Hospitals across the country are busy reconfiguring their delivery systems to improve patient care delivery. In the process, they are overhauling their organizational structures, reaching out to other institutions for input and participation, and embracing local communities. Many hospital executives are also finding that the path itself is at least as interesting and valuable as the end result.
Recently, each of the four largest foundations active in the health care field announced changes in the priorities of funding activities. All four philanthropies are moving away from funding projects involving tertiary care. Instead, they are targeting primary care as a major goal.

Nurse executives continue to witness an increase in their responsibilities and influence, finds a new survey conducted by Witt Associates Inc., Oak Brook, IL, and the Association of Nurse Executives (AONE) of the American Hospital Association, Chicago. The survey of the senior nurse position within U.S. hospitals states that "hospitals are hiring more nurses to cope with a sicker patient population and ... are assigning to senior nurse executives larger areas of responsibility, especially for outpatient services."
The Pew Health Professions Commission began its work to reform health professional education in a bold way last fall, issuing its recommendations up front in a push for immediate changes in the way health professionals are educated. And what needs changing? "I think that we in health care have to ask whether we've become seduced by specialization," says Edward H. O'Neil, the commission's executive director and assistant dean for medical education at Duke University.

The importance of outpatient services to hospitals' total net patients revenues continues to increase steadily. In fact, nearly one-third of those participating in our fourth annual survey on diversification gauged outpatient services as providing 30 percent or more of their total net patient revenues in 1990.
The Glucocorticoid Cascade Hypothesis of Aging and the hypothesis that food restriction retards the aging processes by preventing the development with age of hyperadrenocorticism were investigated. A longitudinal life span study of the daily concentration pattern of plasma corticosterone was conducted in male Fischer 344 rats fed ad libitum or restricted to 60% of the mean food intake of ad libitum fed rats. In another group of ad libitum fed and food-restricted rats, the influence of age on the response of plasma corticosterone levels to restraint stress was measured as was the time course of the return of plasma corticosterone to basal levels following the stress. The findings do not support the hypothesis that food restriction retards the aging processes by preventing the development of hyperadrenocorticism with advancing age. They also indicate that the Glucocorticoid Cascade Hypothesis does not describe a major aspect of the aging processes. Rather, the results suggest the possibility that a lifetime of daily periods of mild hyperadrenocorticism may, if anything, retard the aging processes.

The Canadian health care system continues to appeal to Americans looking for alternatives to our own system. But, according to administrators who have worked in both, there are pluses and minuses to consider in each system.
