Risk and protective factors associated with youth violence among secondary school students in a nationally representative sample in Puerto Rico.

During the past decade, youth violence has received increasing attention as a major public health issue in Puerto Rico as well as in the United States. This study sought to identify risk and protective factors of youth violence in a representative sample of school adolescents in Puerto Rico. Risk and protective factors were grouped into five domains: individual, family, peer group, school and community. From a total of 2,385 participants, 10.7% reported at least one violent behavior and 3.4% reported two or more violent behaviors. In multiple regression analysis the risk factors identified were male gender, junior grade students, having a favorable attitude towards antisocial behavior, use of ecstasy, involvement with antisocial peers and reporting antisocial parents. Participation in family decisions was the only protective factor associated with violence. Findings from this study could have important implications for the development of preventive programs for the adolescent population in Puerto Rico.

[Effectiveness of postoperative autologous blood transfusion after knee replacement surgery and influence of preoperative hemoglobin level].

OBJECTIVE: To evaluate the effectiveness of postoperative autologous blood transfusion on reducing the need for allogenic transfusion during recovery from total knee arthroplasty until hospital discharge, and to determine whether effectiveness is related to preoperative hemoglobin level. MATERIAL AND METHODS: Retrospective study of patients undergoing surgery at Hospital Jerez de la Frontera, Spain, in 2003, assessing the association between postoperative autologous blood transfusion, preoperative hemoglobin, and allogenic transfusion requirements. RESULTS: A total of 107 patients were studied. Eighty-three received autologous blood transfusions after surgery and 15 (14.02%) required allogenic transfusion. The rate of allogenic transfusion was higher in association with hemoglobin levels exceeding 13 g x dL(-1) (P=0.003) and it was lower in patients who received autologous blood transfusions (P=0.046). In patients who received autologous transfusion, preoperative hemoglobin level and risk of allogenic transfusion were unrelated. When autologous transfusion was not given, allogenic transfusion risk was higher when hemoglobin concentration was less than 13 g x dL(-1) (P=0.0008). Autologous transfusion had a significant effect when hemoglobin level was less than 13 g x dL(-1) (P=0.002) but did not affect the rate of transfusion when hemoglobin was 13 g x dL(-1) or more. CONCLUSIONS: Autologous blood transfusion is effective for reducing the need for allogenic transfusion after knee replacement surgery, particulary when a patient's hemoglobin level is less than 13 g x dL(-1).

[Detection of Mycoplasma suis in clinical cases with postweaning multisystemic wasting syndrome in swine].

Mycoplasma suis is a swine erythrocyte obligatory parasite. Its presence may result in chronic or acute anaemia in different pig categories. It is considered that the postweaning multisystemic wasting syndrome (PMWS) is caused by porcine circovirus type 2, but some aspects of the pathogenesis remain unknown. PMWS signs are impaired weight gain, anaemia and jaundice in 5 to 12 week-old pigs that suffer from immunosuppression and bacterial co-infections. The pigs with signs of these diseases on three porcine farms were studied. Compatible M. suis forms in blood smears and typical PMWS lesions in tissue cuts were seen. This is the first communication of the clinical association between these two entities.

[Rupture of the teflon cover of valvular prosthesis in aortic position. Severe postoperative complication].

Fourteen patients (18.5%) are presented from a total of 78 patients who received a Starr-Edwards prosthesis Model No. 2320 with a Telfon covering, in an aortic position. In the late post-operative period these patients developed complications which merited a surgical reintervention. Twelve of these were operated on to substitute this prosthesis for a distinct type and model. In the other two the teflon covering on the cage of the prosthesis was dried thoroughly. The most frequent complication which made it necessary to reoperate was hemolysis, which would not cede to intense medical treatment. The hemolysis appeared at an average of 20 months post-operatively. In all of the cases we found a rupture due to erosion of the telfon material. This is undoubtably an etiological factor of the hemolysis. Eleven of the 14 cases (78.5%) survived. Ten of which were possible to follow up on a long term basis. Seven of these (70%) are asymptomatic and the remaining 3 (30%) are evolving with data suggestive of peri-prosthetic aortic insuficiency. A review is made of the evolution of Starr-Edwards prosthesis and the causes of the post-operative hemolysis in valvular surgery.

[Community participation in health at the Instituto Mexicano del Seguro Social in Jalisco].

This paper describes the achievements and problems of a community participation model in health care, developed by the Mexican Social Security Institute since 1985. The health team involved between 10 and 20 percent of the personnel due to the slow adoption this program requires in new of the cultural change it implies. The community's answer has been satisfactory, as it can be seen by the ware of 100 neighbours and school committees, and around the 2,000 housewives and school students who are been working as promoters. This figures stand monthly. The self-care groups on chronic diseases constitute another form of participation. The community activities are progressively focused on causes of health and disease, embracing our 70 per cent of health promotion and disease prevention actions. The work realized by the community over cause one third of that realized by the preventive medicine personnel. This health community participation model represents a valuable and viable alternative.

Mononucleosis-like illness as a manifestation of carbamazepine-induced anticonvulsant hypersensitivity syndrome.

We describe a case of infectious mononucleosis-like as a manifestation of carbamazepine-induced anticonvulsant hypersensitivity syndrome. Patch testing to carbamazepine and phenytoin were positive and negative to phenobarbital. Our results suggest a cell mediated type of immune response in the pathogenesis of this syndrome. Patch testing may be helpful in the diagnosis of anticonvulsant hypersensitivity syndrome caused by carbamazepine.

Plasma sulfobromophthalein disappearance in Gilbert's syndrome.

BACKGROUND/AIMS: We studied the metabolism of sulfobromophthalein and its relationship with serum bilirubin levels in 40 patients with Gilbert's syndrome (type I 30; type II 6; type III 4). MATERIAL AND METHODS: Plasma sulfobromophthalein disappearance studies were carried out and 72 hours later, serum bilirubin concentrations (total and unconjugated fraction) were determined at baseline and after 24 and 48 hours of dietary restriction to 400 calories/day. RESULTS: The fractional transfer rate of sulfobromophthalein from plasma to liver was significantly higher in types I (14.7 +/- 3.4 ml/min) and II (14.9 +/- 2.7 ml/min) than in type III (8.7 +/- 1.5 ml/min). The fraction of the plasma sulfobromophthalein pool irreversibly cleared per min was significantly higher in type I (12.2 +/- 2.6 ml/min) than in types II (9.5 +/- 1.5 ml/min) and III (9.3 +/- 3.8). In all patients, serum bilirubin concentrations were significantly higher after fasting as compared with baseline. There was a significant correlation between the increments of serum unconjugated bilirubin levels after the fasting test and the transfer rate of sulfobromophthalein from plasma to liver (F = 9.8411, r = -0.4535, p = 0.003). CONCLUSION: These findings indicate the presence of an active uptake system shared by bilirubin and sulfobromophthalein.

Debilitating cutaneous poxvirus infection in a Hodgson's grandala (Grandala coelicolor).

A case of cutaneous avian pox infection in a Hodgson's grandala (Grandala coelicolor) is described. The bird was emaciated and had nodules on the eyelids, bill, neck, legs, and toes. Eosinophilic intracytoplasmic inclusion bodies were visualized by light microscopy in epithelial cells of the cutaneous nodules. Electron microscopy revealed numerous pox virions in the inclusion bodies. This is the first report of cutaneous poxvirus infection in a Hodgson's grandala.

Intestinal cryptosporidiosis in pigeons (Columba livia).

An intestinal disease in pigeons (Columba livia) from the Canary Islands characterized by diarrhea and body weight loss is described. Intestinal cryptosporidiosis was identified in three young pigeons. Cryptosporidia were associated with hyperplasia of the intestinal crypts and moderate inflammatory infiltration in lamina propria. This is the first report of cryptosporidiosis in pigeons.

Stevens-Johnson syndrome from tetrazepam.

We present a case of a patient showing a cutaneous and systemic affectation compatible with the Stevens-Johnson syndrome due to the intake of tetrazepam as a muscular relaxant. The symptoms remitted after the suspension of the involved medicines and after treatment with systemic corticoids. Months later, a study with patch tests, with the standard battery and the medicines that she took (indapamide, tetrazepam, acetyl salicylic acid, botriozolam and levomepromazine) was carried out. She presented positivity to tetrazepam at 48 and 96 hours. The oral provocations with the other medicines, which she consumed, were negative. The study of cross reactivity with other benzodiazepines was negative. In the medical literature, different cutaneous affectations regarding benzodiazepine use (generalised drug eruptions, contact dermatitis, erythema multiforme, ...) have been described byt non Stevens-Johnson syndrome. The patch tests are useful in the diagnosis of this type of cutaneous reactions, avoiding in this way the achievement of oral provocations, with the potential risk for the patient that they imply. The absence of cross-reactions with other substances of the same group has to be pointed out, even with those with which keeps great structural similarity as the diazepam. In spite of the absence of cross reactions with the rest of benzodiazepines, faced with the seriousness of the case presented, it is advised to the patient to avoid this type of medicines.
