[Comparison of 3 high-permeability dialysers].

Three capillary dialyzers, with highly permeable membranes, have been compared for their capacity of epuration of blood urea nitrogen, creatinine, phosphorus, uric acid and beta 2 microglobulin, and for their respective protein losses. There were very little differences between the dialyzers for epuration of small uremic molecules, with no benefit due to high permeability membranes in comparison with conventional dialyzers. Protein losses may be important and have to be known. About beta 2 microglobulin, filtered quantities are not correlated to changes in serum levels, suggesting the presence of other mechanisms (adsorption on the membrane and generation during hemodialysis) which influence, together with epuration, perdialytic changes of serum beta 2 microglobulin concentrations.

[Colonic necrosis, an unusual extrarenal involvement in hemolytic and uremic syndrome].

A seventeen years old girl presented a HUS. The usual gastrointestinal symptoms of the prodromal phase involved secondary complications leading to emergency surgical treatment. Laparotomy exploration revealed segmental necrosis of the colon which was treated by colectomy: histological studies revealed its ischemic nature with specific lesions of thrombotic microangiopathy. Later course was uneventful with the exception of renal failure requiring maintenance hemodialysis. Review of literature confirms the rare and severe nature of gastrointestinal lesions requiring surgery. Great care should be taken on one hand, not to undertake a useless operation and on the other hand to operate in time colonic necrosis involving or not intestinal perforation or intussusception.

[Extrarenal purification in severe salicylate poisoning in adults].

About a new case of severe salicylate intoxication, the authors wanted to confirm the advantages of extrarenal epuration in some circumstances. Besides of usual treatments, it decreases the severity of this intoxication and the delay in its treatment by reducing quickly blood salicylate level to a moderate or benign rate whose prognosis is really better. Of course, the choice among the most common treatments (hemodialysis, hemoperfusion, peritoneal dialysis) depends with technical disposals, but hemodialysis seems the most efficient with minor risks than the intoxication itself.

[Hemolytic-uremic syndrome during oral contraception].

A hemolytic and uremic syndrome is described in a 41 year old woman on oral contraceptives for 18 years, only discontinued during the course of 4 normal pregnancies. A renal biopsy performed on the 20th day, after correction of the thrombopenia, confirms the typical aspect of advanced thrombotic microangiopathy. The usual etiological work-up is negative, but the search for steroidal anti-hormones antibodies by radioimmuno assay is positive. The evolution is favorable with hemodialysis, perfusion of fresh plasma during the sessions, calciparin, dipyridamol and anti-hypertensive drugs with, eight months later, a creatinin clearance of 45 ml/min and important persisting ischemic lesions in a new renal biopsy. The hemolytic and uremic syndrome following oral contraceptives remains rare (about thirty cases reported in the literature) and its prognosis remains severe despite a well adjusted treatment. Prevention includes the detection of risk patients and the search for steroidal anti-hormones antibodies could certainly play a role which remains to be evaluated.

[The kidney failure of Bourneville's tuberous sclerosis: a new form of glomerular damage as a result of hyperfiltration?].

Chronic renal failure, rarely seen in tuberous sclerosis (TS), is usually related to renal hamartomas and/or cysts, but has only been very rarely linked to glomerular changes. In a patient with TS, we have performed repeated renal biopsies at early and advanced stages of renal failure. We have correlated the renal function deterioration with the evolution of focal and segmental glomerulosclerosis. Renal insufficiency would be in relation not only with renal involvement by multiple tumors, but also with the progression of focal and segmental glomerulosclerosis. This would be caused by hemodynamic changes in remnant glomeruli, after reduction in renal mass induced by renal tumors, and would be a new example of renal injury due to hyperfiltration.

[Extramembranous glomerulonephritis of acquired syphilis in a patient recently infected by the hepatitis B virus. Demonstration of the treponemal antigen in the kidney by indirect immunofluorescence].

A 38 years old male homosexual with active secondary syphilis presented with pure nephrotic syndrome while HBs and HBe tests were positive without clinical hepatitis. He had circulating immune complexes, IgG--IgM cryoglobulinemia and high IgA, IgM and IgE levels; the C3 and C4 complement constituents were normal. Examination of renal biopsy sections under light, fluorescent and electronic microscopy showed stage I membranous glomerulonephritis the syphilitic origin of which was confirmed by indirect immunofluorescence and by rapid cure under penicillin treatment. This case calls for the following comments: (1) glomerular deposits are extramembranous rather than subendothelial in syphilitic nephrosis, a disease now classified among circulating immune complexes diseases; (2) in the kidney, the treponema antigen can be demonstrated by indirect immunofluorescence and the anti-treponema antibody, by elution; (3) the outcome of the nephrotic syndrome is always favourable, either spontaneously or after penicillin treatment; (4) syphilis and HBs antigens are frequently associated, particularly in homosexual patients; one should be looked for when the other is discovered.

Acquired cystic kidney disease in renal insufficiency: a multicentre study. Group of Nephrologists of the East of France.

OBJECTIVES: A cross-sectional study was undertaken in order to a) investigate the prevalence of acquired cystic kidney disease and the prevalence of associated cancer; b) evaluate the best diagnostic procedure; c) look for a possible relationship with improvement of anaemia. METHODS: All patients treated in 10 Dialysis Centres in the East of France, except those with polycystic kidney disease, were examined during the same 6-month period by echography and computed tomography scanning, when they were at initiation of dialysis (T0) or when they reached exactly 5 years (T5), 10 years (T10), 15 years or more (T15) of dialysis. RESULTS: One hundred sixty-one patients were evaluated: 50 at T0, 48 at T5, 33 at T10 and 30 at T15. CONCLUSION: a) Prevalence of acquired cystic kidney disease was 57.2%, ranging from 30% at T0 to 83.3% at T15; prevalence of associated cancer was 1.1%; b) computed tomography was the best diagnostic procedure, but when compared with it, echography had a sensibility of 0.88 and a specificity of 1; c) only in patients with large-sized cystic kidneys, there was an improvement of anaemia.
