Modulation of in vitro cell growth of human and murine urothelial tumor cell lines under the influence of interleukin-3, granulocyte-, macrophage- and granulocyte-colony-stimulating factor.

To elucidate possible growth-modulating effects of interleukin 3 (IL-3), granulocyte-macrophage-colony-stimulating factor (GM-CSF) and granulocyte-colony-stimulating factor (G-CSF), human transitional cell carcinoma (TCC) cell lines T24, RT112, EJ and 647 V were solitarily and continuously exposed to these hematopoietic growth factors at concentrations of 1-100 ng/ml. The murine line MBT-2 was used as a negative and the colon carcinoma cell line HTB38 as a positive control, because of species specificity and known proliferation in response to growth factors, respectively. In the T24 TCC-line solitary and continuous exposure to IL-3, GM-CSF and G-CSF at the highest concentration of 100 ng/ml led to a significant proliferation of cell growth in vitro. Significant proliferation in the RT112 line was only achieved with continuous exposure to IL-3 and GM-CSF (100 ng/ml); G-CSF failed to induce growth modulation in the RT112 line. No significant proliferative effect of any of cytokines administered was observed in the 647V line. Exposure of the EJ line to cytokines at the highest activity levels had a proliferative effect only in suboptimal growth conditions.

[Habitual dislocation of the elbow in children].

After a short review on literature the late result of an operated elbow with a follow-up time of 23 years is reported. The actual problems of this rare condition are discussed. Congenital hyperlaxity, a shallow semilunar notch and traumatic sequelae after previous dislocations reduce joint stability. Arthroscopy, X-ray tomography and MRI update accurate preoperative diagnosis. Early operation is recommended to reduce further damage in the joint. Nowadays well proved operative procedures provide good postoperative results.

[Risk of fracture of regenerated bone and metal implant following bone lengthening osteotomy].

Many problems may arise, jeopardize and spoil the outcome of lengthening osteotomies. Fracture of the plate and the newly formed bone bridging the gap between the fragments is only one of the possible complications. Overload is its cause in all the cases. Following the theory of Pauwels the paper analyses the high stresses in hollow bone and the relationship to their principles of construction. Accurate knowledge of bone stressing and correct assessment of bone strength in the X-rays may help to reduce the incidence of fatigue fractures after limb lengthening.

Bone Lengthening

Randomized comparison of flecainide and cibenzoline in the conversion of atrial fibrillation.

The efficacy of oral cibenzoline (260 mg/day and 320 mg/day) and flecainide (200 mg/day and 300 mg/day) in the conversion of chronic atrial fibrillation to sinus rhythm were compared in 31 patients in a randomized order. If sinus rhythm was not restored on the 5th day of oral treatment with either cibenzoline or flecainide (phase of initial treatment) patients were switched to a second phase of treatment with the drug not given in the first phase after a washout phase of 3 days. Sinus rhythm was restored in 7/28 treatment trials with cibenzoline and in 7/23 treatment trials with flecainide (not significant). Trough levels of cibenzoline and flecainide in the plasma were not significantly different between patients in whom sinus rhythm was restored and patients with persisting atrial fibrillation. In patients successfully converted to sinus rhythm, long-term treatment was instituted with flecainide (n = 6) or cibenzoline (n = 6). Atrial fibrillation developed in 2 patients in each group of patients within 3 months. In all other patients, sinus rhythm was maintained during the follow-up period of 12 months. Non-cardiac side effects were observed in 2 patients during treatment with cibenzoline and flecainide respectively. With flecainide, one patient developed sinus arrest up to 5.6 seconds.

Effects of intravenous cibenzoline on ventricular vulnerability and electrophysiology in patients with sustained ventricular tachycardia in comparison to a control group.

The electrophysiologic effects of intravenous (i.v. 1.5 mg/kg) cibenzoline were investigated in 13 patients with organic heart disease and supraventricular arrhythmias (control group) and 13 patients with drug refractory sustained ventricular tachycardia (VT group). In both groups, cibenzoline increased the effective refractory period (ERP) of the right ventricle (p less than or equal to 0.05), QRS duration (p less than or equal to 0.01), and QTc duration (p less than or equal to 0.01). Cycle length of the VT was increased from 311 +/- 73 to 393 +/- 119 ms (p less than or equal to 0.05) as well. The increase in QTc duration was more pronounced (p less than or equal to 0.05) in the VT group as compared with the control group. In four patients in the VT group with the largest increase in QRS-, QTc duration and cycle length of the VT proarrhythmia was noted after i.v. cibenzoline. Cibenzoline plasma levels were not significantly different between the two study groups. In the VT group, induction of VT was prevented in 2 patients, more difficult in 2 patients, easier in 1 patient, and unchanged in 5 patients. Spontaneous occurrence of the VT or induction by atrial stimulation was noted in 4 patients. In the control group, ventricular vulnerability remained unchanged. Cibenzoline may be effective in selected patients with drug refractory VT. In such patients, the increase in QTc duration was more pronounced as compared with a control group.

[Ophthalmologic symptoms of Down's syndrome (author's transl)].

After a short description of the general symptoms of Down's syndrome, the ophthalmologic aspects of the disease are discussed. The study is based on the examination of 1047 mongolian children. The various eye-symptoms were present in different frequencies according to sex and type. The results were compared with those in the literature of the last 100 years and identical as well as highly significant divergent values were found.

Blindness
