[The tissue distribution of racemic S-(+)- and R-(R)-(-)-MPPB (1-methyl-5-phenyl-5-propylbarbituric acid) in the rat].

Distribution of racemic 1-methyl-5-phenyl-5-propyl-barbituric acid (= racem. MPPB), of the S-(+)- and R-(-)-enantiomer (= (+)- and (-)-MPPB) between serum and several tissues was investigated after i.v. administration of the substances to rats. Anesthesia produced by the racem. MPPB--(+)-MPPB causes convulsions whereas (-)-MPPB is anesthetically active--cannot be explained by a stereoselective difference of the MPPB-concentration measured in serum and brain when giving the enantiomers. Reversely as expected in the brain MPPB-concentration is significantly higher after administration of (+)-MPPB than after that of (-)-MPPB during the appearance of the CNS symptoms.--Significant stereoselectively different MPPB-concentrations, which are time dependent, could also be detected in liver, spleen and fatty tissue; however, in contrast to the brain in these tissues MPPB-concentration is higher after administration of (-)-MPPB than after that of (+)-MPPB. Concentration differences which are measured in liver, spleen and fatty tissue already 1--10 min after i.v. administration of the enantiomers cannot be explained by a stereoselectivity of the metabolism and/or renal elimination. However, a stereoselective metabolism and/or renal elimination of the enantiomers can be responsible for concentration differences which are observed during the late period of 60--180 min after administration: in serum, brain, liver and fatty tissue MPPB-concentration is lower if (4)-MPPB is administered. For that reason it can be supposed that (+)-MPPB is faster eliminated than (-)-MPPB.

[Short-term tocolysis of prostaglandin induced uterine contractions comparison between fenoterol and diazoxide (author's transl)].

Uterine contractions were stimulated with a mean dose of 1500 microgram PG in seven pregnant patients between the 14th and 22nd week of pregnancy. The same patients received 300 mg Diazoxide and 20 or 30 microgram Fenoterol alternately in order to tocolize these uterine contractions. After injection of 20 microgram Fenoterol the mean contraction interval lasted for 3.4 min, whereas after 300 mg Diazoxide a mean interval of 48 min was achieved. Side effects after Fenoterol were more marked.

[The inhibition of prostaglandin induced uterine contractions by diazoxide in vitro (author's transl)].

The tocolytic efficiency of Diazoxide, a benzothiadiazine derivative with pronounced musculotropic action, was tested on the isolated uteri of non-gravid and gravid rats. Regular uterine contractions were induced by the prostaglandines E2 and F2 alpha. Their amplitude and frequency could be suppressed totally or subtotally. Contraction intervals lasted 4--22 min. The basal tone was reduced in most cases.

[The inhibition of spontaneous and oxytocin-stimulated uterine contractions in isolated rat uteri with diazoxide (author's transl)].

Diazoxide, a benzothiadizin derivative without diuretic effect, was tested for its tocolytic effectiveness in isolated uteri from pregnant and nonpregnant rats. Amplitude, frequency, basal tonus and duration of the total inhibition of labor pains were interpreted as parameters. Total or subtotal inhibition of amplitude and frequency could be obtained in all experiments. The duration of total inhibition of labor pains was between 3 and 27 minutes. Affect on the basal tonus was not consistant.

[A "hairy polyp" of the throat causes larynx occlusion in a newborn (author's transl)].

A case of "polyp with hair" in the throat is reported. It's occurence is extremely seldom. It is the simplest form of an epignathus. The birth defect is considered as an uncomplete dual formation, it is more often found in female than in male newborns. The left palate is the preferred place of occurence. Because it is anchored with a pedicle, the tumor may change it's position resulting in an occlusion of the larynx, as it was in this case. Recurrence after surgery is not known.

[Posture-dependent changes in lung function and cardiovascular parameters in pregnant females at delivery date].

The objective of the investigation was to clarify whether, from the pulmonary or cardiological viewpoint, preference should be given to delivery in the sitting or lying position. For this purpose a spirometry was carried out in 28 pregnant women, average age 26 (19 to 35) and 15 control persons, average age 30 (20 to 41). In addition, the cardiac output of 12 pregnant women at the final date of confinement as well as of 10 persons of corresponding age for comparison purposes was determined by means of the CO2 rebreathing method. In none of the women investigated had labour set in. There were no significant differences of the VC, the IRV, the ERV and the FEV1 in the two groups examined, either in the lying or in the sitting position. Both in the sitting and in the lying position the arterial oxygen partial pressure of the pregnant women at rest was significantly above that of the control group. After stress both groups showed an increase of PaO2 and no longer differed significantly from one another. A difference in the PaCO2 depending on the position was not ascertainable in either group. However, the pregnant women had a significantly lower PaCO2 than the control group. The cardiac output of pregnant women did not differ significantly in the sitting position from those of the control group (4.7 +/- 1.3 and 5.3 +/- 1.5 1/min. respectively). Whereas the cardiac output of the comparison group rose in the lying position to 5.2 +/- 0.9 1/min, in the case of the pregnant women it dropped significantly (p less than 0.01) to 3.6 +/- 1.2 1/min. These findings show that a decision as to whether the delivery should take place in the sitting or lying position cannot be taken on the basis of parameters that analyse the lung function. The significant reduction of the cardiac output of those women lying on their back during labour, caused by the compression of the vena cava inferior and of the aorta by the uterus, indicates that the second stage of labor is better carried out in a position other than lying, e.g. the sitting position.
