[Nephrogenic adenoma of the urethra. Report of a case].

Nephrogenic adenoma is a benign urothelial neoformation rare in children. It can be present in the entire urinary tract, being more common in the bladder and very infrequent in urethra. These metaplasic changes seem to be due to irritative stimulus to the epithelium, including lithiasis, previous surgery, permanent or repeated catheterization, infective diseases and chemotherapy and or radiotherapy. The diagnosis is established after biopsy and in most of the cases the treatment of choice consists of transurethral resection of the lesion. We report the case of a boy 11 years old who was diagnosed of Nephrogenic adenoma of prostatic urethra after cistoscopy for repetitive urethremorrhagia. The patient was managed conservatively and oral antibiotic therapy was administered being the patient asymptomatic and free of recurrence during the follow up.

[Gastric duplication associated with heterotopic pancreas].

Gastric duplications are the least frequent location for the alimentary tract duplications, these location is approximately 8%. We present a case of gastric duplication in a neonate, who had a palpable abdominal mass and feeding intolerance. Barium study of upper gastrointestinal tract and ultrasound showed an extrinsic compression of antrum and cyst abdominal mass. The surgical procedure consisted of total excision without violation of the gastric lumen. Gastric mucosal was found by hystological study. Heterotopic pancreatic tissue was also found on the major omentum closely gastric cyst duplication.

[Wilms' tumor in Cantabria. Review of our cases (1974-1990)].

We reviewed the records of patients with genitourinary tumors that had been diagnosed and treated at the Section of Pediatric Surgery of Marqués de Valdecilla Hospital from 1974 to 1990. There were 14 such tumors. Of these, 12 (85.7%) were Wilm's tumor. This tumor type is the subject of the present study. Regarding its clinical features, 33.3% of the cases consulted for hematuria and an abdominal mass was the most common finding in the course of the disease (83%). We underscore the usefulness of CT and ultrasound in making the diagnosis. In the cases where these noninvasive imaging techniques were used, their efficacy rate was 100%. Together with IVP, these constitute the fundamental diagnostic tools. With regard to treatment, radical nephrectomy was performed in all cases and combined with radio and chemotherapy according to protocol. Pathological examination confirmed the diagnosis of Wilms' tumor in all the cases. Following the NWTS classification, there were 2 stage I,5 stage II, 2 stage III and 3 stage IV. Apart from tumor stage, the histological features significantly influenced the diagnosis. The survival rates for those with favourable or unfavourable histologic features were 70% and 36%, respectively.

[Idiopathic prepubertal gynecomastia].

Prepuberal gynecomastia is extremely rare. We present a case of unilateral and idiopathic gynecomastia in a six years old male. Endocrinology test and anamnesis did not find pathology or drug ingestion. A mastectomy was performed, because the big size and progressive evolution of breast produced physicology problems. We review the literature and remark infrequent aspects of our case, also the complete clinic and pathologic study.
