeneration of the liver. A hyperuricaemia results from overproduction in the liver as well as from decreased renal excretion. The proline formation is increased. Distinct increase of the gamma-GT-activity is an early and relatively specific indicator of the alcoholic liver defect. Morphologic and clinical manifestations are fatty degeneration of the liver, hepatitis based on fatty degeneration of the liver and cirrhosis. Apart from dose and duration of the alcohol intake additional factors require consideration. The author adopts a definite attitude to etiopathogeneis and therapeutic possibilities.

Acetaldehyde

[Epidemiology, clinical aspects, immunology and morphology of non-A, non-B hepatitis].

Examinations were carried out in a population group of 106 potentially contaminated females and in five donors, affected one year earlier within the frames of a restricted epidemic, caused by NANB-hepatitis virus. It was possible to differentiate a specific antibody-antigen system, identical to that of sirahi et al. The antigen was confirmed in 20 per cent of the females, 21 per cent of the contaminated did not get ill. The high SGPT level came in the foreground in the rest; 38 per cent had the signs of cholestasis. After one year, high transaminases were found in 43 per cent of the diseased, and biopsically--a chronic persistent hepatitis was found or a hepatitis with low activity. The chronification tendency correlates with the favourable clinical findings during the initial acute phase. The patients with prodromal phenomena, high transaminases and cholestasis showed a tendency to noncomplicated course and completely healing. The extraparenteral infection was confirmed, being conditioned by the close contact between mother and child or during breast feeding on one hand, on the base of the clinical manifestations, and biopsically--on the other.

[Risk and ethical problems in endoscopy].

The explosive growth of natural-scientific knowledge and its utilization in medicine has enlarged the diagnostic and therapeutic possibilities. A change of the physician's work is connected with the laboratory and apparative-technical medicine. The simple methods of the anamnestic, at the same time psychoanalytic talk and the immediate examination of the patient are no more used according to their value. The immediate meeting between physician and patient is being forgotten. At the instance of the gastroenterological endoscopy the importance and methodology of the permanent collective judgment of examination techniques under management of a subspecialised team as well as the definition and control of risk and ratio expenses-use are described. For the testing of techniques as well as their use in routine work an informing talk with the patient is necessary as well his agreement, according to the directives of the WHO. Work and responsibility of the physician in Socialist Public Health is characterized by inclusion of state and society and the conscious participation of the citizens.

[Demonstration of hepatitis A virus in the feces].

On the basis of own examinations methodology and results of the demonstration of the virus A in the stools and of the proof of antibodies in the serum are described. The parameters of the virus-A-hepatitis which are important for practice are further the basis for diagnosis, therapy and surrounding prophylaxis in routine work. They were compiled in a table.

[Perspective of tuberculosis control--role of preventive chemotherapy (author's transl)].

After a short review of measures for control and eradication of infectious diseases in general and especially of tuberculosis, the indications of chemoprophylaxis and preventive chemotherapy of tuberculosis are discussed. Using results of a double-blind controlled trial on preventive chemotherapy in a high risk group (persons with fibrotic lung lesions), cost, benefit and side effects of preventive chemotherapy are analysed. Preventive chemotherapy in high risk persons is an effective measure in advanced stage of tuberculosis control. Recommendations to reduce side effects are given.

[EDP (electronic data processing) and medicine].

The paper deals with the description of the regions of the use of automatized processing of medical informations; i.e. the basic processes of the information processing referred to patients in the hospital, the information and documentation referred to patients, the information relations between the place demading efficiencies and bringing efficiencies in the medical institution including systems organized in the territory as well as the finding of decisions in medicine. With the description of the individual regions of use are connected considerations of the utility, issuing from the project of the electronic data processing realised at the Medical Academy Dresden. The regions of application described make clear that the tasks of the automatized processing of medical informations extend to the following: Taking over of functions of the description of information and the storage of information, taking over of functions of the comparison of informations and the control of information, taking over of functions of the organisational preparation and the planning of diagnostic and therapeutic courses as well as the taking over of functions of the information integration. It is clearly to be established that the electronic data processing shall not restrict the turning of the physician to the patient, but shall improve and enlarge it. The electronic data processing is a modern means of work which with progressing scientific and technical development medicine needs just as much as other technical procedures and methods.

Documentation

[The frequency and form of manifestation of liver damage under INH therapy].

It is reported on 305 clinically examined patients with the diagnosis of a functional colon syndrome. In 65 patients somatic diseases were found to be the possible triggering factors. In contrast to a comparative group in the other 240 patients in the former anamnesis more frequently allergies, appendectomies and chronic application of purgatives were to be established. The most frequent complaints were irregularity of defaecation in form of spastic obstipation (62.9%) or diarrhoea (35.4%), intolerance to foodstuff (100%), pain in the colon(66.7%) and meteorism (65.4%). The 4th decennium was most frequently concerned in men as well as in women.

Alcoholism

[Immunopathological phenomenona in liver diseases].

More than 400 patients with acute and chronic liver diseases as well as non-hepatic diseases (control group) were examined for the existence of antibodies against mitochondria, smooth musculature and cytoblasts with the help of the immunofluorescence-serological method. The proof of the hepatitis-B antigen was carried out by means of the transmigration-electrophoresis. Auto-antibodytitres larger than 1 : 160 (limiting titres) could be established only in the chronically aggressive hepatitides and the chronically destructing non-purulent cholangitides. Whilst in the chronically destructing non-purulent cholangitides dominate high mitochondrial antibody-titres, in the chronically aggressive hepatitides the antibodies against smooth musculature and/or cytoblasts are dominating. When the hepatitis-B-antigen was present in all groups of diagnosis auto-antibody titres of 1 : 160 were not transgressed. The kind of the antibody-spectrum and the size of the antibody-titres seem to be suitable for the differential diagnosis. High mitochondrial antibody-titres in women are suspicious of a chronically destructing non-purulent cholangitis. High titres of antinuclear antibodies or such ones against smooth musculature might be a reference to the so-called auto-immune form of the chronically aggressive hepatitis. For a still better differentiation of the chronic liver diseases the determination of these antibodies seems to be valuable.
