A case of polycythaemia vera presenting with intramuscular chest wall haematoma.

A 40 year old male, previously well, presented with a posterior chest wall haematoma. Computerized tomography and ultrasound showed this to be intramuscular. Haematological indices were consistent with a diagnosis of polycythaemia vera. The haematoma and indices responded to hydroxyurea and venesection. This rare presentation of polycythaemia vera in a young person is described and the haemorrhagic complications of polycythaemia vera discussed.

Evaluation of keratoplasty in cases of post-inflammatory corneal opacity with vascularization.

Penetrating keratoplasty was performed in 106 cases of post-inflammatory corneal opacity with vascularization. Donor material obtained post mortem was preserved in a moist chamber, MK-medium at 4 degrees C or in culture medium at room temperature. 55.7% of patients gained a useful vision after keratoplasty. The post-operative spherical error of refraction was found to be -1.4 +/- 3.6 D and the astigmatic error of refraction -3.3 +/- 2.0 D. The post-operative visual acuity was found to be 0.2 +/- 0.2. The ocular pathology associated with keratoplasty is discussed.

[Incidence of lithiasic recurrence after a diuretic therapy, alone or combined with treatment by a thiazide diuretic or phosphorus].

The preventive affects on recurrent renal calcium stones of water diuresis alone or combined with drugs aimed at lowering urinary calcium were evaluated prospectively in 51 patients with calcium nephrolithiasis. Following clinical and metabolic examination, the patients were allocated at random to 3 treatment groups: water diuresis alone (group I, n = 19) or associated with hydrochlorothiazide 50 mg/day (group II, n = 19) or with a neutral phosphate preparation 1500 mg/day (group III, n = 13). Results were assessed on the number of recurrences; 24-h urinary calcium was measured at regular intervals. The mean follow-up (2 years; range 1-4 years) was the same in all 3 groups. A significant fall in recurrence rate as compared with pre-treatment values was observed in groups I and II. The recurrence rate was the same in both groups during treatment. However, less patients had recurrences in group I (1/19) than in group II (5/19). No significant fall in recurrence rate was observed in group III, owing to some patients in this group having frequent recurrences. The recurrence rate was unrelated to clinical findings and biochemical values ( oxaluria , calciuria) measured before treatment and to the urinary Ca/Cr ratio calculated during treatment. This study confirms that water diuresis is effective in preventing recurrent renal calcium stones and that diuretics of the thiazide group reduce the number of patients with recurrences.

Pulpal response to an epimine plastic.

Pulpal reactions to three dental materials, Scutan, a self-curing resin, and zinc oxide-eugenol cement, were comparable. The pulpal reactions to Scutan appeared to be milder than those to a self-curing resin or to zinc oxide-eugenol cement.

Obstruction of the central retinal artery. Comparison of the ophthalmodynamometry measurements in relation to the time.

The clinical picture of occlusion of the central retinal artery was studied in 37 cases. The fundus findings were described in the most interesting cases. The cerebrovascular resistance and cerebral blood flow were calculated on the nonaffected side. The ophthalmic artery blood pressure was found to be elevated in 5 cases due to obstruction of the internal carotid artery distal to the origin of the ophthalmic artery. The ophthalmic blood pressure was decreased in the rest of the cases within the first 3 days after the onset of the clinical manifestations of the disease. After 3 weeks, a recanalization of the central retinal artery had occurred in most cases. Almost all cases show no side difference after 2 months. A minimal side difference was found in a very few cases only. In 54% of the patients, ophthalmodynamometry of the nonaffected side revealed, an elevation of the cerebrovascular resistance. The cerebral blood flow is compensated in 67% due to the elevation of the internal carotid blood pressure. The findings indicate that patients with elevated vascular resistance are susceptible to obstruction of the central retinal artery.

Clinical trial of topical disodium chromoglycate in vernal keratoconjunctivitis.

The clinical picture of Spring catarrh was studied in 96 cases. The duration of the disease was found to be 3.5 +/- 2.6 years. It exists all the year round with a higher frequency between April and August. Male children were affected about three times more frequently than females. The effect of DSCG was studied in 20 cases. It had a significant therapeutic effect. We advise a combined DSCG and corticosteroid treatment in the period of exacerbation.
