Liver cirrhosis and liver cancer. A study of their relationship in 2563 autopsies.

A sample of 2,563 cases of liver cirrhosis diagnosed among 26,879 autopsies performed consecutively in the years 1976/85 was considered in order to study the morphologic feature of the liver cirrhosis and their relationship with primary liver cancer (PLC) outbreak. The study showed that age of patients affected by cirrhosis is a significant risk factor for PLC. In fact, in the 10 years considered, the observed mean annual increase of age at death of the cirrhotic patients was associated with an increase in PLC cases. This may be explained as a well known consequence of the conversion of micronodular type of cirrhosis to mixed and macronodular ones (for which cancer complication is more frequent) but also as a statistically significant higher risk of PLC in the older group of patients affected by micronodular cirrhosis. The older age of cirrhotic patients at death observed in the last few years in the Trieste area is probably correlated to a general European trend: our results, although relative to a restricted geographic area, seem to be of interest in order to identify better the patients at risk of PLC among those affected by cirrhosis.

[Oncocytic carcinoma in a horseshoe kidney].

The Authors report a rare case of oncocytoma in a horseshoe kidney. The diagnostic path needed to assess the precise anatomy of the malformation and tumoral staging is out lined. Oncological problems dealing with oncocytoma are further discussed, together with the technical and surgical issues pertinent to the clinical case.

Acute idiopathic scrotal oedema: rare disorder or difficult diagnosis?

This study reports our experience in 6 cases of acute idiopathic scrotal oedema. Although children were the primary targets, this pathologic condition was also encountered in adults. Specific diagnosis of acute idiopathic scrotal oedema, opposed to other causes of scrotal swelling, is based on history, an objective examination, velocimetric Doppler exam and echography. Correct diagnosis is important in order to avoid unnecessary surgery. We are inclined to consider acute idiopathic scrotal oedema as an allergic disorder and recommend a follow-up within two days.

Bladder reconstruction after cystectomy: use of ileocecal segment and three-loop ileal reservoir.

We report on 11 patients who in 1980-1982 had bladder reconstruction, after cystectomy for bladder cancer, utilizing the ileocecal valve as an antireflux mechanism and a direct urethrocecal anastomosis. They received preoperative irradiation (2,000 rad) and had some early complications such as pelvic abscesses and temporary urinary fistulas. We have since omitted preoperative radiation on cases performed in 1984-1987 and they did not have these complications.

[Percutaneous drainage of abdominal abscesses].

Percutaneous puncture with echographic guide was performed in 37 patients with suspected abdominal abscesses. The abscess has been confirmed in 31 cases and it has been related to previous surgery in 21. In 3 cases the 3 small cavities were evacuated by a single-stage aspiration; in the other cases different drainage catheters, in relation to the size and the content of the cavities have been employed. Benefits of proteolytic agents, irrigation with saline solution and permanent aspiration are stressed. Successful drainage avoiding surgery was achieved in 23/31 patients (74%), and positive results have been obtained in multiple abscesses, multilocular abscesses and infected haematomas as well. A partial success was achieved in 3 cases (10%): the patients were submitted to surgery after a clinical improvement. Two complications occurred (bowel perforation and small liver laceration). The percutaneous drainage turned out to be a technique of choice in the great majority of abdominal abscesses, and should be considered a good alternative to surgery, particularly in high risk patients.

[Long-tem results of surgical treatment of biliopancreatitis (author's transl)].

Conservative treatment of Recurring Acute Pancreatitis (R.A.P.) is characterised by a fairly high disease and death rate due to the numerous relapses involving pancreatic lesions of ever increasing severity. 42 patients with R.A.P., subjected to biliary tract surgery at Trieste University Institute of General Surgical Clinic, were re-examined (average follow-up of three years) in order to assess the effectiveness of the treatment given. The follow-up results confirm the need for prompt surgical operation aimed at correcting the biliary pathology and at obtaining complete "restitutio ad integrum" from the pancreatic anatomo-functional standpoint when such therapy has been correctly applied.

[Anatomo-surgical assumptions in typical hepatic resections].

The synthetic resin moulds of the human normal livers permit the schematization of the distribution and the disposition of the vascular elements of the hepatic tree. The authors have described all the segments and sectors of the ilus in their modal disposition that are important from the point of view of the operation. Beside that, they have pointed out the presence of anastomosis, at the level of the ilaris plate and the existence of anastomotical ramifications between the right and left sectors, and the close dependence of each sector from his own vascular ilus. The authors emphasize the importance of this observation from the surgical point of view.

Anatomo-functional considerations on venogenic impotence.

The authors report their clinical experience with cavernosography and cavernosometry on impotent patients. The indications of method and its correct place in the diagnostic workup are underscored. The methods of evaluation are described, highlighting the advantages of a diagnostic pharmacological approach in respect to a near as possible physiological haemodynamic evaluation. Finally they propose a future development for the method, such as the use of PGE1 instead of the commonly employed drugs, and a new classification for the veno-occlusive disease with stage and grade.
