The value of fat-suppressed T2 or STIR sequences in distinguishing lipoma from well-differentiated liposarcoma.

The objective of this study was to evaluate the diagnostic value of fat-suppressed T2-weighted (FS-T2) images or short tau inversion recovery (STIR) imaging in distinguishing lipoma from lipoma-like subtype of well-differentiated liposarcoma. Spin-echo T1-weighted and STIR or fat-suppression T2-weighted sequences were performed in 60 lipomas and 32 lipoma-like well-differentiated liposarcomas, histologically proven, looking for thick septa or nodules in T1-weighted images and linear, nodular, or amorphous hyperintensities on FS-T2/STIR sequences. Fourteen lipomas (23.3%) showed thick septa and/or nodules on T1, whereas on FS-T2 or STIR sequences only seven (11.7%) displayed hyperintense nodules and/or septa. All well-differentiated liposarcomas contained these signs on FS-T2 or STIR sequences. The presence of hyperintense septa or nodules in a predominantly lipomatous tumor on FS-T2/STIR sequences helps to differentiate malignant tumors from lipomas. Employing the presence of hyperintense nodules and/or septa as criteria of malignancy specificity was 76.6% and sensitivity 100%. Overdiagnoses of well-differentiated liposarcoma can occur due to the presence of non-lipomatous areas within lipomas.

Volatile oil variability in Thymus serpylloides ssp. gadorensis growing wild in Southeastern Spain.

Volatile oils from single plants of Thymus serpylloides ssp. gadorensis were collected from Southeastern Spain and studied to check for chemical variability using gas chromatography (GC) and gas chromatography-mass spectrometry (GC-MS). Many of the samples showed a phenolic chemotype, while another important group had significant levels of linalool. Geraniol, myrcene, caryophyllene oxide, terpinen-4-ol and 1,8-cineole were commonly present. Principal Component Analysis (PCA) and Cluster Analysis (CA) of this chemical variability separated two groups of plants characterized by either phenols or linalool, and an isolated third type with geraniol. A few samples were found to have both phenolic and non-phenolic compounds in high quantities, thus showing a mixed chemotype. Multidimensional scaling analysis (MDS) of the percentage concentration for each component of the essential oil showed that thymol, linalool, 1,8-cineole, borneol and geraniol have clear divergent vectors.

[Evaluation of pulmonary arteries using nuclear magnetic resonance in patients with pulmonary atresia].

OBJECTIVES: Patients with pulmonary atresia and a ventricular septal defect have one outlet vessel; the aorta. The pulmonary arteries may be present or absent, and there is a collateral shunt. Magnetic resonance imaging is used to study this. MATERIAL AND METHODS: We evaluated 10 patients with pulmonary atresia with ventricular septal defect. Five were males and the others were females, ages ranged between 1 day and 25 years, three patients were newborns. We used common planes, but mainly: axial-transverse and coronal, normal and right or left oblique projections. RESULTS: The size of the ventricular septal defect was 1.75 +/- 0.67 cm (0.5-3.2). The pulmonary trunk was absent in 7 patients, it was atretic in 2 and it was present in one. Confluent pulmonary branches were seen in 6 patients (right 0.58 +/- 0.12, left 0.53 +/- 0.19), not confluent in 2 (with only one branch in each case) and absent in the other two. There was collateral shunt flowing from the descending aorta in 7 patients, from the ascending aorta in two and from the aortic in the final case. The ascending aorta was dilated in all cases (diameter 3.04 +/- 1.39), afterwards the relation between the ascending aorta and the descending aorta was 1.87 +/- 0.25. The aortic arch was right sided in 5 patients. CONCLUSIONS: Magnetic resonance imaging is an alternative diagnostic method to angiocardiography. It gives complete information about the size, the position and the connection of the pulmonary arteries. It is also useful to evaluate the aortopulmonary to bronchial shunt.

Recombinant gamma-interferon as adjuvant to hepatitis B vaccine in hemodialysis patients.

Patients undergoing long-term hemodialysis are at high risk of acquiring hepatitis B yet tend to have poor rates of response to hepatitis B vaccine. The effect of recombinant human gamma-interferon (2 million units/m2) on the response to a recombinant hepatitis B vaccine was evaluated in a prospective, randomized controlled trial in 81 hemodialysis patients. A similar proportion of both groups of vaccinees ultimately developed antibody to HBsAg including 81% of the 41 recipients of vaccine alone (group I) and 89% of the 40 recipients of vaccine with gamma-interferon (group II). However, the antibody to HBsAg response occurred earlier in recipients of vaccine with gamma-interferon, so that at 4 mo 63% of group I and 88% of group II had antibody to HBsAg (p less than 0.025). Furthermore, titers of antibody to HBsAg tended to be higher in the vaccinees given interferon; the final geometric mean titers were 232 IU/L in group I and 330 IU/L in group II (p = not significant). Retrospective testing for antibody to hepatitis C virus revealed that 21 (26%) hemodialysis patients were seropositive at entry into this trial, but the presence of antibody to hepatitis C virus did not appear to affect the response rate to the hepatitis B vaccine. These results suggest that the effects of gamma-interferon as an adjuvant in increasing the response rate to hepatitis B vaccination deserve further evaluation perhaps most appropriately in persons who have not responded to an initial course of vaccine.

[Echography in the diagnosis on childhood nephrocalcinosis. Apropos of 5 cases].

We have studied 5 patients with nephrocalcinosis by means of plain film and sonography. Our results, which are in agreement with other authors, show that it is possible to find nephocalcinosis with no findings in the plain film. We correlate both examinations and propose a new radiologic approach for this condition.

Postmediastinoscopy changes in chest CT.

OBJECTIVE: To know the CT changes that may be seen in the mediastinum after uncomplicated mediastinoscopy. MATERIALS AND METHODS: Noncontrast CT was performed on 10 patients on the 1st, 3rd, and 5th days after the procedure. RESULTS: We encountered linear and diffuse increased attenuation within the mediastinum as well as air bubbles. By the 5th day postmediastinoscopic changes resolved. We never found air-fluid levels, fluid collections, or other abnormalities. CONCLUSION: Expected postmediastinoscopic changes are not to be confused with potential postprocedure complications such as hemorrhage or infection.

Preoperative CA-125, CA 19-9 and nuclear magnetic resonance in endometrial carcinoma: correlation with surgical stage.

Serum CA 125 and CA 19-9 determinations and nuclear magnetic resonance (NMR) of the pelvis were performed in 100 consecutive endometrial carcinoma patients, who were surgically staged afterwards. The aim was to assess the value of these combined studies to predict the surgical stage of endometrial carcinoma. The results were correlated with the final surgical stage. NMR or elevated tumor marker values alone were able to predict a surgical stage equal to or higher than IC in 84.6 and 78.8% of the cases, respectively. The combination of both procedures yielded better positive and negative predictive values (90.9 and 88%, respectively), but only when both were concordant (p < 0.0001). When only one of them suggested a higher stage (28% of the cases), the positive predictive value decreased to 70.4%. The accuracy in predicting a higher surgical stage in endometrial carcinoma by preoperative noninvasive means is slightly improved when combining NMR and serum tumor marker determinations. The predictive value, however, is not absolute and clearly unsatisfactory in 28% of the cases.
