Subxiphoid echocardiographic assessment of left ventricular wall in old myocardial infarction.

We studied by cineangiography and standard and subxiphoid echocardiography the left ventricular wall motion in 50 patients with old myocardial infarction and 10 normal subjects. Coronary artery angiography showed a good correlation with the electrocardiographic site of infarction and with the asynergic areas detected by cineventriculographic examination and bidimensional echocardiography. From the correlation of data related to the asynergic areas detected by angiographic and echocardiographic methods it was possible to establish that subxiphoid approach allows to visualize the posterior-lateral wall whereas the parasternal approach the posterior-inferior wall of the left ventricle. Two-dimensional echocardiography from the subxiphoid approach resulted to be a useful and reliable technique for detecting the apical asynergies. The left ventricular wall motion parameters detected by M-mode echocardiography showed significant modifications in all the groups either in the comparison SUBX versus ST or in that with the control group. The subxiphoid approach either M-mode or bidimensional was, therefore, useful in the evaluation of the asynergic areas of the left ventricular lateral wall. The association of SUBX with ST provides further information on the left ventricular wall motion and then, since different areas are visualized with the two techniques, on the extent of myocardial damage.

[Study of the inotropic effect of potassium-canrenoate by systolic time intervals technique (author's transl)].

Systolic time intervals (TS) were determined before and after intravenous injection of 400 mg of Potassium-Canrenoate (CK) in 33 patients 30 days after acute myocardial infarction, in order to evaluate the inotropic effect of this drug. It was observed a reduction of the pre-ejection period (I. PEP) and of the pre-ejection period/left ventricle ejection time (PEP/LVET) ratio without any significant changes in the electromechanical systole (I. Q-2), left ventricle ejection time (I. LVET) and isometric contraction time (ICT). These results do not allow to attribute positive inotropic effect to CK. In fact drugs which share this activity, like digitalis glycosides, shorten also I. Q-S2, I. LVET and ICT, with a reduction of I. PEP and PEP/LVET ratio by far greater than that determined by CK. TS determination, in 10 patients, 15, 30 and 45 min after the injection of CK yields results confirming these conclusions.

[Intestinal occlusions due to enteroliths].

Enteroliths or intestinal autochthonous calculi are among the rare causes of intestinal occlusion. A personal case in which the central nucleus of the stercolith consisted of gunshot is reported. The literature is reviewed, particular attention being laid on pathogenetic aspects, and the frequent complications of this pathology are recalled. The casual radiological finding of an enterolith generally means surgery only for those with caeco-appendicular localization.
