Urinary hydroxylysine and hydroxylysyl glycosides excretion in patients with Turner's syndrome.

Urinary excretion of 5-hydroxylysine (Hyl) and its two glycosides, a monoglycoside (Gal-Hyl) and a diglycoside (Glc-Gal-Hyl), and the ratio of the diglycoside to monoglycoside have been studied in 30 patients with Turner's syndrome and in 38 healthy controls. In patients, the urinary excretion of Hyl and its diglycoside was similar to that obtained in the controls, while the excretion of the monoglycoside was significantly lower before the age of 17 years. As a consequence, between 6 and 17 years of age the Glc-Gal-Hyl/Gal-Hyl ratio is significantly higher in patients with Turner's syndrome than in normal subjects. The results of our study seem to indicate a disturbance in the turnover of collagen in Turner's syndrome.

Bioassay of growth-stimulating activity of serum: comparison of lymphocyte and cartilage assays in normal and growth hormone-deficient children.

The somatomedin and/or growth-stimulating activity of serum from hypopituitary children and short children with normal growth hormone (GH) response to stimulation tests were studied using different bioassays: thymidine incorporation into human activated lymphocytes; sulfate incorporation into chick embryo cartilage; and simultaneous thymidine uptake into the same cartilages. The results showed that lymphocyte assay is highly sensitive to small amounts of serum and is GH-dependent in children with low GH secretion. On the contrary, the cartilage assays need higher serum concentration and their GH-dependence appears only in subjects with normal or low-normal GH secretion. The lack of correlation between the results of the three bioassays suggests that they measure both somatomedins and different serum factors involved in the regulation of growth.

Benzydamine for the topical treatment of vulvovaginitis in children and adolescents.

A study was undertaken in children and adolescents aged 4-18 years on the value of benzydamine vaginal douche in addition to chemotherapy for moderate to severe vulvovaginitis, including those also suffering from insulin-dependent diabetes mellitus (IDDM). It is considered that topical benzydamine gave beneficial results compared with the controls receiving chemotherapy alone and had in the IDDM patients a useful prophylactic effect.

Immunoglobulin levels, T-cell markers, mitogen responsiveness and thymic hormone activity in Turner's syndrome.

Immunoglobulin levels (IgG, IgA, IgM), percentage and number of circulating B and T lymphocytes, mitogen responsiveness (PHA and Con A) and activity of thymus-dependent serum factor (SF) were estimated in 29 patients with Turner's syndrome (TS), ranging in age from 2.2/12 to 18.4/12 yr. IgG and IgM were decreased in 14 and 12, respectively, while IgA was very low in 1, and increased in 7. Peripheral blood B and T lymphocytes, in vitro responsiveness to PHA, and Con A, and SF activity were in the normal range. The present immunological study suggests that TS is often associated with low levels of serum IgG and IgM. The possible mechanism of these derangements, whether genetic or hormonal (low estrogen/androgen ratio), is discussed.

Factitious hypertension with mineralocorticoid excess in an infant.

A 2-month-old breast-fed baby is described, who, admitted for a prolongation of the QT interval on ECG, was found to be severely hypokalemic, alkalotic and hypertensive (blood pressure 200/100 mm Hg). Marked generalized hypotonia was present, and length was less than 3% for age. The results of endocrinological evaluation showed profound suppression of the pituitary-adrenal function and of the renin-aldosterone mechanism. CT scan, cavography and pyelography were normal. A pharmacological cause for the symptoms was sought and found: the mother had been using for the care of bruised nipples a cream containing 1% 9-alpha-fluoroprednisolone-21-acetate. Blood pressure remained elevated for 6 months, but became normal after one year, and growth has resumed normally. The possible differential diagnoses are considered, and the risks presented by topical steroids are emphasized.

Age-related variation in growth-promoting activity human plasma measured in human lymphocytes.

The age-related variations in the growth-promoting activity of human plasma have been studied from birth (cord blood) to adulthood using a bioassay which measures the incorporation of tritiated thymidine into lectin-activated human lymphocytes. Cord blood values were low (0.69 +/- 0.004 U/ml). A definite increase was found at 5 days of age, correlating with the level at birth. Higher levels were attained after 1 month of age, with a 2-fold increase during the first months of life. Lower values were found in children 1-10 yr old, and high levels were found during puberty. This pattern, different from those of sulfation activity and plasma somatomedins suggests that factors other than somatomedins may be involved in growth stimulation during the first year of life in humans.
