[Clinical evaluation of carbenicillin in a daily dose of 10 g in chronic airway infections].

Carbenicillin (CBPC) was evaluated in 4 patients with chronic airway infections and 2 with infections secondary to lung cancer. The daily dose of CBPC was 10 g for all patients. The following results were obtained. Three out of 4 patients with chronic airway infections showed good response. Both cases with infection secondary to lung cancer showed good response. The overall effectiveness rate was 83.3%. There were no side effects or abnormal laboratory values related to carbenicillin therapy. CBPC is still considered to be a drug of choice for the initial acute exacerbation of chronic airway infection and for infections secondary to lung cancer.

[Effect of suloctidil on the dynamics of the peripheral artery of the dog].

The effects of suloctidil, an antispasmodic agent, on the femoral, renal, superior mesenteric, common carotid and vertebral blood flows were compared with those of papaverine and cinnarizine using electromagnetic flow meters in pentobarbital anesthetized dogs. In i.v. administration, the peripheral vasodilating action of suloctidil, papaverine and cinnarizine dose-dependently occurred from the dose of 0.1 mg/kg, and the order of their potencies in blood flow increase was vertebral greater than or equal to femoral greater than or equal to common carotid much greater than mesenteric artery. However, the renal blood flow decreased by i.v. administration of suloctidil and papaverine. I.a. administration (1-100 micrograms/kg) of suloctidil, papaverine and cinnarizine dose-dependently increased femoral and common carotid blood flows. I.a. administered suloctidil and papaverine caused a transient increase of renal blood flow followed by a slight decrease. Suloctidil-induced increase of femoral blood flow was not affected by the pre-treatment with atropine, propranolol or diphenhydramine. Thus, suloctidil may cause direct vasodilation to increase regional blood flow, and its influence on the various blood flows is roughly similar to those of papaverine and cinnarizine.

[Carcinoma of the esophagus with hepatic metastasis].

Six patients with esophageal cancer with liver metastasis are reported and the clinicopathological features are discussed based on collected autopsy cases. 1) The incidence of liver metastasis was higher when the primary site was in the lower part of the esophagus. 2) There were many cases without abdominal lymphnode metastasis. 3) Metastasis was noted primarily as small nodules in the left lobe of the liver. 4) The prognosis was poor; liver metastasis was rarely the cause of death. In esophageal cancer, liver metastasis seems to occur via the portal route and combined liver resection is indicated when possible.

[Clinical studies of cefoxitin in the field of internal medicine].

Ten patients with sepsis and pneumonia complicated by leukemia or lung cancer were treated with cefoxitin (CFX) at daily dose of 6 g. The following results were obtained. 1. Clinical effects of CFX were good in 5 patients, fair in 2 and poor in 3 with effective rate of 50%. 2. Out of 8 patients with sepsis, 5 showed good response to CFX and effective rate was 62.5%. 3. Bacteriological outcomes were eradicated in 1, unchanged in 1, replaced in 2 and unknown in 6 cases. 4. Diarrhea was observed in 1 patient but this was not considered related to CFX therapy. 5. No abnormal laboratory finding due to CFX was observed. 6. It should be considered that 6 g or more of CFX is given in case of severe infections, such as sepsis or pneumonia complicated by serious underlying diseases.

[Phase II study on peplomycin for esophageal cancer].

For the purpose of verifying the effectiveness of peplomycin, one of the derivatives of bleomycin, against carcinoma of the esophagus and the safety of it, the analysis of the data for total 113 cases collected from 25 institutions in Japan was made. The results are as follows. It was effective in 19 out of 74 evaluable cases of carcinoma of the esophagus (25.7%). In case of treatment with peplomycin alone, it was effective in 6 out of 39 cases (15.4%). In case of the combination treatment with peplomycin and some other therapy, it was effective in 13 out of 35 cases (37.1%). As for the side effects, the incidence of fever was the highest in both the cases of peplomycin alone and the combination treatment such as 39.6% and 37.0%, respectively. Anorexia, nausea, vomiting, respiratory symptoms and tiredness were found in relatively many cases. In the clinical laboratory tests, the vital capacity after the treatment tended to be lower than that before the treatment, but there was little change in the hematological tests, pulmonary function test and renal function test.
